Perioperative factors limiting the discharge of patients treated with microvascular reconstruction after oral cancer resection.

BACKGROUND: Enhanced Recovery After Surgery (ERAS®) protocols reduce length of stay (LOS) in patients with head and neck cancer, but outcome … ffective pain management, no suspected infections, tracheostomy closure, and normal bowel function. Early ambulation and a selective, individualized approach to tracheostomy are suggested to enhance outcomes.

A Comparative Study of Spinal Anaesthesia With Hyperbaric Ropivacaine 0.75% and Hyperbaric Bupivacaine 0.5% in Hypertensive Patients Undergoing Lower Abdominal and Lower Limb Surgery.

BACKGROUND: Ropivacaine's stereoselective and lipophilic properties contribute to its cardiovascular stability and promote the early recovery of sensory and motor function, facilitating quicker ambulation and discharge. This study aimed to compare the hemodynamic stability measured by blood pressure and heart rate changes of intrathecal 0.75% ropivacaine versus 0.5% bupivacaine in patients with controlled hypertension undergoing spinal anaesthesia. METHODS: Eighty patients classified as American Society of Anesthesiologists (ASA) physical status II and III with controlled hypertension were ra

Use of Ultrasonically Interdigitated Anchors in Haglund's Deformity Correction Surgery.

Haglund's deformity is a common cause of foot pain, with high success rates of surgical intervention. Because early weightbearing rehabilitation correlates with better surgical results, the integrity of anchors employed in these surgeries needs to be evaluated for proper fixation strength, leading to improved postoperative outcomes. The ultrasonically interdigitated anchor is a biodegradable suture anchor that is melted into bony trabeculae using ultrasound, leaving a small biological footprint and less interference on computed tomography and magnetic resonance imaging scans, without loss of f

Maternal ambulation during labor.

PURPOSE OF REVIEW: To describe the recent advances in labor epidural analgesia, which may have an impact on maternal ambulation during labor. RECENT FINDINGS: With the advent of new epidural adjuvant drugs and new epidural delivery systems, we are now able to use very low concentration local anesthetic solutions with a reduction in the total doses of local anesthetic administered. This allows a much greater preservation of lower limb motor function in the parturient, with a subsequent positive effect on maternal ambulation. Although it is well documented that maternal satisfaction scores are

[Limb-girdle muscular dystrophy type 2M with adult-onset loss of ambulation. A case report].

We report a 29-year-old man with limb-girdle muscular dystrophy type 2M (LGMD2M) caused by a compound heterozygous mutation of 3-kb insertion in the 3'-untranslated region and c.1073A > C (p.Q358P) mutation in exon 9 in FKTN. He had been diagnosed since childhood as having Becker muscular dystrophy based on limb-girdle muscle weakness and calf muscle hypertrophy. Loss of ambulation occurred at age 26 years and cardiomyopathy was noted one year later. Muscle biopsy at age 29 revealed dystrophic changes with loss of immunoreactivity to alpha-dystroglycan (alpha-DG), which prompted us to analyze

Cast brace management of the femoral shaft fracture in children and young adults.

Seventy-two children and adolescents, 5-19 years of age, sustaining femoral fractures were treated with immediate cast bracing at Oklahoma C … n the uninjured femur at a 6-month follow-up. Immediate cast bracing can provide an opportunity for early ambulation and avoid the sequelae of immobilization. Weekly assessment for the first 4 weeks is mandatory to monitor length and angulation. The adolescent male with a midshaft fracture was most difficult to manage, and in this situation, closed intramedullary …

Evaluating the Efficacy of Dexmedetomidine as an Adjunct to Local Anesthetics in Adductor Canal Blocks for Adult Patients Undergoing Primary Total Knee Arthroplasty: A Systematic Review and Meta-Analysis.

Over 700,000 total knee arthroplasties (TKAs) are performed in the United States annually. Long-term functional recovery following TKA depends heavily on early ambulation and rehabilitation, which requires effective pain management. Adductor canal blocks (ACBs) offer targeted analgesia while preserving quadriceps strength. However, postoperative pain often outlasts single-shot peripheral nerve blocks. This analysis evaluated whether perineural dexmedetomidine added to local anesthetics increased the efficacy and duration of ACBs in patients undergoing TKA. This systematic review and meta-analy…

Chronic Arsenic Toxicity Without Classical Cutaneous Manifestations: A Case Report.

Chronic arsenic toxicity can present with gastrointestinal and neurological symptoms, and diagnosis is often difficult without characteristic cutaneous features. This report describes a 55-year-old woman with a three-month history of progressively worsening small bowel-type diarrhea that progressed to frequent episodes of melena. She developed painful stocking-distribution peripheral neuropathy with slipper slippage, followed by progressive lower-limb weakness that ultimately resulted in loss of ambulation. She had a seven-year history of unsupervised use of herbal preparations and Ayurvedic B

Prompt use of mechanical cardiopulmonary resuscitation in out-of-hospital cardiac arrest: the MECCA study report.

INTRODUCTION: Early use of mechanical cardiopulmonary resuscitation (CPR) for out-of-hospital cardiac arrest (OHCA) may improve survival outcomes. Current evidence for such devices uses outcomes from an intention-to-treat (ITT) perspective. We aimed to determine whether early use of mechanical CPR using a LUCAS 2 device results in better outcomes. METHODS: A prospective, randomised, multicentre study was conducted over one year with LUCAS 2 devices in 14 ambulances and manual CPR in 32 ambulances to manage OHCA. The primary outcome was return of spontaneous circulation (ROSC). Secondary outco

Association between thoracic ultrasound findings and respiratory function in children with Duchenne muscular dystrophy.

UNLABELLED: Duchenne muscular dystrophy (DMD) is characterized by progressive muscle weakness, atrophy, and respiratory failure. As the primary inspiratory muscle, the diaphragm plays a crucial role in the respiratory progression of DMD. Therefore, this study aimed to evaluate diaphragmatic function and structure in children with DMD. This study included 60 children with DMD who underwent diaphragmatic evaluation with B-mode thoracic ultrasonography (USG) and pulmonary functional testing. The same pediatric pulmonologist (P1) and pediatric radiologist (P2) performed all USG scans. The particip

Urologic Surgery with Multisystem Comorbidities: A Case Report.

BACKGROUND Originally implemented for colorectal surgery, enhanced perioperative protocols have been incorporated into many surgical fields in an effort to improve outcomes. The cornerstone of many strategies includes patient education, liberalized oral intake on the day of surgery, no routine bowel prep, targeted multimodal analgesia, cautious use of IV hydration, early extubation, avoidance of NG tubes and/or surgical drains, and encouraging early postoperative ambulation. CASE REPORT We report on the successful outcome of a single patient with a rare autosomal dominant disorder (hereditary

Evaluation of back pain caused by epidural neoplasm.

Back pain is a common symptom in the cancer population. For some patients, the complaint reflects a process independent of the underlying disease, such as myofascial pain, discogenic disease, or osteoporosis. Many patients, however, experience pain as the first indication of a spinal neoplasm, and this possibility increases the diagnostic challenge. This article discusses the presentation, evaluation, and treatment of spinal epidural neoplasm in patients with cancer.Symptomatic epidural neoplasm occurs in approximately 5 to 10% of patients with metastatic cancer.1-4 Without effective treatment

Response of the Australian Medical Services to restoration of mobile warfare on the Western Front in 1918 (part II).

On 4 July 1918, at the Battle of Hamel, the Australian Medical Services used a Field Ambulance Resuscitation Team for the first time, delivering life-saving blood transfusion and early definitive surgery to badly wounded soldiers very soon after their wounds had been inflicted. During the closing months of the war, many lives and limbs were saved by early resuscitation and effective surgery, an achievement that stands out in marked contrast to the situation in 1914, when inadequate resuscitation, outdated surgical methods and appalling delays in delivering treatment resulted in great numbers o
