Untreated posterior dislocation of the elbow in children.

Fifteen children with an untreated posterior dislocation of the elbow were seen between 1965 and 1980. Three had a useful range of painless flexion and were not treated by operation. Twelve had a stiff elbow and had an open reduction between three weeks and three years after injury. The triceps was lengthened when it prevented reduction and Kirschner wires were used when necessary to stabilize the elbow. Complications included a transient paralysis of the hand in one patient and myositis ossificans with a rigid elbow in another. The length of follow-up ranged from one to six years. In eleven p…

Unusual aspects of synovial chondromatosis of the elbow.

A 60-year-old man suffered locally from a rather aggressive chondromatosis of the elbow. Clinically the presentation was rather uncommon, with some radiological aspects (juxta-articular bone rarefaction) differing from those of ordinary chondromatosis. Neither the pathologist nor the radiologist recognized signs of malignancy. Regional osteoporosis, although rarely described in synovial chondromatosis, was considered as the diagnosis in this patient, but a low grade primitive chondrosarcoma must be sought in such cases.

Valgus extension overload in the pitching elbow.

Five baseball pitchers, three college and two professional, with an average age of 24 years, exhibited pain between the acceleration phase and follow-through phase of the pitching motion. This caused the players to be unable to continue at the level of competition necessary to play. A significant osteophyte on the posteromedial aspect of the olecranon process was identified in all pitchers. This caused impingement with the articular wall of the olecranon fossa and often created an area of chondromalacia. The more commonly identified posterior osteophyte was present in all cases. However, if ju

Elbow eruptions in nickel and chromate dermatitis.

In 7 patients with nickel or chromate allergy and dyshidrotic hand eczema, we observed symmetric hyperkeratotic lesions on the elbows which ran a similar course to the hand dermatitis. Histopathological examinations show findings suggestive of lichen simplex chronicus and subacute-chronic dermatitis, but exclude psoriasis. We conclude that these elbow lesions are a characteristic of systemic allergic contact dermatitis.

[Recurrent dislocation of the elbow (author's transl)].

The authors has observed a case of recurrent dislocation of the elbow in a child of 11. The lesion has been studied by arthrography and a good result has been obtained by surgical reconstitution of the capsule on the lateral aspect of the joint. Experiments have been made on cadavers which confirm the importance of the role of the lateral ligaments in this condition.

Ipsilateral elbow and forearm injuries in children.

Twenty-five children with combined elbow injury and forearm bone fractures have been reviewed. The incidence of serious complications was low, and only one patient was significantly handicapped. Open reduction and internal fixation of the supracondylar fracture with closed treatment of the forearm fracture gave the best results.

Our operative approach to the elbow joint.

Several surgical approaches to the elbow joint have been described. The disadvantage of all is an insufficient view of a single incision to the anterior and posterior as well as the ulnar aspect of the joint. They require therefore an additional incision or the resection of the radial head with a detachment of the radial collateral ligament. Our single incision allows for an extensive view to the anterior and posterior aspect of the elbow joint without resection of the radial head and the sacrificing of the radial collateral ligaments. It makes in most instances an additional ulnar incision un…

The operative treatment of elbow dislocation in the adult.

Operative treatment of traumatic dislocation of the elbow joint is indicated when there is proved gross instability or osteochondral fractures and loose bodies, or both. We performed a capsule-ligament repair in twenty-eight patients, and no cast was applied postoperatively. Satisfactory results were achieved with an average follow-up of two and one-half months. In all patients postoperative assisted range-of-motion exercise was begun after the operative wound and healed. The functional outcome depended on the osseous lesions involving the articular surface and on the time interval between the…

A comparison of axillary, elbow and Canadian crutches.

Crutches can be used by patients with injuries to the lower extremities in order to perform 'swing-through' gait. Because axillary crutches have been associated with axillary artery thrombosis and crutch palsy, they have to a large extent been superseded by elbow crutches. Canadian crutches combine many of the features of axillary and elbow crutches, and a study of the three types suggests that they have advantages in terms of energy cost.

Infective arthritis of the elbow joint in horses.

Six horses were found to have infective arthritis of one elbow joint. The history, and presence of a small wound on the lateral aspect of the elbow suggested the condition was trauma induced. Two horses recovered following joint lavage under general anaesthesia and antibiotic therapy. The joint changes found post mortem in the others were characteristic of an infective arthritis.

Recurrent ulnar-nerve dislocation at the elbow.

Recurring luxation of the ulnar nerve at the elbow is not uncommon (16.2%), occurring about equally in young and old, male and female, athletes and non-athletes but the greater mobility is usually at the dominant arm. The probable cause of such dislocation is congenital laxity of supporting ligaments. Being more vulnerable to injury than normally-positioned nerves, however, complicating neuritis can does occur. Subluxating nerves which stop on the tip of the medial humeral epicondyle upon 90 degrees or more of flexion at the elbow are more subject to direct trauma than completely displaced neu…

Pulled elbow: a study of 100 patients.

Pulled elbow is a common condition but may not be recognized by practitioners unaware of its existence. Most of a series of 100 children with this condition were aged 1 or 2 years, and none were over 6 years. An important precipitating factor was the normal recalcitrance of the 1-3 age group. The three children who were over 5 years had all had earlier episodes which may have stretched the annular ligament. The wrist was the most common site of pain after the elbow. A single manipulation was successful in 96 cases but multiple manipulations may be needed.

Elbow Flexor Release Combined With Selective Neurectomy of Musculocutaneous Nerve for Spastic Elbow Flexion Deformity in Children and Adolescents.

BACKGROUND: Spasticity of the elbow flexors causes a dynamic and/or static elbow flexion deformity. This position interferes with the functional use of the hand, is aesthetically unpleasant and can cause problems with hygiene and comfort. A lengthening procedure of elbow flexor muscles can improve elbow posture and range of motion. However, causal spasticity is not addressed directly. Selective neurectomy of the musculocutaneous nerve has a direct effect on underlying spasticity but is not sufficient when contracture is present. In this study, we examine the long-term results of a combined sur…
