Pilot report: non-operative treatment of Mayo Type II olecranon fractures in any-age adult patient.

BACKGROUND: We report on the non-operative treatment of Mayo Type II olecranon fractures. METHODS: Fourteen isolated Mayo Type II olecranon fractures were treated non-operatively, followed to discharge, and retrospectively reviewed. Treatment was splinting in extension followed by protected active motion beginning 3 weeks to 4 weeks post-injury. Mayo Elbow Performance Index (MEPI) and Shortened Disabilities of the Arm, Shoulder and Hand (QuickDASH) scores were available in 86% and 64% of cases, respectively. Follow-up radiographs were obtained. RESULTS: At discharge, the mean (SD) MEPI score

Clinic research on the treatment for humeral shaft fracture with minimal invasive plate osteosynthesis: a retrospective study of 128 cases.

INTRODUCTION: Minimal invasive plate osteosynthesis (MIPO) is one of the most important techniques in the treatment for humeral shaft fractures. This study was performed to evaluate the efficacy of MIPO technique for the treatment for humeral shaft fractures. MATERIALS AND METHODS: We retrospectively evaluated 128 cases with humeral shaft fractures that were treated with MIPO technique from March 2005 to August 2008. All the patients were followed up by routine radiological imaging and clinical examinations. Constant-Murley score and HSS elbow joint score were used to evaluate the treatment o

[Indication for Radial Head Resection in Traumatology].

PURPOSE OF THE STUDY The aim of our study was to determine the indications for radial head resection at the present day. MATERIAL AND METHODS The radial head resection was performed in the period from 2008 to 2015 in 63 patients divided into three groups. The first group marked "CR" consisted of 33 patients with the Mason type III fracture. The second group marked "CRLUX" included 20 patients with the Mason-Johnston type IV fracture, i.e. a fracture of the proximal end of the radius with a dislocation of the elbow joint. Within this group, in 8 cases also the coronoid process of the ulna was f

Evaluation and treatment of the elbow and forearm injuries in the emergency department.

Elbow and forearm injuries result most commonly from direct blows to the area, or from fall on outstretched hand. The elbow may be injured if it is locked at the time of impact. Elbow or forearm bone dislocations may occur alone or in conjunction with fractures and generally require reduction to minimize future morbidity. The primary goal of management is to achieve anatomic reduction of any fracture or dislocation, while allowing for early range of motion to minimize future morbidity, including in particular elbow stiffness and consequently limited mobility of the joint.

Negative Pressure Wound Therapy With Instillation and Dwell Time Used to Treat a Complex Open (Terrible Triad) Injury of the Elbow: A Case Report.

INTRODUCTION: Treating a complex elbow injury known as the terrible triad, combined with a severe soft tissue trauma, is challenging for most orthopedic surgeons and can lead to permanent disabilities and poor functional outcomes if reconstruction is inadequate. CASE REPORT: A 75-year-old male with a history of high blood pressure was injured in an accident involving agricultural equipment and presented with a triad injury of the left elbow: a posterolateral dislocation combined with fractures of the radial head (Mason-Johnson Type II) and ulnar coronoid process (Morrey Type 1). Coronoid Fractures.

UNLABELLED: Fractures of the coronoid process of the ulna are usually part of a more complex injury. The type of coronoid fracture corresponds with a specific pattern of injury with known pitfalls and recommended treatments. Tip fractures usually correspond with dislocation of the elbow and fracture of the radial head (the so-called terrible triad of the elbow because it is prone to redislocation, subluxation, and arthrosis) and are best repaired with a suture passed through drill holes along with repair or replacement of the fractured radial head and reattachment of the lateral collateral lig

Fractures of the capitellum--a comparison of two fixation methods.

Isolated capitellar fractures are rare, accounting for only 1 per cent of all elbow fractures (Bryan and Morrey, The Elbow and its Disorders, 1985). Many different fixation methods have been described but no series has compared these treatment modalities because of the rarity of these fractures. This paper compares the outcome of two types of fixation of type I capitellar fractures. Group one (n = 6) had open reduction and Kirschner wire fixation while group two (n = 6) had open reduction and Herbert screw fixation. Both groups were compared clinically, functionally and radiographically. We fo…

Radial Head Fractures Treated with Modular Metallic Radial Head Replacement: Outcomes at a Mean Follow-up of Eight Years.

BACKGROUND: Radial head arthroplasty is commonly used to treat acute unreconstructible radial head fractures. The purpose of this study was to report on the clinical and radiographic outcomes at a minimum follow-up of five years after radial head arthroplasty with a modular metallic implant for the treatment of acute radial head fractures. METHODS: The cases of fifty-five patients with unreconstructible radial head fractures treated acutely with a smooth-stemmed modular metallic radial head implant were retrospectively reviewed. A wide variety of injuries, which ranged from isolated radial he

Terrible triad injuries of the elbow: does the coronoid always need to be fixed?

BACKGROUND: The "terrible triad" of the elbow is a complex injury that can lead to pain, stiffness, and posttraumatic arthritis if not appropriately treated. The primary goal of surgery for these injuries is to restore stability of the joint sufficient to permit early motion. Although most reports recommend repair and/or replacement of all coronoid and radial head fractures when possible, a recent cadaveric study demonstrated that type II coronoid fractures are stable unless the radial head is removed and not replaced. QUESTIONS/PURPOSES: The purposes of this study were to determine the (1) r

Concurrent little leaguer's elbow and shoulder in a 15-year-old baseball pitcher and football quarterback.

Little leaguer's elbow and Little leaguer's shoulder are overuse pathologies seen in overhead-throwing athletes. No instance of simultaneously occurring pathologies has been published. A 15-year-old baseball pitcher and football quarterback developed pain in his throwing shoulder and elbow during spring baseball, which partially resolved with several months of rest. During fall football practice, he felt a pop and pain over his medial throwing elbow. Five days after the initial injury, medial elbow tenderness, mild swelling, and decreased range of motion were noted. Radiographs revealed a Salt

Management of Geriatric Elbow Injury.

Approximately 4.1% of all fractures in the elderly involve the elbow. Most elbow injuries in geriatric patients occur as the result of low-energy mechanisms such as falls from standing height. Elbow injuries in elderly patients present complex challenges because of insufficient bone quality, comminution, articular fragmentation, and preexisting conditions, such as arthritis. Medical comorbidities and baseline level of function must be heavily considered in surgical decision making.

Fractures of capitellum and trochlea.

The principles of management of capitellar and trochlear fractures are similar to any other intra-articular fracture. Accurate anatomic reduction, rigid internal fixation, and early mobilization of the elbow are prerequisites for a good functional result. Small fragments with minimal subchondral bone, not amenable to rigid internal fixation, are best treated by surgical resection. This article outlines the diagnosis, treatment, controversies, postoperative results, and complications of these rare elbow fractures.

Management of fractured scaphoid bone. A prospective study of 100 fractures.

One hundred fractured carpal scaphoids were immobilised alternatively in above-elbow and below-elbow casts. Preventing the pronation and supination of the forearm did not reduce the immobilisation time, which, with either type of cast, averaged 7 weeks, after exclusion of the fractures with delayed union. These eight fractures, which did not unite in 3 months, were operated on using a lag screw fixation, whereafter consolidation was achieved.

Retrospective analysis of 29 fractures of the olecranon process of the equine ulna.

Twenty-nine fractures of the olecranon process of the ulna in horses treated at The Ohio State University College of Veterinary Medicine from 1973 through 1983 were evaluated. The objectives of the study were to determine the prognosis for various configurations of fractures of the olecranon process of the ulna, to determine what preoperative factors influenced the prognosis, and to evaluate the efficacy of the treatment methods used. The fractures were placed in one of 4 categories on the basis of severity of the injury: type-1 fractures involved the physeal plate; type-2 fractures involved t

Skinny wire and locking plate fixation for comminuted intra-articular distal humerus fractures: a technical trick and case series.

INTRODUCTION: Intra-articular distal humerus fractures present a challenge to orthopedic surgeons. Stable fixation is difficult to achieve in fractures with articular and metaphyseal comminution and osteoporotic bone. Hence, these fractures are more commonly being managed with total elbow arthroplasty. We describe a novel surgical technique that confers stable fixation, allowing for early range of motion resulting in a high rate of union, a functional range of motion, and excellent patient reported outcome scores without the activity restrictions of total elbow arthroplasty. METHODS: Retrospe

Clinical Outcomes and Management Strategies for Capitellum and Trochlea Fractures: A Systematic Review.

PURPOSE: Capitellum and trochlea fractures, also referred to as coronal shear fractures of the distal humerus, are infrequent yet challenging intra-articular fractures of the elbow. There are a variety of surgical approaches and fixation methods with often variable outcomes. This systematic review investigates interventions, outcomes and complications of capitellum and trochlea fractures. METHODS: A systematic review of studies published in MEDLINE, EMBASE, Web of Science and Cumulative Index to Nursing and Allied Health literature (CINAHL) was conducted to assess the clinical outcomes of cap

Medial Condyle Fracture of the Distal Humerus in an Elderly Patient With Fishtail Deformity and Lateral Condyle Nonunion: A Case Report.

A medial condyle fracture of the humerus with preexisting fishtail deformity and lateral condyle nonunion is very rare, and there have been few reports describing favorable treatment options. We herein report the case of an 83-year-old woman who sustained a medial condyle fracture of her elbow with a comorbidity of long-lasting limited elbow motion with a history of elbow trauma in childhood. After conservative treatment with casting for four weeks, unstable medial condyle fracture in the presence of fishtail deformity and lateral condyle nonunion remained. Due to persistent pain, the patient
