Upper extremity injuries in the paediatric athlete.

Injuries to the upper extremity in paediatric and adolescent athletes are increasingly being seen with expanded participation and higher competitive levels of youth sports. Injury patterns are unique to the growing musculoskeletal system and specific to the demands of the involved sport. Shoulder injuries include sternoclavicular joint injury, clavicle fracture, acromioclavicular joint injury, osteolysis of the distal clavicle, little league shoulder, proximal humerus fracture, glenohumeral instability and rotator cuff injury. Elbow injuries include supracondylar fracture, lateral condyle frac

Secondary aneurysmal bone cyst in the distal humerus after resection of intra-articular nodular fasciitis of the elbow.

BACKGROUND: Nodular fasciitis most often occurs within subcutaneous tissues, but may also arise within skeletal muscle, dermis, vessels, peripheral nerves and, although rarely, within joints. Knowledge regarding the cause of aneurysmal bone cysts, its natural history, and the results of treatment is limited. Secondary aneurysmal bone cysts are associated with other neoplastic processes. Intra-articular nodular fasciitis in the elbow joint has not been reported previously, nor has the development of aneurysmal bone cyst secondary to intra-articular nodular fasciitis in the elbow joint. CASE PR…

Virtual Coaching and the Reduction of Radial Head Subluxation: A Case Series.

OBJECTIVES: After the establishment of the virtual pediatric emergency medicine clinic at our institution, we noted that several physicians independently began to instruct caregivers virtually on reducing a radial head subluxation. We thus conducted a case series to investigate the number, success, and follow-ups for the virtual reduction of radial head subluxation. METHODS: The electronic medical records at our institution were searched from the inception of the virtual clinic in May 2020 until August 2022 (inclusive), for visits and discharge diagnosis containing the word "elbow" or "arm."

Essex-Lopresti Lesions and Longitudinal Radioulnar Instability: A Narrative Review.

»: An Essex-Lopresti injury (ELI) is classically described as a radial head fracture with a concomitant interosseous ligament complex (IOC) injury. However, multiple injury patterns may be present, and an interosseous membrane (IOM) injury should be evaluated for in any axial load injury through the forearm. ELI may result in longitudinal radioulnar instability (LRUI). »: Diagnosis of an IOC injury can be difficult. Evaluation begins with standard wrist and elbow radiographs and is supplemented with radiographs and intraoperative maneuvers to assess for proximal radial migration. Magnetic res

Sequelae of childhood elbow fracture.

Elbow fracture is frequent in children, and often requires surgery. There are many potential sequelae: neurovascular, ligamentous and osseous. Some are liable to be overlooked, due to young age and the cartilaginous nature of the joint. There is little bone remodeling in the elbow and displacement fracture, and especially supracondylar or lateral condyle fracture, has to be corrected rigorously, notably in older children. In case of lateral tilt in fracture of the neck of the radial head, on the other hand, remodeling is effective. Deformity in cubitus varus and cubitus valgus leads to neurolo

An Unusual Case Of Ipsilateral Shoulder And Elbow Dislocation.

A 34 years old male, labourer by occupation was brought to our trauma centre in an inebriated state with a history of fall from a height of approximately 10 feet. He complained of pain in the right elbow. A complete examination of the affected upper limb revealed an ipsilateral anterior shoulder dislocation and a posterior elbow dislocation with no other concomitant injuries. Both the dislocations were reduced conservatively and the patient was successfully managed. The patient is absolutely fine now, 6 months post his dislocation and has resumed back to his work. This case has been presented

[Isolated elbow dislocation in pediatric patients: non-operative treatment and complications associated with an infrequent pathology. Series of 4 cases].

Even though the elbow is the most often dislocated joint in children, this injury accounts for 3-6 % of elbow pathology. Dislocations without associated fractures are extremely rare. They result from a fall onto an outstretched hand. The patient is always referred with a painful joint, movement impairment and even clinical deformity. Acute treatment aims to achieve quick reduction and adequate joint stability, avoiding neurovascular injuries. We sought to analyze the functional outcomes and the complications after non-operative treatment. Our 4 patients Luxaciones puras de codo en pacientes pe

Injury patterns following simple elbow dislocation: radiological analysis implies existence of a pure valgus dislocation mechanism.

INTRODUCTION: The aim of the present study was to analyze the injury pattern and thus the dislocation mechanism after simple elbow dislocation using radiographs and magnetic resonance imaging (MRI) data sets. MATERIALS AND METHODS: The MRI data sets of 64 patients with a mean age of 44 years (18-77 years) were analyzed retrospectively. The inclusion criteria for the study were (1) radiograph with confirmed simple elbow dislocation, (2) low-energy trauma, (3) MRI of the affected elbow ≤ 3 weeks after trauma. The dislocation direction was determined using radiographs. The integrity of the later

Surgical treatment of post-traumatic elbow stiffness: a systematic review.

BACKGROUND: Trauma to the elbow is a common cause of joint stiffness that might require surgical release. Release of the stiff elbow can be done by open or arthroscopic approach. There is no high-level evidence for the best surgical treatment modality for post-traumatic elbow stiffness. The objective of this article was to review current available literature of studies reporting on open or arthroscopic release of post-traumatic elbow stiffness. METHODS: A comprehensive literature search was performed. All titles and abstracts of potentially relevant studies were reviewed, with a set of predef

Elbow arthroscopy: setup, portal placement, and simple procedures.

Elbow arthroscopy has become an accepted treatment for numerous elbow conditions, including loose bodies, lateral epicondylitis, contractures, painful osteophytes, synovitis, osteochondritis dissecans, synovial plica, and osteoarthritis. It is absolutely necessary that the treating surgeon have complete knowledge of elbow anatomy. Three options exist for patient positioning: supine, prone, and lateral decubitus. Standard arthroscopic probes, grasping forceps, punches, and motorized shavers and burrs are used in the procedure. Retractors are essential for visualizing, exposing, and protecting n

[Isolated traumatic luxation of the radial head in adults: report of a case and review of the literature].

One case of isolated traumatic dislocation of the radial head in a 59-years-old female secondary to an injury in pronation and extension of the non dominant left elbow is reported. Treatment consisted of immediate closed reduction followed by immobilisation in a plaster cast with the elbow flexed at 110 degrees, for 10 days. An MRI scan 2 months after the injury showed the lesion of annular ligament which did not have any clinical consequences. At 5 years follow-up, functional outcome is excellent compared to the normal contralateral elbow with no difference between both sides. Only 20 cases o

Concurrent perilunate dislocation in patients with elbow dislocation: case reports.

In three patients with elbow dislocations after falls on an outstretched upper extremity, concurrent perilunate dislocation was found. Closed reduction of the elbow dislocation achieved excellent functional results. Open reduction and internal fixation for perilunate dislocation provided anatomic reduction and satisfactory functional recovery in two patients. In the third, a delay in diagnosis of a perilunate dislocation necessitated proximal row carpectomy at 6 weeks.

Radial Head Arthroplasty among Patients with Radial Head Fracture in a Tertiary Care Centre: A Descriptive Cross-sectional Study.

INTRODUCTION: Proximal radial fractures are a common type of fracture around the elbow joint. Among these comminuted radial head fractures are commonly associated with secondary injuries and instability of the elbow joint. Management of the radial head in such cases is very important in restoring the stability of the elbow joint and starting early mobilization. The aim of this study was to find out the prevalence of radial head arthroplasty among patients with radial head fracture in a tertiary care centre. METHODS: A descriptive cross-sectional study was conducted from 25 January 2019 to 25 …

Editorial Commentary: For Surgical Treatment of Elbow Lateral Epicondylitis, Arthroscopic Treatment Is Recommended for Surgeons With Elbow Arthroscopy Training.

Lateral epicondylitis is a commonly encountered and persistent problem in the active, middle-aged population, with a reported annual incidence as high as 3.4%. Lateral epicondylitis is often treated successfully with conservative measures but may necessitate surgical intervention in refractory cases. Most of the review literature has failed to definitively identify arthroscopic or open debridement as the superior surgical approach. We favor the arthroscopic approach because it allows for the examination and treatment of concomitant intra-articular pathologies, which may be underappreciated on

Current concepts: pathology in the overhead athlete's lateral elbow.

The lateral elbow is subjected to increasing compressive force in response to repetitive valgus stress. Alterations or deficiencies in overhead mechanics and the kinetic chain may predispose an athlete to injury. Evaluation includes a focused elbow physical examination and imaging, supplemented by a robust screening of core strength, balance, and mechanics of the kinetic chain. Treatment of osteochondritis dissecans, a common lateral elbow pathology, varies based on stability of the lesion, with variable, but often positive outcomes. Proper pitching mechanics, kinetic chain integrity, and work

Elbow Trauma: An Evidence-Based Approach to Improving Outcomes.

The management of elbow fractures remains difficult and controversial. The failure rate of surgical intervention in elbow fractures remains higher than that seen with other fractures, and there remains significant room for improvement in the care of these injuries. Evidence-based management strategies for elbow fractures and how to prevent and manage complications following elbow fracture surgery have been described.
