Comparison of compound action potential audiograms with distortion product otoacoustic emissions in experimentally induced hydrops.

Endolymphatic hydrops was induced in a series of adult pigmented guinea pigs. Distortion product oto-acoustic emissions in general reflected the typical form of the hydropic compound action potential (CAP) audiogram, either with predominant low-frequency losses or a relatively flat audiogram. However, at certain two-tone combinations in some control ears the emissions could be smaller than expected from respective CAP audiograms, while in some hydropic ears the emissions could be larger than expected. The data presented here suggest that whilst the diagnostic value of distortion product emissi…

Medical devices; ear, nose and throat devices; reclassification of the endolymphatic shunt tube with valve. Final rule.

The Food and Drug Administration (FDA) is reclassifying the endolymphatic shunt tube with valve from class III (premarket approval) into class II (special controls). The device is intended to be implanted in the inner ear to relieve the symptoms of vertigo and hearing loss due to endolymphatic hydrops (increase in endolymphatic fluid) of Meniere's disease. FDA is also identifying the guidance document entitled "Class II Special Controls Guidance Document: Endolymphatic Shunt Tube With Valve; Guidance for Industry and FDA" (the guidance) as the special control that the agency believes will reas…

Endolymphatic space size in patients with vestibular migraine and Ménière's disease.

Ménière's disease (MD) is characterized by episodic vertigo, fluctuating hearing loss and tinnitus. Vestibular migraine (VM) is a relatively new disorder that is characterized by episodic vertigo or dizziness, coexisting migraine and absence of hearing loss. It is occasionally difficult to distinguish between VM and vestibular MD with headache. Because endolymphatic hydrops (EH) is a characteristic sign of MD, we attempted to evaluate endolymphatic space size in both diseases. Endolymphatic space size in the vestibule and the cochlea was evaluated in seven patients with VM and in seven age- an…

The vestibular epithelia in experimental hydrops.

Morpho-pathological features, observed by scanning electron microscopy, in guinea pigs with endolymphatic hydrops of 4-14 months included shortening of the hair cell tufts, loss of tufts, retraction of sensory hair cells away from the surrounding tissue and hair cell loss. After 22 months of hydrops, there was complete loss of hair cells with indifferentiation of the epithelium. The loss of ciliary tufts involved loss of both stereocilia and kinocilia identified as short stubs and holes respectively. Control macular epithelia showed no hair tuft loss although ampullae in control ears could sho…

Clinical usefulness of glycerol vestibular-evoked myogenic potentials: preliminary report.

The detection of intense sound-induced vestibular-evoked myogenic potentials (VEMPs) on the sternocleidomastoid muscle comprises the basis of the saccular function test. In order to evaluate the endolymphatic hydrops (EH) of the saccule of the inner ear, a glycerol VEMP (GVEMP) test was performed in 15 patients with unilateral typical Ménière's disease (UMD) and 7 with delayed endolymphatic hydrops (DEH). Using the GVEMP test, 8 of the 15 patients (53%) with UMD were evaluated as being abnormal. In addition, a greater number of patients (67%) were judged to be abnormal when the results of the …

Ocular vestibular evoked myogenic potential frequency tuning in certain Menière's disease.

Ocular vestibular evoked myogenic potentials (oVEMP) represent extraocular muscle activity in response to vestibular stimulation. To specify the value of oVEMP in the diagnostics of Menière's disease, the amplitude ratio between 500 and 1000 Hz stimuli was investigated. Thirty-nine patients with certain Menière's disease, i.e. definite Menière's disease with visualization of endolymphatic hydrops by magnetic resonance imaging and 19 age-matched healthy controls were enrolled in this study. oVEMP were recorded using 500 and 1000 Hz air-conducted tone bursts. For Menière's ears, the 500/1000 Hz …

Structural Analysis of Tensor Tympani Muscle, Tympanic Diaphragm, Epitympanum, and Protympanum in Menière's Disease: a Human Temporal Bone Study.

HYPOTHESIS: We hypothesized that there would be significant anatomic differences of the tensor tympani muscle (TTM), tympanic diaphragm, epitympanum, and protympanum in patients with versus without Menière's disease. BACKGROUND: The effects of tenotomy on Menière's disease suggested it relieves the pressure on the inner ear of the contraction of the TTM and of negative middle ear pressure. METHODS: Using human temporal bones from patients with Menière's disease, two studies were conducted. We examined the presence of otitis media, cholesteatoma, and endolymphatic hydrops, the length, diamete…

[The current problems related to Meniere's disease].

Over 150 years have elapsed since Prosper Meniere's described for the first time the clinical symptoms of the disease that is called by his name at the present time. Nevertheless, the etiology of this pathological condition remains unknown. It has been believed during many decades that the cause of Meniere's disease Is the presence of an excess fluid (hydrops) in the endolymphatic space of the inner ear leading to the rupture of membranes responsible for hearing impairment and vestibular disorders. The recent histopathological studies have demonstrated that endolymphatic hydrops (RH) is not ne…

Obstructing lesions of the endolymphatic sac and duct mimicking Ménière's disease.

In this retrospective case series and literature review, we demonstrate that temporal bone lesions that obstruct the endolymphatic sac or duct can cause symptoms of Ménière's disease. This finding is likely attributable to endolymphatic hydrops; initially, such cases typically masquerade as Ménière's disease. Between July 1995 and April 2002, a total of 379 patients were treated for an initial diagnosis of Ménière's disease at our institution. Among this group, 3 patients were found to have an obstructing lesion of the endolymphatic sac or duct that we felt was causally related to their Ménièr…

Hypothesis: could Meniere's disease be a channelopathy?

Meniere's disease is a clinical syndrome of uncertain aetiology but it is a widespread belief that it is related to endolymphatic hydrops. Clinically, it is a paroxysmal disorder with vertigo and subsequent deafness. It is responsive to acetazolamide and sensitive to the sodium content in the diet, many of the features of the channelopathies. The present paper explores the possibility that it may be related to a channelopathy.

Tinnitus in Normal-Hearing Participants after Exposure to Intense Low-Frequency Sound and in Ménière's Disease Patients.

Tinnitus is one of the three classical symptoms of Ménière's disease (MD), an inner ear disease that is often accompanied by endolymphatic hydrops. Previous studies indicate that tinnitus in MD patients is dominated by low frequencies, whereas tinnitus in non-hydropic pathologies is typically higher in frequency. Tinnitus of rather low-frequency (LF) quality was also reported to occur for about 90 s in normal-hearing participants after presentation of intense, LF sound (120 dB SPL, 30 Hz, 90 s). LF sound has been demonstrated to also cause temporary endolymphatic hydrops in animal models. Here…

Vascularity of endolymphatic sac in Meniere's disease. A histopathological study.

The vascularity of the endolymphatic sac (ES) in the vestibular aqueduct (VA) and of periaqueductal bony channels was studied in 18 temporal bones from patients with Meniere's disease and in the same number of control bones with no endolymphatic hydrops and no premortem history of otologic disease. The control bones were selected on the basis of having VAs of the same sizes as the hydropic bones. This was important in order to eliminate any possibility that differences in degree of vascularity of the ES could have been due simply to differences in the degree of development of the VA. All speci…

Prevention of complications and failures in endolymphatic system surgery.

Endolymphatic system surgery for Meniere's disease has been shown to be effective in selected cases. However, substantial long term relief of symptoms may not be achieved in some patients in spite of early success and revision surgery may be required. Revision endolymphatic system surgery has been reported as being required in 9.7 to 16.3 per cent of the patients. As part of an ongoing prospective analysis of endolymphatic valve implant surgery, the first 400 procedures we performed over six years were analyzed to determine the incidence and reasons for revision surgery, as well as the inciden

Ménière's disease associated with serous retinopathy.

A case history is presented of a patient suffering from classical Ménière's disease who concurrently developed central serous retinopathy. On review, these conditions are found to have several features in common and on the basis of the changes seen by fluorescein angiography in the eye, an hypothesis is presented for the development of endolymphatic hydrops.

Claudin expression in the rat endolymphatic duct and sac - first insights into regulation of the paracellular barrier by vasopressin.

Hearing and balance functions of the inner ear rely on the homeostasis of the endolymphatic fluid. When disturbed, pathologic endolymphatic hydrops evolves as observed in Menière's disease. The molecular basis of inner ear fluid regulation across the endolymphatic epithelium is largely unknown. In this study we identified the specific expression of the tight junction (TJ) molecules Claudin 3, 4, 6, 7, 8, 10, and 16 in epithelial preparations of the rat inner ear endolymphatic duct (ED) and endolymphatic sac (ES) by high-throughput qPCR and immunofluorescence confocal microscopy. Further we sho…

[The effectiveness of transtympanic use of dexamethasone 4 mg in Meniere's disease, severe and moderate course of the disease].

The results of transtympanic administration of 4 mg dexamethasone in 37 patients with moderate and severe Meniere's disease are presented. The endolymphatic hydrops was detected in all inner ear structures with predominant accumulation in vestibule and semicircular canals by MRI-visualization before therapy. After 1 month of 4 mg dexamethasone transtympanic therapy the hydrops degree decreased. Daily transtympanic administration of 4 mg dexamethasone for a month leads to reduction of vertigo attacks duration and expression in 92% of cases (follow-up period 24 months) and can be considered when…
