Meta-analysis of sero-epidemiologic studies of the relation between Chlamydia pneumoniae and atherosclerosis: does study design influence results?

BACKGROUND: The results of sero-epidemiologic studies of the relation between Chlamydia pneumoniae and atherogenesis vary considerably. A meta-analysis was performed to calculate the overall odds ratio (OR) and to identify potential sources of variability. METHODS: The MEDLINE database was searched from January 1997 to December 2000 for studies describing the seroprevalence of IgG antibodies to C pneumoniae in relation to clinically manifest atherosclerosis. Data extracted included 1) the study design used, 2) the number of included patients, 3) the type of disease (or end point), 4) the numb

A robust and efficient method for Mendelian randomization with hundreds of genetic variants.

Mendelian randomization (MR) is an epidemiological technique that uses genetic variants to distinguish correlation from causation in observational data. The reliability of a MR investigation depends on the validity of the genetic variants as instrumental variables (IVs). We develop the contamination mixture method, a method for MR with two modalities. First, it identifies groups of genetic variants with similar causal estimates, which may represent distinct mechanisms by which the risk factor influences the outcome. Second, it performs MR robustly and efficiently in the presence of invalid IVs

Mannose-binding lectin deficiency and disease severity in non-cystic fibrosis bronchiectasis: a prospective study.

BACKGROUND: Mannose-binding lectin (MBL) is a key component of innate immunity. MBL deficiency is common (10-30% of the general population depending on the definition used) and has been associated with disease progression in cystic fibrosis. We aimed to assess the effect of MBL deficiency on disease severity in non-cystic fibrosis bronchiectasis. METHODS: We recruited patients with non-cystic fibrosis bronchiectasis and age-matched and sex-matched controls at a specialist bronchiectasis clinic in Edinburgh, UK. We assessed MBL function with genotyping (low-expressing genotype [deficiency] def

Design and baseline characteristics of a prospective cohort study for determinants of osteoporotic fracture in community-dwelling elderly Japanese men: the Fujiwara-kyo osteoporosis risk in men (FORMEN) study.

BACKGROUND: Osteoporosis and osteoporotic fracture in men are significant public health problems in an aging society. However, information on male osteoporosis remains impressively lacking, especially for Asians. We designed the Fujiwara-kyo Osteoporosis Risk in Men (FORMEN) study as an ancillary study of a cohort study, the Fujiwara-kyo study, to determine risk factors for osteoporotic fractures in Japanese men. DESIGN: A community-based single-centre prospective cohort study with at least a 5-year follow-up. SUBJECTS: All the male participants of the Fujiwara-kyo study who were living in t

Comparison of single- and double-stage designs in the prevalence estimation of eating disorders in community samples.

The aim of this research was to compare two different case-identification designs: (a) a one-stage anonymous design using the Eating Disorders Examination-Questionnaire (EDE-Q; Fairburn & Beglin, 1994) as diagnostic instrument and (b) a two-stage-non-anonymous design using the Eating Attitudes Test (EAT; Garner & Garfinkel, 1979) and the EDE-Q as screening instruments and the clinical interview Eating Disorders Examination (EDE; Fairbumrn & Cooper, 1993) as diagnostic instrument, in the estimation of eating disorders prevalence in community samples. Both epidemiological designs were compared i

Impact of the Fogarty Training Program on Trainee and Institutional Research Capacity Building at a Government Medical College in India.

BACKGROUND: Strengthening health research is essential to inform public health policies. However, few research training programs have systematically measured their impact on capacity building and most evaluations have been limited to reporting of individual trainee metrics. Hence, we conducted an evaluation of the impact of a five-year training program focused on building both trainee and institutional research capacity at a public medical college in India. METHODS: Quantitative and qualitative methods were used to assess the individual and institutional research capacity building of a five-y

Rabies research in Ethiopia: A systematic review.

Rabies is an important zoonosis in Ethiopia, where lack of research is cited as a constraint to implementation of the national rabies control strategy. We conducted a systematic review of publications and theses on rabies in Ethiopia, to document research gaps and areas of knowledge saturation in relation to geographic and species focus, methods and findings. We also examined funding sources and extent of local researcher participation. After screening titles and abstracts, the full text of 119 publications was included in data extraction. More than 40% of publications involved data collection

Anthropological perspectives on injections: a review.

Qualitative studies from developing countries have pointed to the widespread popularity of injections. In addition to their use by formal and informal providers and traditional healers, there is now increasing evidence of the use of injections and injection equipment by lay people. Epidemiological research links the large number of unsafe injections to serious bloodborne infections such as viral hepatitis B and C and acquired immunodeficiency syndrome (AIDS). The present article examines the reasons behind the demand for injections by consumers and the administration of unnecessary or unsafe i

Using Administrative Data to Better Treat Chronic Limb Threatening Ischemia.

Chronic limb threatening ischemia (CLTI) is the most severe manifestation of peripheral arterial disease and represents a particularly high-risk subgroup of patients. As such, efforts to better understand this complex patient population through well-designed clinical research studies are critical to improving CLTI care. Prospective randomized clinical trials (RCTs) remain the gold standard in clinical research, but these trials are resource-intensive and have highly selective patient populations, which limit their feasibility and generalizability. Alternatively, retrospective studies are less

Advancing tinnitus research and researcher training: A case study review and future perspectives.

Tinnitus affects one in 10 people and for many significantly impairs quality of life. Whilst there has been a notable increase in tinnitus research funding and tinnitus-related scientific publications in recent years, this is not keeping pace with other disciplines such as hearing loss. Tinnitus research is less attractive to industry funders, in particular because the field lacks consensus on many topics, objective measures, and treatment success stories. There is a clear need for even more funding, better quality studies, increased capacity, and more interdisciplinary working in the field. O

Cardiovascular Health Risks of Organophosphate Flame Retardant Exposure: A Narrative Review of the Available Evidence.

Organophosphate ester flame retardants (OPFRs) have emerged as ubiquitous environmental contaminants following regulatory restrictions on brominated flame retardants. Despite widespread human exposure across global populations, cardiovascular health implications remain incompletely characterized. This Review summarizes current evidence regarding OPFR exposure and cardiovascular health risks, identifying biological mechanisms and vulnerable populations requiring enhanced protection. A systematic literature search in PubMed (January 2015-June 2025) identified 108 relevant publications encompassi

The link between chronic periodontitis and COPD: a common role for the neutrophil?

BACKGROUND: The possible relationship between chronic inflammatory diseases and their co-morbidities has become an increasing focus of research. Both chronic periodontitis and chronic obstructive pulmonary disease are neutrophilic, inflammatory conditions characterized by the loss of local connective tissue. Evidence suggests an association and perhaps a causal link between the two diseases. However, the nature of any relationship between them is unclear, but if pathophysiologically established may have wide-reaching implications for targeted treatments to improve outcomes and prognosis. An international research agenda for clozapine-resistant schizophrenia.

Treatment-resistant symptoms occur in about a third of patients with schizophrenia and are associated with a substantial reduction in their quality of life. The development of new treatment options for clozapine-resistant schizophrenia constitutes a crucial, unmet need in psychiatry. Additionally, an overview of past and possible future research avenues to optimise the early detection, diagnosis, and management of clozapine-resistant schizophrenia is unavailable. In this Health Policy, we discuss the ongoing challenges associated with clozapine-resistant schizophrenia faced by patients and hea

Psychological stress and type 1 diabetes mellitus: what is the link?

Type 1 diabetes mellitus (T1DM) is a chronic disease characterized by the destruction of insulin-producing β-cells of the pancreas. The current paradigm in this disease's etiopathogenesis points toward the interplay of genetic and environmental factors. Among the environmental variables, dietary factors, intestinal microbiota, toxins, and psychological stress have been implicated in disease onset. Areas covered: This review aims to investigate the relationship between psychological stress and T1DM by presenting evidence from epidemiological studies, animal models, and to provide the mechanism

Nationwide monitoring of end-of-life care via the Sentinel Network of General Practitioners in Belgium: the research protocol of the SENTI-MELC study.

BACKGROUND: End-of-life care has become an issue of great clinical and public health concern. From analyses of official death certificates, we have societal knowledge on how many people die, at what age, where and from what causes. However, we know little about how people are dying. There is a lack of population-based and nationwide data that evaluate and monitor the circumstances of death and the care received in the final months of life. The present study was designed to describe the places of end-of-life care and care transitions, the caregivers involved in patient care and the actual treat
