Isolated uvulitis: An uncommon but not a rare clinical entity.

Infections of uvula have been described in association with group A streptococcal pharyngitis (Rapkin, JAMI, 43, 1980, 1843), or Haemolytic influenzae type b epiglottitis. Medicine (Gorjinkel HJ, 58, 1979, 80) however, to our knowledge, only two cases of isolated uvulitis are reported in world literature. We report five cases of isolated uvulitis in adults.

Surgical treatment of severe laryngomalacia: Six month follow up.

BACKGROUND: Laryngomalacia (LM) is the most common congenital anomaly of larynx that causes stridor in children. We evaluated the efficacy of epiglottic suture and laser epiglottopexy for treatment of infants with severe LM. METHODS: Surgical intervention was performed in 19 patients with severe LM, after the diagnosis was established using flexible laryngotracheobronchoscopy. Five patients had isolated type 1 LM, and 14 patients had a combination of type 1 and 3 LM. Indication for surgical treatment was the presence of LM with at least one of the following: malnutrition (body mass index [BMI

Pathophysiology of dorsal displacement of the soft palate in horses.

Dorsal displacement of the soft palate (DDSP) is an intermittent obstructive upper airway condition that occurs in athletic horses during high-intensity exercise. The pathogenesis of this condition is unknown, but may involve epiglottic hypoplasia, malformation, or neuromuscular dysfunction. In this paper, we report on investigations into the pathophysiology of DDSP. In 3 separate experiments, Standardbred horses were exercised on a high-speed treadmill at speeds corresponding to 50, 75 and 100% of maximum heart rate. The upper airway was evaluated by videoendoscopy and measurement of tracheal

The periepiglottic space: topographic relations and histological organisation.

Important aspects of histological organisation and topographic relations of the pre-epiglottic space are not fully understood. This region was therefore reinvestigated in plastinated serial sections of 19 human adult specimens. The cranial part of the pre-epiglottic space is homogenously filled with adipose tissue and extends around the epiglottis in a horseshoe fashion. Therefore, the term periepiglottic space (PES) is a more accurate description of this region. The cranial border of the PES is constituted by the hyoepiglottic membrane, which extends between the epiglottis and the tongue, and

[Vertical partial laryngectomy and its reconstruction methods].

In the past ten years, we performed vertical partial laryngectomy in 25 patients with glottic carcinomas. These included two T1 cases, seven T2 cases, six T3 cases, and ten post-irradiation recurrent cases. Three types of reconstructive surgery were performed in 24 patients which included 14 patients reconstructed by superior based sternohyoid muscle flap covered with A-E fold or free oral mucosa flap, 7 patients by epiglottic flap, and 3 patients by free fat graft. Most patients healed well post-operatively. Only two post-irradiated patients suffered from fistula due to poor healing, one of w

Isolated uvulitis due to Haemophilus influenzae type b.

Infections of the uvula are infrequently recognized and have been previously described only in association with group A streptococcal pharyngitis or Haemophilus influenzae type b epiglottitis. Three cases of H influenzae type b bacteremic uvulitis are described. In suspected cases of H influenzae type b uvulitis, a lateral neck radiograph should be performed and parenteral antibiotics initiated.

Respiratory emergencies in infants and young children.

Emergencies involving the respiratory tract are not uncommonly encountered in infants and young children. The spectrum of disorders causing respiratory distress varies from self-limited disorders such as bronchiolitis to potentially life-threatening conditions such as acute epiglottitis. This article outlines the clinical manifestations of some of the common respiratory emergencies presenting in infants (beyond the neonatal period) and young children, as well as strategies for management.

[Special infections of the respiratory tract in children].

Inspiratory or exspiratory stridor associated with dyspnea represents a common pediatric symptomatology compatible with various diseases which usually are of infectious etiology. The causative obstruction of the infantile airways may lead to life-threatening respiratory failure. A review is conducted of the most important emergencies characterized by stridor and dyspnea in early childhood: croup syndromes (acute viral laryngotracheitis, spasmodic croup, bacterial tracheitis), epiglottitis and bronchiolitis.

Coblation-assisted lingual tonsillectomy for dysphagia secondary to tongue base hypertrophy.

OBJECTIVES: Lingual tonsillar hypertrophy is an underappreciated cause of dysphagia and is believed to impede swallowing function by inhibition of laryngeal elevation and epiglottic inversion due to mechanical interference by bulky tongue base tissue. We present a case of severe dysphagia secondary to idiopathic tongue base hypertrophy that was treated with coblation lingual tonsillectomy and tongue base reduction. METHODS: We report a case and discuss the relevant literature regarding tongue base hypertrophy and surgical interventions to treat the enlarged base of the tongue. RESULTS: Sympt

A peculiar site of chondroma: the epiglottis.

Chondroma of the laryngeal cartilage is an uncommon benign cartilaginous neoplasm. The commonest location is the posterior lamina of the cricoid cartilage, followed by the thyroid cartilage. The occurrence of chondroma in the epiglottis is extremely rare. Depending on the size and location of epiglottic chondroma the clinical manifestations are variable and include a lump in the neck, difficulty in swallowing fluids and dyspnea. In this study, an additional rare case of chondroma arising in the epiglottis is reported. The mass was located over the tip of the epiglottis and caused no significan

Anatomical adaptation of a custom 3D prosthesis for canine laryngeal stenosis: a preliminary cadaveric study.

IMPORTANCE: Laryngeal stenosis is a frequent and life-threatening cause of upper airway obstruction in dogs. Current surgical options, although effective, are invasive and associated with high morbidity. Species-specific laryngeal stents may provide a safer alternative. OBJECTIVE: To evaluate the anatomical adaptation and feasibility of a novel custom three-dimensional (3D)-printed laryngeal prosthesis designed for canine laryngeal stenosis in a cadaveric model. METHODS: A prototype stent was produced using 3D printing and assessed in eight canine cadavers of different breeds and skull confo

Gastric H+/K+-ATPase Expression in Normal Laryngeal Tissue and Laryngeal Carcinoma.

BACKGROUND: Several studies have suggested that laryngopharyngeal reflux disease (LPRD) or gastroesophageal reflux disease (GERD) is an independent risk factor for laryngeal carcinoma. However, it remains unclear whether either condition affects the level of H+/K+-ATPase expression in laryngeal carcinoma. MATERIALS AND METHODS: Immunohistochemistry, real-time RT-PCR, and Western blotting were used to explore the distributions of proton pump (H+/K+-ATPase) α- and β-subunits in normal laryngeal tissue and laryngeal carcinoma. RESULTS: Messenger RNAs encoding both the α- and β-subunits were fou

An unusual cause of stridor-a case report.

Slridor in children is usually acute in onset and most commonly caused by infections such as acute laryngitis, laryngotracheobronchitis, and acute epiglottitis. Stridor due to foreign body is also not uncommon. The case reported is that of a large upper thoracic pre and paravertebral abscess presenting with stridor. Although a rare cause, it presents as a life threatening entity requiring urgent treatment.

Complications in the use of prochlorperazine.

This case presentation is of a patient who had the clinical appearance of epiglottitis, but actually had an oro-pharyngeal dystonic reaction to prochlorperazine. The intent of the discussion is to alert physicians that the appearance of epiglottis can occur from causes other than infection and that a surgical airway should not be the first thought when such a case arises.

[Serious streptococcal infections of the head and neck region].

This article describes the four cases of serious infections caused by Streptococcus pyogenes in the head and neck. Three patients presented with invasive infections of the soft tissues in the neck and one with epiglottitis. Two patients fulfilled the criteria for toxic streptococcal shock syndrome. The incidence of severe streptococcal disease is increasing in Norway, which emphasizes the importance of an aggressive attitude when diagnosing and treating atypical throat infections.

Role of sulbactam/ampicillin and sultamicillin in the treatment of bacterial infections of the upper respiratory tract of children.

The combination of sulbactam and ampicillin is a safe and effective therapy for acute otitis media and acute epiglottitis in infants and children. Despite the lack of similar studies proving efficacy for other infections of the upper airway and certain adjacent structures, such as sinusitis, tonsillitis and cellulitis/abscess of the head and neck, this drug combination should also have a therapeutic role in the future for these conditions.

Bag-mask ventilation as a temporizing measure in acute infectious upper-airway obstruction: does it really work?

Jaw-thrust and bag-mask ventilation usually provide adequate oxygenation in patients with acute infectious upper-airway obstruction (AIUAO). It is the treatment of choice for patients on the way to hospital or in an emergency department until definitive stabilization is achieved with available resources. We report three fatal case studies showing ineffective bag-mask ventilation in AIUAO that raise concerns over this treatment. Case 1 is a 4-year-old patient with epiglottitis who suffered complete obstruction during transport to the hospital. Case 2 is a 3-year-old patient with epiglottitis wh…

Transcervical Epiglottopexy for the Management of Type 3 Laryngomalacia.

Type 3 laryngomalacia (LM) is characterized by prolapse of the epiglottis into the airway. Endolaryngeal suturing is technically challenging considering the limited exposure. In the present article, we describe a simple technique of transcervical epiglottopexy (TE) via an exo-endolaryngeal technique using an 18-G needle prethreaded with a 2-0 Prolene suture in a looped fashion inserted through the inferior epiglottis. Another 20-G needle with a 2-0 Prolene suture with one free end is inserted above the previous stitch through the superior epiglottis. The single stitch is passed through the loo
