The Influence of Body Composition on Pulmonary Rehabilitation Outcomes in Chronic Obstructive Pulmonary Disease Patients.

INTRODUCTION: Alterations in body composition are commonly present in chronic obstructive pulmonary disease (COPD). The hypothesis of this s … rtant difference (MCID) after PR. CONCLUSION: A comprehensive PR program in COPD patients improved exercise tolerance and quality of life independent of muscle mass depletion or obesity. Similarly, muscle depletion or obesity had no effect on the percentage of patients achieving the MCID for measures of quality of life and exercise tolerance after PR.

Exercise physiology and testing in adult patients with congenital heart disease.

As the longevity of patients with congenital heart disease improves, the number surviving to adulthood will continue to rise. Consequently, practicing physicians can expect to encounter an increasing number of adult patients with various congenital cardiac conditions. Impaired exercise tolerance in this patient population is exceptionally common; adult patients with congenital heart disease have reduced exercise capacity compared with healthy, age-matched counterparts. The different methods of evaluating exercise capacity, the characteristic physiologic abnormalities encountered in patients wi…

Exercise intolerance and impaired aerobic capacity in heart transplant recipients.

Heart transplant recipients exhibit reduced exercise tolerance and persistently diminished aerobic capacity despite exercise training. Exercise tolerance depends on integrated oxygen transport and utilization, which are impaired in heart transplant recipients. We synthesize evidence supporting that the combined effects of impaired pulmonary gas exchange, reduced haemoglobin concentration, heart denervation, chronic calcineurin inhibition, decreased muscle capillary density, and detraining from limited physical activity contribute to: (1) reduced pulmonary diffusion and alveolar ventilation-per…

[Efficacy of the different therapeutic exercise modalities in cardiac rehabilitation after myocardial infarction. A review of the literature].

We aimed to assess the efficacy of different therapeutic exercise modalities in cardiac rehabilitation after acute myocardial infarction. A search of randomized controlled studies published between 2016 and 2021 in Scopus, PubMed, CINAHL, Web of Science and Cochrane Library was carried out using the MeSH terms "exercise therapy" and "myocardial infarction". Ten articles met the inclusion criteria; in these studies, the used exercises were combined or traditional programs that included relaxation training, and were performed either at home or at the hospital. We examined exercise tolerance, car…

Hemodynamics and exercise capacity during pacemaker stimulation.

This review summarizes the present knowledge concerning the hemodynamic and myocardial effects of various pacing modalities with special reference to the importance of heart rate variability and atrioventricular synchronization. An adequate increase in heart rate, irrespective of atrioventricular synchronization, seems to be the most important denominator for cardiac output and exercise tolerance. Atrioventricular synchronization will add some hemodynamic benefit, which is most pronounced at rest. The importance of a rate-adaptive atrioventricular delay and a normalized ventricular activation …

Preoperative inspiratory muscle training preserved diaphragmatic excursion after esophagectomy: a randomized-controlled trial.

BACKGROUND: Preoperative inspiratory muscle training (IMT) enhances diaphragmatic excursion (DE) in patients with esophageal cancer; however, the postoperative effects of IMT on DE have not yet been evaluated. This study aimed to investigate the effect of preoperative IMT on perioperative diaphragmatic function as measured by DE, inspiratory muscle strength, lung function, and exercise tolerance. METHODS: This was a parallel, randomized-controlled trial. Patients with thoracic or abdominal esophageal cancer scheduled for esophagectomy were randomized into either the incentive spirometry (IS) …

Sex Is Not an Independent Predictor of Exercise-Induced Pain After Adjustment for Performance and Pain Sensitivity.

INTRODUCTION: Exercise-induced pain (EIP) is a transient pain phenomenon that emerges during physical exertion and resolves soon after exercise cessation. Despite being recognized as a performance-limiting variable in endurance sports, the mechanisms driving its interindividual variability remain poorly defined. We aimed to determine whether sex remains a significant predictor of EIP intensity after adjustment for performance and pain sensitivity. MATERIALS AND METHODS: This cross-sectional study enrolled 122 recreational athletes (61 males and 61 females), including 48 trail runners and 74 h

Nasal high-flow therapy as an adjunct to exercise in patients with cystic fibrosis: A pilot feasibility trial.

BACKGROUND: Exercise tolerance in people with CF and advanced lung disease is often reduced. While supplemental oxygen can improve oxygenation, it does not affect dyspnoea, fatigue or comfort. Nasal high-flow therapy (NHFT), thanks to its pathophysiological mechanisms, could improve exercise tolerance, saturation and dyspnoea. This study explores the feasibility of conducting a clinical trial of using NHFT in patients with CF during exercise. METHODS: A pilot, open-label, randomized crossover trial was performed, enroling 23 participants with CF and severe lung disease. Participants completed…

Coronary microvascular function, insulin sensitivity and body composition in predicting exercise capacity in overweight patients with coronary artery disease.

BACKGROUND: Coronary artery disease (CAD) has a negative impact on exercise capacity. The aim of this study was to determine how coronary microvascular function, glucose metabolism and body composition contribute to exercise capacity in overweight patients with CAD and without diabetes. METHODS: Sixty-five non-diabetic, overweight patients with stable CAD, BMI 28-40 kg/m(2) and left ventricular ejection fraction (LVEF) above 35 % were recruited. A 3-hour oral glucose tolerance test was used to evaluate glucose metabolism. Peak aerobic exercise capacity (VO2peak) was assessed by a cardiopulmon

Rate control in atrial fibrillation: looking beyond the average heart rate.

PURPOSE OF REVIEW: The aim of this article is to provide a perspective on rate control in atrial fibrillation which emphasizes patient wellbeing (exercise tolerance, symptoms, quality of life) over attempts to reduce resting or exercise heart rate to an arbitrary range. RECENT FINDINGS: Recent trials of rhythm versus rate control strategies of treatment in patients with atrial fibrillation suggest that rate control is a viable first line strategy in many patients. The adverse consequences of atrial fibrillation with rapid ventricular response are partly due to factors other than rate itself, …

Metabolic causes of equine exercise intolerance.

Metabolic causes of exercise intolerance can be subtle and difficult to document in horses. Because of increased metabolic rate in exercising muscle, most metabolic causes of exercise intolerance are clinically manifested by muscle abnormalities such as ER. Newer causes of ER are being documented by current research and are summarized in the article on muscular causes of equine exercise intolerance. Endocrine causes of exercise intolerance have been poorly documented, but recent work has shown the detrimental effects of hypothyroidism on exercise tolerance in horses. Many metabolic manifestati…

Comprehensive cardiac rehabilitation for patients following infective endocarditis: results of the randomized CopenHeartIE trial.

AIMS: Infective endocarditis is a complex and highly mortal disease requiring lengthy treatment. Physical and mental deconditioning is common. Nonetheless, rehabilitation is virtually unexplored in this population. The aim of this trial was therefore to investigate the effects of cardiac rehabilitation in patients following endocarditis. METHODS AND RESULTS: In a randomized trial, adults with left-sided or cardiac device endocarditis were randomized 1:1 to 12 weeks of physical exercise training and five psycho-educational consultations (cardiac rehabilitation) vs. usual care without rehabilit

[Long-term physical activity after a myocardial infarction : a permanent challenge].

Cardiac rehabilitation is a well-defined multidisciplinary program with the objective to reduce mortality and morbidity, while also improving the exercise capacity and quality of life of the patient following a myocardial infarction. Despite the fact that a cardiovascular rehabilitation program is now recommended by international guidelines for all patients who have suffered from an acute coronary syndrome, only half of all patients actually participate to such a program in Switzerland. Even worse, especially when taking into consideration the population in Geneva, less than 5 % of patients fo
