Recent advances in paranasal sinus surgery.

Recent advances in paranasal sinus surgery have occurred in the treatment of trauma and neoplasia, rather than in the more traditional management of inflammatory disease. Cranialization of the frontal sinus in severe penetrating trauma was first described in 1978. By removal of the sinus posterior wall, excision of the mucosa, and preservation of the anterior wall fragments, the forehead contour can be preserved and the cavity ablated by the forward expansion of the frontal lobes. Craniofacial surgery for malignancies of the paranasal sinuses has vastly improved the survival rate for these pat

Study of Clinicopathological Presentation and Management of Malignant Tumours of the Ear and Temporal Bone at Tertiary Care Cancer Hospital.

Tumors of the ear can be benign or malignant. They can occur on the external ear or in the ear canal, the middle ear, and inner ear. Tumors in different areas of the ear behave differently. They are rare which causes several problems in diagnosis and appropriate treatment. The purpose of this study was to analyze retrospectively the patients with malignant neoplasm of the ear. The main objectives of this study were to study the clinicopathological presentation of ear and temporal bone malignancy and to study the management of different types of malignancy of ear and temporal bone. This study i

Bone resection for facial cutaneous malignancies.

BACKGROUND AND OBJECTIVES: The aim of this study is to analyze the clinical outcomes of patients who underwent bone resection for cutaneous malignancy of the face and scalp. METHODS: We retrospectively collected patient data from 62 patients who underwent bone resection for craniofacial cutaneous malignancy of the face and scalp over the last 10 years. We investigated risk factors for disease progression and assessed the utility of pre-operative imaging to predict bone, dura, and brain infiltration. RESULTS: Out of all factors analyzed, brain invasion, surgical margin involvement, and dural

Submuscular fibrolipoma of the forehead.

Fibrolipoma is a rare variant of lipoma with a mixture of fibrous connective tissue. Clinically, it presents as an asymptomatic, slowly growing mass with a smooth, firm, or soft consistency. Although fibrolipoma can occur in any part of the body, in the head region it mainly occurs in the oral cavity and sometimes in the lip, parotid gland, nose, and cheek. We present the first case of a 65-year-old man with a submuscular fibrolipoma of the forehead that showed no evidence of recurrence after excision.

The numb cheek syndrome: a sign of infraorbital neuropathy.

Three patients with skin cancer had numbness isolated to one cheek, in the distribution of the infraorbital nerve. Hypesthesia also involved the medial and lateral upper incisors and canine teeth, and adjacent gingiva, sparing the more posterior teeth and gums. The molar and premolar teeth and gums are innervated by the posterior and middle superior alveolar nerves; because these structures were spared, the pathologic process was localized to the infraorbital foramen, and we could exclude involvement of the maxillary division more proximally. In two patients, cheek numbness heralded recurrent

Comparative analysis of facial nerve outcomes in petroclival versus posterior petrous meningioma surgery: A systematic review and meta-analysis of 2884 patients.

BACKGROUND: Even conservative resection of petroclival (PC) and posterior petrous (PP) meningiomas may cause facial nerve (FN) morbidities. While inconsistent classifications by tumor's dural origin circulate among different cohorts, a comprehensive assessment of postoperative FN outcomes is lacking. We systematically evaluate early and long-term FN outcomes related to surgical extirpation of PC and PP meningiomas. METHODS: A PRISMA-tailored survey of PubMed, Embase, Ovid/Medline, and Scopus was used to assess pooled perioperative FN deficit rates in PC and PP meningiomas. Outcomes were dicho

Vesiculobullous variant of adult T-cell leukemia/lymphoma in a Caribbean Émigré.

Adult T-cell leukemia/lymphoma (ATLL) results from human T-cell lymphotropic virus (HTLV) type I infection and may present as a diverse array of cutaneous findings. Often these clinical manifestations are non-specific and overlap significantly with cutaneous T-cell lymphoma (CTCL). However, it is exceedingly rare for a patient suffering from ATLL to develop vesicular or bullous pathology and only a handful of such cases have been reported in the literature. The authors describe a patient of Jamaican descent afflicted with ATLL who developed an impressive vesiculobullous eruption. This case pro

A neurilemmoma of the parotid gland: report of a case.

Benign primary tumors of the facial nerve are frequently misdiagnosed because of the variety of their clinical manifestations. Much attention has been paid to neurilemmomas in the recent otolaryngologic literature, but far less has been focused on intraparotid facial nerve neurilemmomas. In this article, we describe a new case of this truly rare tumor, and we review its diagnosis, pathology, and treatment.

Our experience with surgical dissection of the facial nerve in parotid gland tumours. (A preliminary report).

Conservative parotidectomy is a primary method in the surgical treatment of parotid gland tumours. The classic parotid gland surgery uses the styloid process as a landmark for dissection of the facial nerve. The anatomical variations of the styloid process, however, prevent it from being always used as a reference landmark in this operative approach. This requires finding reliable invariable anatomical landmarks to be applied in the facial nerve dissection. The author presents his experience from the surgical treatment of 37 patients (25 females and 12 males) with tumours of the parotid gland.

[Fat suppression in contrast-enhanced MRT of the base of the skull and of the head-neck area: its clinical value].

167 patients with abnormalities at the skull base and at the cervical-skull junction were examined by MRT in order to compare a FATSAT technique with T1- and T2-weighted SE sequences before and after intravenous injection of 0.1 mmol Gd-DTPA/kg KG. The diagnostic information from corresponding FATSAT and T1-SE sequences was correlated with the histopathological findings. In 10.7% of patients fat suppression was inadequate and in a further 11.3% of patients chemical shift artifacts limited the diagnostic value. The number of detectable lesions was not increased by the use of FATSAT sequences bu

Characteristics of the ocular surface in neurotrophic keratitis induced by trigeminal nerve injury following neurosurgery.

PURPOSE: To analyse and quantify ocular surface parameters in patients with unilateral neurotrophic keratitis (NK) induced by trigeminal nerve injury post-neurosurgery. METHODS: The study included 26 unilateral NK patients who had undergone neurosurgery, and 20 matched normal controls. Demographic and clinical characteristics of all participants were collected and analysed. Slit-lamp examination, Cochet-Bonnet aesthesiometry, Keratograph 5 M, and LipiView interferometer were performed on both eyes of 17 mild NK patients. For nine moderate/severe NK patients, sub-basal nerve density was measur

Management strategies in neurofibromatosis type 2.

The objective of this study was to review the experience of the Neurotology and Skull Base Surgery Unit of Addenbrooke's Hospital in Cambridge, England, in the management of patients with neurofibromatosis type 2 (NF2). This was a retrospective review of the institution's series conducted at a neurotological tertiary referral centre. Over a 17-year period (1984-2001), 35 patients with NF2 were managed. These patients presented with multiple cranio-spinal neoplasms, including 62 cerebellopontine angle tumours of which 59 were vestibular schwannomas (nine patients with unilateral tumours and 25

High-dose radiation and the emergence of thyroid nodular disease.

High-dose radiation (in excess of 2500 rads or centiGray) to the head and neck area is reputedly infrequently associated with the emergence of thyroid nodular disease. Thirty-three patients who underwent high-dose radiation and who developed thyroid nodular disease have been described. Radiation was originally administered for hyperthyroidism in 11 patients, postmastectomy in five, oral cancer in three, Hodgkin's disease in three, facial hirsutism in three, hemangioma in three, cancer of the larynx in one, skin cancer in one, desmoid tumor of the neck in one, Ewing's tumor in one, and pituitar

Relationship of Postoperative Temporary Facial Nerve Dysfunction With Tumor Location in Parotid Surgery.

Introduction Postoperative temporary facial nerve dysfunction (TFND) is a common complication after parotidectomy, and the tumor location is believed to influence its risk. This study aimed to evaluate the relationship between tumor location and postoperative TFND, as well as to assess the effects of lobe involvement and surgical approach. Methods This prospective observational study was conducted at Bangladesh Medical University, Dhaka, between June 2022 and December 2023, and 35 patients with parotid neoplasms who underwent surgery were enrolled in this study. Preoperative magnetic resonance

An Intrapatient Concordance Study of Mismatch Repair Protein Immunohistochemical Staining Patterns in Patients With Muir-Torre Syndrome.

IMPORTANCE: Appropriate use criteria for Muir-Torre syndrome (MTS) screening suggest that mismatch repair protein (MMRP) immunohistochemical (IHC) testing is usually appropriate in patients with 2 or more sebaceous neoplasms (SNs). While MTS is known to be caused by a germline mutation in mismatch repair genes, data are limited as to whether individual sebaceous tumors in these patients with multiple lesions show identical MMRP IHC staining patterns. OBJECTIVE: To determine concordance of MMRP IHC staining patterns in lesions of patients with MTS who have multiple SNs. DESIGN, SETTING, AND P
