Previous studies performed in France have suggested that handling hay contaminated with high amounts of moulds, and especially Absidia corymbifera and Eurotium amstelodami, may favour farmer's lung disease. The circumstances favouring farmer's lung disease and the distinctive microbiological composition of hay samples that provoke attacks need to be specified. Farmer's lung (FL) is characterized by an intense lymphocytic alveolitis which persists after an acute episode with continuous exposure to the offending antigens. This study was based on data provided by 9,483 farmers from a larger sample of 12,056 Finnish farmers. A 33-year-old woman farmer developed an acute episode of fever, cough, and shaking chills with persistent shortness of breath. Her PaO2 was 51 with a restrictive pattern on pulmonary function. Her diffusion capacity was 36 percent of predicted. In spite of these abnormalities, she always had normal chest roentgenographic findings. Further studies, including a lung biopsy, led to the diagnosis of farmer's lung disease.

We present a case of total IgA deficiency associated with farmer's lung. The IgA deficiency is combined with the IgE one. IgA are absent both in the serum and in the bronchial secretions; they are present on the surface of B-lymphocytes, also after the enzymatic surface digestion and resynthesis test is performed. With intravenous perfusion of human gamma globulins, during repeated courses, IgA reach a normal level and exceed it, despite the IgA concentration in the perfused compound is very low and despite the fact that injected gamma globulins are not detectable with electrophoretic method.

A critical and comparative analysis between chest radiograms and CT pictures in pulmonary fibrosis of various degree, occurring in farmer's lung, reveal that the latter are apt to give a somewhat more diversified informations about pulmonary changes as simple radiography. This would prove useful in evaluating the extension and distribution of anatomical changes due to fibrotic process, although as far not able to allow an earlier diagnosis.

The authors present a retrospective study of 82 cases of farmer's lung disease in the Aveyron department. An agar gel precipitin test, using osmophoresis, has been devised for use with weakly reacting sera. The test was applied to various categories of sera, including sera from cases of farmer's lung which had reacted with a conventional immuno-diffusion test, and others which had not.
