Randomised controlled trial of cardiotocography versus Doppler auscultation of fetal heart at admission in labour in low risk obstetric population.

OBJECTIVE: To compare the effect of admission cardiotocography and Doppler auscultation of the fetal heart on neonatal outcome and levels of obstetric intervention in a low risk obstetric population. DESIGN: Randomised controlled trial. SETTING: Obstetric unit of teaching hospital PARTICIPANTS: Pregnant women who had no obstetric complications that warranted continuous monitoring of fetal heart rate in labour. INTERVENTION: Women were randomised to receive either cardiotocography or Doppler auscultation of the fetal heart when they were admitted in spontaneous uncomplicated labour. MAIN O

Association of normal-range fetal heart rate variations during labor with umbilical cord arterial blood gas parameters and neonatal outcomes: A cross-sectional study.

BACKGROUND: Fetal heart rate (FHR) monitoring is a routine, non-invasive method for assessing fetal well-being during labor. However, its low sensitivity and high false-positive rate often lead to unnecessary cesarean deliveries. While fetal scalp blood sampling offers a more accurate assessment of intrauterine hypoxia, it is invasive, technically challenging, and prone to failure. OBJECTIVE: To evaluate the association between intrapartum FHR beat-to-beat variability (within the normal range) and umbilical cord arterial blood gas parameters, and to assess their predictive value for neonatal

Tocolysis and delayed delivery versus emergency delivery in cases of non-reassuring fetal status during labor.

AIM: To determine whether fetal intrauterine resuscitation using tocolysis and delayed delivery is better for the fetus than emergency delivery when fetal hypoxia is suspected because of a non-reassuring fetal heart-rate (FHR) pattern using conventional heart rate monitoring. METHODS: This was a prospective and randomized study, conducted between 2001 and 2004 at Pereira Rossell Hospital, Montevideo, Uruguay. The population consisted of 390 fetuses, in which intrauterine distress was diagnosed using electronic FHR monitoring. Of these, 197 were randomly assigned to the emergency delivery grou

[Fetomaternal macrotransfusion with severe fetal anemia].

A fetomaternal macrotransfusion endangers fetal life. This report describes a fetomaternal macrotransfusion of about 300 ml blood. Ante partum and intra partum monitoring showed a mild fetal tachycardia. After spontaneous delivery the newborn was severe anemic and has a mild hepatosplenomegaly and edema. The diagnosis of fetomaternal macrotransfusion was made by Kleihauer-Betke-Test with maternal blood.

Umbilical artery waveform analysis and biophysical profile. A comparison of two methods to identify compromised fetuses.

Sixty-nine singleton high-risk pregnancies were assessed with both umbilical artery flow velocity waveform analysis and fetal biophysical profile within 10 days of delivery. An abnormal outcome, as defined as either small for gestational age at birth (SGA) or fetal distress during labour, was found in 30 babies. The peak systolic/end diastolic (A/B) ratio from the umbilical artery had a higher sensitivity (37%), specificity (92%), positive predictive value (79%) and negative predictive value (66%) than the fetal biophysical profile (27%, 82%, 53%, and 59%, respectively) in the diagnosis of abn

The significance of sinusoidal fetal heart rate pattern during labor and its relation to fetal status and neonatal outcome.

Twenty-seven cases of sinusoidal fetal heart rate were studied. This group had a mean scalp pH of 7.288, significantly lower (p less than 0.005) than that of the control group. The mean one-minute Apgar score was 7.148, significantly lower (p less than 0.001) than the control group's mean score. Almost half (48,15%) of the fetuses had a one-minute Apgar score of 7 or less. Every fetus was viable. Amplitude of the pattern was found to correlate with neonatal depression, the larger the amplitude, the more profound the depression. Over 96% of the fetuses had cord-related deceleration patterns, an

Intrauterine growth pattern in Butajira HDSS, Southern Ethiopia: BUNMAP pregnancy cohort.

BACKGROUND: Abnormal fetal growth pattern during pregnancy including excessive fetal size and intrauterine growth restrictions are the major determinants for perinatal outcomes and postnatal growth. Ultrasonography is a useful tool in monitoring fetal growth for appropriate care and interventions. However, there are few longitudinal studies using serial ultrasonography in low and middle-income countries. Moreover, the reference charts used for fetal growth monitoring in low-income countries comes from high income countries with distinct population features. Therefore, the purpose of this study

[The diagnosis of fetal atrio-ventricular block (author's transl)].

The case is reported of a primigravida who was found to have a fetal bradycardia of 70 per minute. A caesarean section was done with the differential diagnosis intrauterine hypoxia or fetal arrhythmia. The infant was found to have a single ventricle, single atrium, a hypoplastic aortic arch and a coartation of the aorta. The infant died neonatally.--The case was reported to emphasize the necessity of fetal electrocardiography in continual antenatal monitoring to rule out an atrio-ventricular block prior to caesarean section.

Fetal heart rate mirrors maternal temperature during posterior fossa craniotomy: a case report.

While it is well known that maternal temperature affects fetal heart rate, the exact relationship is not well described. The circumstances accompanying most cases of maternal hypothermia and rewarming (e.g. a drowning event) have precluded a precise quantitative description of this relationship. We describe hypothermia and controlled rewarming during resection of a maternal brain stem tumor in the early third trimester. Continuous electronic fetal heart rate and core temperature monitoring demonstrated a near linear relationship during the development of hypothermia and rewarming. Recognition

Maternal serum bile acid levels and fetal distress in cholestasis of pregnancy.

Cardiotocography (CTG) and serum total bile acid level were used in the perinatal surveillance of 117 pregnancies with intrahepatic cholestasis. Signs of fetal distress occurred more commonly in cholestasis pregnancies with high maternal bile acid levels. Despite careful monitoring one intrauterine fetal loss occurred without any warning signs in CTG. In this case the serum bile acid level was only moderately elevated. CTG seems to be suitable for detection of fetal distress in cholestasis pregnancies. Those with high maternal bile acid level should be subjected to a more intensive follow-up.

Fetal pulse oximetry for fetal assessment in labour.

BACKGROUND: The use of conventional cardiotocographic (CTG) monitoring of fetal well-being during labour is associated with an increased caesarean section rate, compared with intermittent auscultation of the fetal heart rate, resulting in a reduction in neonatal seizures, although no differences in other neonatal outcomes. To improve the sensitivity of this test and therefore reduce the number of caesarean sections performed for nonreassuring fetal status, several additional measures of evaluating fetal well-being have been considered. These have demonstrated some effect on reducing caesarean

Doppler ultrasound in all three trimesters of pregnancy.

Doppler ultrasound velocimetry of uteroplacental, umbilical, and fetal vessels provides the clinician with important information on the hemodynamics of the vascular area under study. Transvaginal color Doppler is a new technique that allows precise investigation of fetomaternal circulation in early pregnancy. Doppler can be used to follow physiologic hemodynamic changes in uteroplacental and fetal vessels in this period of gestation, and to measure circulatory disturbances in some pathologic conditions. In late pregnancy, Doppler is a useful diagnostic tool for monitoring fetuses with intraute

Probabilistic Learning Coherent Point Drift for 3D Ultrasound Fetal Head Registration.

Quantification of brain growth is crucial for the assessment of fetal well being, for which ultrasound (US) images are the chosen clinical modality. However, they present artefacts, such as acoustic occlusion, especially after the 18th gestational week, when cranial calcification appears. Fetal US volume registration is useful in one or all of the following cases: to monitor the evolution of fetometry indicators, to segment different structures using a fetal brain atlas, and to align and combine multiple fetal brain acquisitions. This paper presents a new approach for automatic registration of

Fetal movements as a predictor of health.

The key determinant to a fetus maintaining its health is through adequate perfusion and oxygen transfer mediated by the functioning placenta. When this equilibrium is distorted, a number of physiological changes, including reduced fetal growth, occur to favor survival. Technologies have been developed to monitor these changes with a view to prolong intrauterine maturity while reducing the risks of stillbirth. Many of these strategies involve complex interpretation, for example Doppler ultrasound for fetal blood flow and computerized analysis of fetal heart rate changes. However, even with thes

Fetal Sleep: A Cross-Species Review of Physiology, Measurement, and Classification.

Fetal sleep is a relatively underexplored yet vital aspect of prenatal neurodevelopment. Understanding fetal sleep patterns could provide insights into early brain maturation and help clinicians detect signs of neurological compromise that arise due to fetal hypoxia or fetal growth restriction. This review synthesizes over eight decades of research on the physiological characteristics, ontogeny, and regulation of fetal sleep. We compare sleep-state patterns in humans and large animal models, highlighting species-specific differences and the presence of sleep-state analogs. We review both invas

Stomach herniation prior to nonreassuring fetal status in a case of fetal gastroschisis.

The neonatal prognosis in gastroschisis is generally good; however, intrauterine fetal death (IUFD) and nonreassuring fetal status (NRFS) sometimes occur in fetal cases of gastroschisis. Previously, we reported stomach herniation as a possible risk factor for IUFD or NRFS in fetuses with gastroschisis. We report a case of fetal gastroschisis showing stomach herniation and increased velocity of the umbilical venous (UV) flow the day before NRFS by electronic fetal monitoring (EFM). A 27-year-old pregnant woman was followed up from 16 weeks' gestation due to fetal gastroschisis. At 33 + 1 weeks'…

Diffuse placental chorioangiomatosis causing multiple fetal cerebral embolism: a case report.

BACKGROUND: Placental chorioangiomas are benign vascular tumors. Large chorioangiomas have been reported to cause several obstetric complications, including premature labor, placental abruption, polyhydramnios, fetal hydrops, fetal growth restriction, fetal hepatosplenomegaly, cardiomegaly, a congestive heart failure and fetal death. The clinical significance of small hemangiomas is less known. CASE: Multiple small placental hemangiomas occupied 70% of the total placental volume. A shower of emboli in the fetal cerebral circulation occurred 1 week before the delivery, causing permanent brain
