The aims of the present study were to carry out surveillance for dengue virus infection in adults with short-duration fever, and serological study of dengue virus infection in persons without fever. Patients were divided into two groups. Group 1 included patients above 12 years of age with fever of 2-12 days duration without any apparent cause. Human infection with the rickettsia Coxiella burnetii presents as acute influenza-like primary Q fever, subacute granulomatous hepatitis, or chronic endocarditis with hepatitis. To investigate whether persistent infection is associated with a possible immunologic defect, we tested lymphocyte proliferation specific for Coxiella in vitro in peripheral blood mononuclear cells from patients and controls. West Nile fever is an infection affecting thousands of people each year, and is responsible for a wide spectrum of clinical manifestations from a mild febrile illness to lethal encephalitis. The prevalence of this infection and its associated morbidity has risen dramatically in recent years. We present a patient with non-specific complaints and prolonged fever for 7 weeks. He underwent thorough investigation, which almost ended with abdominal laparoscopy until the final diagnosis of West Nile Fever was established.

A prospective evaluation of 146 children with fever of unknown origin (FUO) and prolonged fever was performed from 1990 to 1996. FUO was defined as a documented daily temperature of > or = 38 degrees C for at least 14 days without diagnostic signs or symptoms.
