OBJECTIVE: To improve the outcomes in patients with tracheoesophageal fistula (TEF), including those with concurrent tracheal stenosis and recurrent fistula, using various modern surgical interventions.
MATERIAL AND METHODS: There were 115 patients with TEF in 2004-2023. Annual number of patients doubled when comparing with the period from 1963 to 2002. In 85.4% of cases, TEF was caused by iatrogenic injury of trachea and esophagus after mechanical ventilation or tracheostomy: 56 patients (48.7%) had fistulas after tracheostomy, 42 (36.7%) ones - after intubation. Vesico-vaginal fistulae require complex reconstructions and robotic assistance can be beneficial in obtaining successful outcomes. In this video, we demonstrate two of our techniques of vesicle-vaginal fistula repair, one with transverse cystotomy and other with vertical cystotomy, along with salient steps of the procedure. The indication, preparation, instrumentation, and surgical steps are also discussed along with a review of available literature.

CASE: Vesicouterine fistula (VUF) is the rarest form of genitourinary fistulas. As lower-segment cesarean section becomes a more common mode of delivery, they have become the leading cause of VUF formation. We present four VUF patients with varied symptoms such as menouria, amenorrhea, with or without urinary incontinence. We diagnosed all of our cases through cystoscopy in conjunction with methylene blue dye test or hysteroscopy.

The use of seton still remains a reasonably valid option for its simplicity and optimal results in treating high anal fistulae. Ours was a single-centre retrospective study on patients with supra-sphincteric and transsphincteric types of high anal fistula, using the 'kharsutra' as seton. The first group were treated with a seton alone, while the second with a seton and partial fistulotomy. Results were similar and quite acceptable in both groups.
