Demyelinating polyradiculoneuritis following Coxiella burnetti infection (Q fever).

Neurological complications of Coxiella burnetii infection (Q fever) are rare, although the occurrence of headache, paresthesias, and transient focal deficits has been reported. We report the case of a patient with a relapsing demyelinating polyradiculoneuritis as an aftermath of C. burnetti endocarditis and pneumonia.

Too old to fight? Aging and its toll on innate immunity.

Elderly individuals display increased susceptibility to chronic inflammatory diseases and microbial infections, such as periodontitis and oral aspiration pneumonia. The resurgent interest in innate immunity in the 2000s has been accompanied by parallel studies to understand the impact of aging on the function of the innate immune system, which not only provides first-line defense but is essential for the development of adaptive immunity. This review summarizes and discusses our current understanding of age-associated molecular alterations in neutrophils and macrophages, key inflammatory phagoc

Lemierre syndrome from an odontogenic source: a review for dentists.

Lemierre syndrome, which can result from a recent oropharyngeal or odontogenic infection, is characterized by clinical or radiographic signs of thrombosis of the internal jugular vein, distant infected emboli and the presence of anaerobic pathogens, usually Fusobacterium necrophorum. The septic emboli resulting from the infected thrombophlebitis of the internal jugular vein give the syndrome its constellation of central nervous system, pulmonary and many other manifestations including septic shock. This condition was so rare that, historically, it became known as the "forgotten disease," but a

Potential sites of infection that develop in febrile neutropenic patients.

Three hundred episodes of neutropenia were reviewed for the occurrence of potential sites of infection. Ninety sites (30 per cent) were identified at the onset of fever independent of initial bacteraemia which was encountered in 104 episodes (35%) and predominantly involved Gram-positive cocci. A further 90 sites were recorded involving mainly the lower respiratory tract (58%) and skin and soft tissue (18%). These changes evolved significantly later (mean of 5.1 and 4.3 days respectively) than did other foci which mainly presented at the onset of fever (p < 0.01). However the infectious aetiol

Acute focal bacterial nephritis and renal abscess associated with vesicoureteral reflux. Report of a case.

We report the case of a child with acute focal bacterial nephritis and a renal abscess in the right kidney associated with bilateral vesicoureteral reflux. The patient was treated successfully with antibiotics and bilateral ureterocystoneostomy. Urine has remained sterile since the operation, but the affected kidney has developed progressive scarring. This case suggests that acute focal bacterial nephritis and a renal abscess may cause a small kidney, which looks like a hypoplastic kidney, associated with vesicoureteral reflux.

Disseminated Mycobacterium colombiense Disease in a Person Living with HIV: Diagnostic and Therapeutic Considerations.

BACKGROUND: Disseminated non-tuberculous mycobacterial (NTM) disease is an uncommon but serious complication in people living with HIV. Rare species such as Mycobacterium colombiense are infrequently reported, and guidance on diagnosis and treatment remains limited. This case illustrates challenges in identifying and managing disseminated M. colombiense infection. DESCRIPTION: A middle-aged man living with HIV presented with disseminated Talaromyces marneffei infection complicated by hemophagocytic lymphohistiocytosis. He received antifungal therapy, immunomodulatory treatment, and antiretrov

Cytomegaloviral adrenalitis and adrenal insufficiency in AIDS.

In a review of 74 autopsy cases of acquired immune deficiency syndrome (AIDS), the incidence of cytomegalovirus (CMV) infection was found in 50% (37 cases). Of these 37 cases the adrenal glands were affected by CMV in 84%. The authors analyzed the histologic features of 30 of these 37 cases of CMV adrenalitis by grading the CMV inclusions, inflammation and necrosis (CMV score), focal versus diffuse involvement, and the degree of fibrosis (fibrosis score). The clinical data were not significant except for sodium to potassium ratio (Na:K), which was less than 30 in 11 cases. Focal CMV adrenaliti

TLR2/CXCR4 coassociation facilitates Chlamydia pneumoniae infection-induced atherosclerosis.

Chlamydia pneumoniae infection could play a role in atherosclerosis. Toll-like receptor 2 (TLR2) and C-X-C motif chemokine receptor 4 (CXCR4) have been both shown to be involved in atherosclerosis. However, whether and how TLR2/CXCR4 cross talk is involved in C. pneumoniae infection-induced atherosclerosis remains to be determined. Our study aims to demonstrate that C. pneumoniae infection induced the cross talk between TLR2 and CXCR4 to mediate C. pneumoniae infection-induced vascular smooth muscle cell (VSMC) migration and even accelerate atherosclerosis. We first found that C. pneumoniae in

Abscesses of the frontal lobe of the brain secondary to covert dental sepsis.

The bacterial species found in pus aspirated from brain abscesses in two patients were typical of those found in dental sepsis. Subsequently apical-root abscesses were demonstrated in the upper jaws of both patients. This evidence strongly suggests that these cerebral abscesses were secondary to dental sepsis which could have spread from the teeth to the frontal lobes by several possible antaomical pathways.

[Wegener's granulomatosis: classification and therapy].

Wegener's granulomatosis in its classic form is manifested by necrotizing granulomas of the upper and lower respiratory tract, focal necrotizing vasculitis involving both arteries and veins which may be widely disseminated, and focal necrotizing glomerulitis. The disease may present with incomplete expression involving any combination of the major sites, including the upper respiratory tract (E), the lung (L), or the kidney (K). A newly emerging test for the disease called antineutrophil cytoplasmic antibody (ANCA) shows promise in studying the disease in its various expressions. Standard curr

Rapid emergence of a focal epidemic of Lyme disease in coastal Massachusetts.

We describe a focal epidemic of Lyme disease, which spread from a nature preserve and affected an adjacent community of permanent residents in coastal Massachusetts. The attack rate from 1980 through 1987 was 35 percent among 190 residents living within 5 km of the nature preserve and was greatest (66 percent) among those living closest to the preserve. The risk of infection bore little relation to sex or age. Late Lyme disease, which clustered near the preserve, occurred mainly in residents infected early in the epidemic who did not have a history of erythema migrans and did not receive antib

The acquired immune deficiency syndrome: an international health problem of increasing importance.

The Acquired Immune Deficiency Syndrome (AIDS) is a new disease which first appeared in human populations about 1979. The disease is defined by the development of unusual types of cancer (e.g. Kaposi's sarcoma), or severe cellular immunodeficiency manifested by opportunistic infections (e.g. Pneumocystis carinii infection), or both. Although the etiology of AIDS is unknown, the epidemiologic evidence is consistent with an infectious agent transmitted by blood (e.g. transfusion, needle sharing) or sexual intercourse. Over three-quarters of the cases have been in homosexual or bisexual males and
