BACKGROUND: Anesthetic management of an adult with failing Fontan physiology is complicated given inherent anatomical and physiological alterations. Neurosurgical interventions including thromboembolectomy may be particularly challenging given importance of blood pressure control and cerebral perfusion.
CASE PRESENTATION: We describe a 29 year old patient born with double outlet right ventricle (DORV) with mitral valve atresia who after multi-staged surgeries earlier in life, presented with failing Fontan physiology. She was admitted to the hospital almost 29 years after her initial surgeries

Pulmonary hypertension (PH) due to elevated left atrial pressure (LAp) caused by restrictive left atrioventricular valve and interatrial communication sometimes precludes patients with univentricular heart from undergoing the Fontan operation. We have created atrial septal defect (ASD) for such patients to reduce LAp and then pulmonary arterial pressure (PAp) and pulmonary vascular resistance (Rp) in an attempt to qualify the patients for the Fontan repair. This study was performed to clarify the efficacy and limitation of this approach. Twelve patients with PH (mean PAp > 20 mm Hg and/or Rp >

BACKGROUND: Patients with complex congenital heart defects, such as tricuspid atresia, often require palliative surgical interventions like the Fontan procedure to optimize systemic and pulmonary circulation. While these surgeries improve survival rates, they are associated with long-term complications, including arrhythmias, due to significant anatomical and electrophysiological alterations. Among these, atrioventricular nodal reentrant tachycardia (AVNRT) is relatively uncommon but poses substantial challenges in diagnosis and management.
CASE SUMMARY: This case report describes a 36-year-o…

Despite improvements in surgical techniques and perioperative management, the overall results of traditional staged palliation for hypoplastic left heart syndrome (HLHS) remain suboptimal. We report the evolution of a hybrid strategy in 34 patients coupling innovative surgical and transcatheter techniques that requires one open-heart procedure to reach a Fontan completion. Experience has led to modifications in technique, timing of procedures, and follow-up. Current initial palliation is by intraoperative, off-pump, placement of bilateral pulmonary artery bands and a PDA stent. Transcatheter c

OBJECTIVES: We hypothesized that hepatic injury in single-ventricle CHD has origins that predate the Fontan operation. We aimed to measure h … ng subjects who had BCPC, subjects who were healthy, and a cohort of patients who had undergone the Fontan procedure.
RESULTS: Forty subjects (20 patients who had BCPC; 20 age- and gender-matched control subjects) were studied. The hepatic stiffness in BCPC was elevated, compared with that in control subjects (7.2 vs 5.7 kPa; P = .039). Patients who had BCPC had …

BACKGROUND: Biatrial tachycardia (BiAT) is a rare form of macroreentry not previously characterized in adults with congenital heart disease (ACHD).
OBJECTIVE: The purpose of this study was to determine the prevalence, mechanisms, and outcomes of catheter ablation for BiAT in ACHD.
METHODS: All ACHD undergoing catheter ablation for macroreentrant atrial tachycardia over a 10-year period were evaluated for evidence of BiAT. Patients were categorized as prior Senning, Fontan, or other biventricular operation. A novel biatrial global activation histogram (GAH) analysis was used to demonstrate th

In this review, we provide a brief description of recently published articles addressing topics relevant to pediatric cardiologists. Our hope is to provide a summary of the latest articles published recently in other journals in our field. The articles address (1) developing a risk score to predict adverse outcomes in patients with hypoplastic left heart syndrome using the national pediatric cardiology quality improvement collaborative, (2) outcomes of the Ross procedure in patients with a previous aortic valve surgery showing better outcomes in these patients compared to patients with the pri

The presence of left ventricular hypoplasia in double-outlet right ventricle may increase the risk of biventricular repair and Fontan procedures. The hypoplastic left ventricle of an 11-year-old girl with complex double-outlet right ventricle was successfully incorporated in a modified biventricular repair by Dacron patch closure of the ventricular septal defect, diversion of venous return of the inferior vena cava to the mitral valve with an intraatrial baffle, insertion of a left ventricular apex to pulmonary artery valved aortic homograft, and diversion of residual systemic venous return di…

We report the case of a 15-year-old boy with a single left ventricle who underwent total cavopulmonary connection (Fontan circulation). Due to a progredient idiopathic scoliosis he had to undergo two surgical correction procedures of the vertebral column. Fontan circulation is characterized by the functional absence of the right ventricle. Blood from the systemic circulation passively flows directly into the pulmonary artery. Therefore, central venous preload as well as pulmonary vascular resistance gain essential significance for cardiac output. After volume preload, in both procedures anaest

OBJECTIVE: To characterize patients with protein-losing enteropathy (PLE) diagnosed before age 18 years within a nationwide Swedish Fontan cohort.
METHODS: Surgical records and medical charts were reviewed for all patients born after January 1, 1993, who underwent Fontan completion before January 1, 2021.
RESULTS: Among 573 reviewed patients, 28 (4.8%) developed PLE. The median time to onset of PLE after Fontan was 1.5 years (interquartile range, 0.5-5.5 years). Right ventricular morphology was associated with PLE (odds ratio, 2.3; 95% confidence interval, 1.04-5.0). Twenty-three patients (8

PURPOSE: It is important to identify those children with a Fontan circulation who are at risk for impaired health-related quality of life. We aimed to determine the predictive value of functional health status - medical history and present medical status - on both physical and psychosocial domains of health-related quality of life, as reported by patients themselves and their parents.
METHODS: We carried out a prospective cross-sectional multi-centre study in Fontan patients aged between 8 and 15, who had undergone staged completion of total cavopulmonary connection according to a current tec

BACKGROUND: Fenestration of the Fontan circulation that results in a residual right-to-left shunt has improved operative survival rates among high-risk patients. Late closure of the fenestration by use of a transcatheter umbrella device has achieved separation of the systemic and pulmonary venous circulations, "completing" the Fontan pathway. Because use of umbrella devices is restricted, many institutions continue to perform only nonfenestrated Fontan procedures.
METHODS AND RESULTS: Five children 3.5 to 8.3 years old (mean, 5.1 years) underwent cardiac catheterization 0.5 to 24 months (mean…

BACKGROUND: Protein-losing enteropathy (PLE) is a serious complication affecting approximately 5–12% of patients following the Fontan procedure for congenital heart disease and is associated with high mortality. Its pathophysiology is multifactorial, involving elevated central venous pressure, abnormal intestinal lymphatics, and altered mesenteric vascular resistance. No gold-standard therapy has been established, and current management approaches include hemodynamic optimization, nutritional therapy, and medications such as somatostatin analogues and spironolactone. Though spironolactone is f…

To evaluate the long-term results of atriopulmonary Fontan connection, we reviewed the outcome of 20 consecutive patients (single ventricle 8, double outlet right ventricle 8, tricuspid atresia 3, mitral atresia 1; mean age, 13 +/- 11) who underwent this operation between 1981 and 1997 at our institution. Glenn shunt preceded Fontan operation in 1 patient. Four patients had a concomitant bidirectional Glenn anastomosis at the time of the Fontan operation. Follow-up was 100% complete, and total cumulative follow-up of all patients was 241 patient-years (maximum, 20 years). Three hospital deaths

AIMS: Many patients develop Fontan-associated liver disease (FALD) after undergoing the Fontan procedure-a surgical treatment for congenital heart disease such as single ventricle-owing to changes in venous pressure and cardiac output. Liver biopsy is the gold standard for diagnosing FALD, but has limitations. Magnetic resonance elastography (MRE) is a popular non-invasive method for evaluating liver stiffness and fibrosis in FALD; however, no unified view exists. This study aimed to assess the usefulness of MRE in evaluating the pathophysiology of FALD, including its correlation with cardiova…
