Dear Editor, We report a case of a patient admitted to our Department presenting a typical digital myxoid cyst. A 54-year-old man was referred by his general practitioner to the Department of Dermatology at our hospital due to the presence of an asymptomatic lesion on the third right toe. Physical examination revealed a rounded, red-bluish lesion in the eponychium less than 0.5 cm in diameter with a cystic appearance that secondarily caused a longitudinal depression in the nail plate (Figure 1). Dermoscopy showed arboriform telangiectasias over white, bluish, and reddish-orange diffuse areas (

BACKGROUND: There is no general agreement as to what constitutes cure or treatment success in onychomycosis. Regulatory guidelines differ in the United States and Europe, and outcomes reported in clinical trials do not consistently report secondary endpoints.
METHODS: We reviewed definitions of onychomycosis cure to develop a less stringent and more practical approach to assess improvement and treatment success.
RESULTS: Complete cure (totally clear nail and mycologic cure) remains an important regulatory standard. Mycologic cure (negative fungal culture and negative potassium hydr

The authors describe a case of onychomycosis by Paecilomyces lilacinus in an 81-year-old Caucasian woman. The infection was characterized clinically by yellow-brown discoloration of the nail plate, distal-lateral onycholysis and subungual hyperkeratosis of the first left toenail. Microscopic examination showed irregular hyphae. Four cultures showed the development of lilac and powdery colonies. Conidiophores with divergent long narrow terminal phialides were observed at microscopic examination of culture samples. Therapy with amorolofine nail laquer, itraconazole and terbinafine was uneffectiv

We report the first case of onychomycosis caused by Onychocola canadensis in Estonia. We believe that the number of nail infections caused by this fungus is underestimated due to the current diagnostic algorithm of non-dermatophytic onychomycosis. The need to define categories and criteria for 'proven' and 'probable' non-dermatophyte mold infections to promote more extensive studies in the future is also discussed.

BACKGROUND: Managing toenail onychomycosis with topical treatments is challenging. It is difficult for topical medication to penetrate the nail plate, and this is reflected in lower cure rates with topical treatment than with oral treatment. However, oral medications may not be suitable for some patients, because of drug interactions; therefore, topical treatments are critical in managing the disease in certain patient populations.
OBJECTIVE: This paper reviews the quality and content of the scientific literature on topical treatments for toenail onychomycosis.
METHODS: PubMed, Ovid (Medline

Article whose content was exposed in the workshops of the GNEAUPP Congress, held in Seville in November2012, and which refers to ulcers by pressure on the heels as a location exposed to the analysis. A pressure ulcer is a lesion located in skin I underlying tissue usually over a bone prominence, as a result of the pressure, or pressure in combination with the shears. A number of contributing factors or confounding factors are also associated with ulcers by pressure; the importance of these factors still not been elucidated. The heels are next to the sacred area, parts of the body that most fre

Curvularia lunata is a dematiaceous fungus that rarely causes cutaneous infections in immunocompetent humans. We describe a case of mycetoma pedis caused by Curvularia lunata in a 50-year-old woman from a rural farming community who had prolonged morbidity due to diagnostic delay. She improved with antifungal therapy, showing a satisfactory clinical response after 12 months of treatment.

Molluscum contagiosum (MC) is a skin infection caused by the double-stranded DNA virus of the family Poxviridae that typically presents as flesh-colored asymptomatic umbilicated papules. Plantar MC is uncommon. We describe a 23-year-old man who presented with multiple plantar MC. We also summarize the epidemiologic features of the 34 previously reported patients with plantar MC and discuss the clinical characteristics, differential diagnosis, and treatment of plantar MC. The patients were immunocompetent and the median age at diagnosis was 21 years. Although the plantar MC were asymptomatic in

Seven Bennett's wallabies (Macropus rufogriseus rufogriseus) presented within a period of several months with onychodystrophy, onychomadesis, and severe digital tumefaction. Histopathologic findings included a pseudocarcinomatous hyperplasia of the claw matrix surrounding a cavity filled with keratin and septate hyphae stained with periodic acid Schiff reagent. The fungal species Chrysosporium keratinophilum was identified on cultures. The wallabies were orally treated with ketoconazole (15 mg/kg s.i.d.) for 20 wk. Material and enclosures were cleaned and sprayed with 0.2% enilconazole solutio

Study of a rare case of Elephantiasis Nostra in verrucous form on the dorsum of the foot of an 80 year-old male with a history of recurrent erysipelas infection. The vegetant, confluent lesions on the foot resemble Trumpet Coral (Caulastrea curvata).

The systemic treatment of fungal infections has changed considerably over the past 10 to 20 years. Though griseofulvin was introduced in the late 1950s and was at the time the only Food and Drug Administration (FDA)-approved drug in the United States for the systemic treatment of onychomycosis, its cure rate seldom exceeds 40%. Newer drugs appear to be reducing treatment times, improving cure rates with a minimum of side effects, and achieving long-term remissions in recalcitrant infections. Itraconazole was FDA approved in 1995, and terbinafine was FDA approved in 1996. Both have been used sa
