Traumatic chylothorax is a rare complication of fracture-dislocation of the thoracic spine. Not treated adequately, it will be associated with high mortality and morbidity. A review of the English literature shows that bilateral traumatic chylothorax is rarely discussed. Complete posterior fracture-dislocation of the lumbar spine at L4-L5 level occurred in an 18-year-old male patient, who remained neurologically intact. Seventeen patients who had suffered a Bennett's fracture-dislocation have been reviewed with an average follow up of 26 years. All were treated conservatively. At review, only seven patients had symptoms, but all had a decreased range of movement and grip strength. Twelve had a characteristic deformity in the hand. Radiographs showed persistent subluxation of the first carpo-metacarpal joint and marked degenerative changes. We suggest that, in the light of the poor long-term outcome, this injury should not be managed conservatively but by some operative means.

A 41-year-old agricultural sustained a lumbosacral fracture-dislocation when a tree fell across his back. The initial anterior displacement of half the depth of the body of L5 progressed to three-quarters of the body over a 2-year period. Neurological deficit was minimal and the management was conservative. Spontaneous arrest of the displacement occurred by anterior sacral buttressing and the patient has returned to his previous heavy work.

PURPOSE: Bosworth fracture-dislocations (BF) with entrapment of a fibular fragment behind the posterior rim of the distal tibia are rare but potentially serious injuries to the ankle.
MATERIALS AND METHODS: We analyzed the radiographs of 23 consecutive patients with a mean age of 44 years who were treated for BF. All patients underwent routine radiological examination of the ankle and presence of a BF was confirmed intraoperatively in 22 cases and / or with CT in 15 cases.

The Monteggia injury, a combination of an ulnar fracture and proximal radial dislocation, is an uncommon but high-stakes paediatric injury that is frequently missed at initial presentation. Early recognition is crucial, as delayed diagnosis is associated with malunion, instability, restrictions in rotation, and deformity. The aim of this review is to highlight key concepts relevant to exams and clinical practice, including the epidemiology, anatomy, classification, and evidence-based management of both acute and neglected Monteggia injuries.

Volar fracture-dislocations of the proximal interphalangeal joint are rare injuries caused by an axial force on a flexed digit resulting in an avulsion fracture of the dorsal lip of the middle phalanx with volar dislocation of the joint.
