[Open reduction and internal fixation via a posterior approach for posterior fractures of tibial plateau].

OBJECTIVE: To discuss the fracture patterns, operative procedures and clinical results of open reduction and internal fixation via a posterior approach to treat posterior fractures of tibial plateau. METHODS: From June 2008 to February 2011, 8 patients with posterior tibial plateau fractures treated with posterior approach, were reviewed retrospectively. There were 5 males and 3 females, with an average of 41.1 years ranging from 23 to 55. Of the 8 cases, 5 cases were caused by traffic accidents, 3 caused by fall. Two cases of posterior coronal fractures combined with avulsion of posterior cruc

Open reduction and internal fixation with 90-90 plating of bicolumn distal humeral fractures.

A bicolumn intra-articular fracture is the most challenging distal humeral fracture to treat. Open reduction and internal fixation of these fractures is best achieved using either 90-90 medial-lateral or parallel opposing plates. Either technique can be used to treat most patients, although in some instances, one technique may be preferred over another. The goal of treatment is accurate anatomic restoration and stable fixation that will permit early range of motion.

Optimal internal fixation for femoral neck fractures: multiple screws or sliding hip screws?

INTRODUCTION: The number of hip fractures is likely to exceed 500,000 in the United States and 88,000 in Canada annually over the next 40 years. Hip fractures are associated with a 30% mortality rate at 1 year and profound temporary, and sometimes permanent, impairment of independence and quality of life. OBJECTIVES: Although much focus has centered around the comparison of arthroplasty versus internal fixation devices in the treatment of femoral neck fractures, the optimal approach for internal fixation has been largely ignored. Identifying the optimal technique for internal fixation could r

Esthetic positioning of rigid internal fixation in tripod zygomatic fractures: an innovative surgical technique.

Tripod displaced zygomatic fractures are generally treated with rigid internal fixation at the intraorbital rim (IOR), frontozygomatic (FZ) suture, and zygomaticomaxillary buttress. Intraorbital rim fixation is associated with complaints of poor esthetic results and miniplate intolerance. Although different solutions were previously reported as 2-point fixation or resorbable fixation at the IOR, a 3-point fixation is considered the best choice for maintaining an optimal zygomatic stability after reduction. Consequently, the best goal is to perform a surgical technique that provides a 3-point f

Treatment of select open calcaneus fractures with reduction and internal fixation: an intermediate-term review.

BACKGROUND: Open calcaneal fractures are devastating, high-energy injuries frequently occurring in multiply-injured patients. Unsatisfactory outcomes with significant complications rates are frequently reported. Our purpose was to assess outcomes of delayed ORIF for open calcaneal fractures with medial hindfoot wounds and limited posterior facet comminution. MATERIALS AND METHODS: Following acute management of the open fractures, seven patients were determined to have stable medial hindfoot wounds and posterior facet comminution of three or fewer fracture fragments. The average age was 32.5 y

Soft tissue sleeve approach to open reduction and internal fixation of proximal phalangeal fractures.

Proximal phalangeal fractures often lead to motion loss at the proximal interphalangeal joint primarily because of adhesions between the zone 4 extensor tendon and the fracture site. This most commonly manifests as an extensor lag at the proximal interphalangeal joint, but can also include incomplete recovery of flexion range. The occurrence of the fracture alone likely generates some degree of insurmountable motion loss. Additional motion loss may be iatrogenic based on the specific surgical technique and rehabilitation strategy. It has been promoted that percutaneous pinning methods, as oppo

[Treatment of femoral shaft fractures in polytrauma patients using Mitkovic type internal fixator].

Polytrauma remains a major social, economic and medicine affliction. Successful surgical treatment of polytrauma patient's requires an approach predicated on prioritizing injuries. An isoleted fractures rarely poses any threat but in association with multiple injuries, a fractures assumes greater significance. Proper management of femur fractures in po-lytrauma can gretly reduce the mortality and morbidity. In this work the results of the operative treatments of the femoral shaft fractures in polytrauma patients using Mitkovic type internal fixator (14 patients) and Kuntscher nail (9 patients)

Open reduction and internal fixation compared with excision for unstable displaced fractures of the radial head.

PURPOSE: To determine if excision of the radial head for treatment of an unstable, displaced fracture is associated with a higher rate of early complications or late arthrosis as compared with open reduction and internal fixation (ORIF). METHODS: Unstable, displaced fractures of the radial head treated with either excision (15 patients) or ORIF (13 patients) were compared. Implants were removed routinely during the study period. Three patients had instability after radial head excision: 2 were treated in a cast and 1 had temporary pinning of the ulnohumeral joint. Two patients in the ORIF coh

Tourniquets may increase postoperative swelling and pain after internal fixation of ankle fractures.

UNLABELLED: Tourniquets frequently are used in orthopaedic surgery to provide a bloodless field. There is still controversy among authors regarding the benefits and potential risks of using a tourniquet. The objective of this prospective randomized study was to quantify the effect of tourniquet use on postoperative swelling, pain, and range of motion after open reduction and internal fixation of ankle fractures. Fifty-four patients with closed ankle fractures were treated surgically; 26 patients were operated on using a thigh tourniquet (Group A), and 28 patients had surgery without the use of

Management of intra-articular fractures of the proximal interphalangeal joint by internal fixation and bone grafting.

INTRODUCTION: Intra-articular fractures of the proximal interphalangeal joint are usually treated by one of many methods of external fixation, but this does not restore the articular surface. We describe a procedure where bone grafting and internal fixation are used to restore the articular surface. PATIENTS AND METHODS: A prospective study was conducted in which three patients underwent surgical treatment for this fracture. We describe a surgical procedure where bone graft harvested from the distal radius is packed through a diaphyseal window to restore the articular surface. This is followe

Internal fixation of displaced proximal humeral fractures with two one-third tubular plates.

BACKGROUND: Operative treatment of displaced proximal humeral fractures often results in poor functional outcome. We report a technique that provides improved rotational stability of the fracture and thus allows early functional treatment. METHODS: Seventy-one consecutive patients (67 +/- 18 years) with displaced fractures of the proximal humerus (two-part, n = 10; three-part, n = 41; four-part, n = 20) were treated by open reduction and internal fixation using two one-third tubular plates on the anterior and lateral aspects of the proximal humerus. Passive motion was started on the third pos

Internal fixation of valgus-impacted humeral head fractures using the screw-wiring technique.

Between February 1996 and July 2000 eight patients (ages 35-74 years) with nine severely impacted humeral head fractures were operated on with the aim of preserving the humeral heads. Patients were treated surgically with open reduction, autologous bone grafting, and internal fixation with a screw-wiring technique. There were no complications and all patients were satisfied with the results. After 10-63 months there was no evidence of nonunion, arthritis, avascular necrosis, or loosening of implants. The mean Constant score was 83 (66-97) points. Results of this study support the observation t

Open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus.

BACKGROUND: Controversy persists concerning the preferred treatment of displaced fractures of the proximal part of the humerus. The present study was undertaken to evaluate the results of open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus and the functional limitations of patients in whom avascular necrosis of the humeral head develops as a complication of this fracture. METHODS: We assessed the intermediate and long-term results for sixty patients with a three or four-part fracture of the proximal part of the humerus who had undergone op

Decompression and stable internal fixation of femoral neck fractures in children can affect the outcome.

Pediatric femoral neck fracture is rare and known to be associated with a high complication rate. The effect of early decompression and operative fixation is not frequently reported in the literature. This is a retrospective study of 14 patients younger than 16 years with traumatic fractures of the femoral neck and intertrochanteric region over a 10-year period with a follow-up of 2-11 years (mean, 4.6 years). Road traffic accidents and falls from a height were the main causative factors, and the fractures were subclassified into transcervical, 58%; intertrochanteric, 29%; transepiphyseal, 7%;

Wound-healing risk factors after open reduction and internal fixation of calcaneal fractures.

This retrospective study investigated outcomes of wound healing in a series of 63 consecutive patients with 64 fractures of the calcaneus who underwent open reduction and internal fixation done by two surgeons experienced in this fracture during a 3-year period. Thirty-nine patients were managed preoperatively as outpatient referrals before surgery. Twenty-four patients were admitted directly to the trauma service and were managed as inpatients preoperatively. Minimum patient follow-up was 6 months, with an average follow-up of 18 months. A trend correlating the time between injury and operati
