The abducens nerve’s long intracranial course makes it highly vulnerable to injury during traumatic brain injury. While traditionally attributed to skull base fractures, high-resolution MRI has identified avulsion in cistern as a significant, yet underrecognized, mechanism, which can occur even without adjacent fractures. The study retrospectively analyzed 31 patients with traumatic abducens nerve palsy and radiologically suspected or surgically confirmed cisternal segment avulsion. Injuries were classified via MRI as complete or partial/suspected avulsion. Patients received different treatmen

Traumatized teeth present a clinical challenge with regard to their diagnosis, treatment plan and prognosis. The injuries to the permanent dentition include crown and root fractures, subluxations, luxations and tooth avulsion. Common complications are pulp necrosis, periapical pathosis and root resorption. This article will describe new clinical modalities for the treatment of avulsed teeth and discuss the rational for their use.

BACKGROUND: Fixation of the small bony fragments of the phalanges is often difficult. In this study a clinical and radiological evaluation was carried out after operative treatment using the mini-hook plate.
PATIENTS AND METHODS: Between 2003 and 2006 a total of 36 fractures were treated operatively using the mini-hook plate. Of the patients 24 had an basal avulsion fracture of the distal phalanx and 11 patients (12 fractures) had other bony avulsion fractures of the phalanges. The patients were evaluated clinically and radiologically as well as using the disabilities of the arm, shoulder and

OBJECTIVE: To investigate the clinical effect of vacuum sealing drainage (VSD) on late-stage large skin avulsion injury with infection.
METHODS: From May 2007 to August 2008, 9 patients with large-area skin avulsion injury and infection were treated. There were 1 male and 8 females aged 9-52 years old (median 27 years old). All patients suffered from closed skin avulsion injury involving the lower back, buttock, and part of the thigh. The injury area varied from 30 cm x 25 cm to 92 cm x 38 cm. The time between injury and hospital admission was 15-23 days. The skin avulsion injury was complica

Traumatic female urethral avulsion in the absence of a pelvic fracture is an exceedingly rare entity, with no consensus on its' management. Here, we present a 35-year-old pregnant woman with severe anterior vaginal wall laceration and complete urethral avulsion secondary to straddle injury. Management consisted of primary urethral and vaginal repair.

BACKGROUND: The importance of the apophyseal plates during growth is often underestimated. They act as a muscular insertion and influence the joint mechanics by the load-dependent change in shape.
PATHOMECHANISMS: An anatomically functional adaptation occurs as protection from overloading. In special kinds of sports with highly dynamic movements, sudden changes of direction and eccentric/concentric muscle activities the resulting stress may exceed the strength of the apophyseal plate. In adolescence this results in a total or partial tearing of the apophysis in the sense of an avulsion injury

CASE: A 41-year-old man presented with a transverse patella fracture and proximal patellar tendon avulsion after a fall from standing. Disruption of the extensor mechanism of the knee at multiple points is rare. He was treated operatively for his patella fracture and patellar tendon avulsion but experienced early failure of the patellar tendon fixation requiring reoperation. Both components of injury ultimately healed, and he returned to function.
CONCLUSION: This case describes a rare presentation of an uncommon injury pattern affecting the extensor mechanism. This is the first report to des

A 13-year-old boy sustained an injury to both knees upon landing after a forceful jump in a soccer game. Plain radiography and magnetic resonance imaging demonstrated bilateral distal patellar tendon avulsions without fracture of the tibial tuberosities and the physes. To our knowledge, this particular injury has not been previously described in the literature. Open surgeries and internal fixation were performed with excellent functional outcome. This type of injury was similar to the well-recognised acute tibial tuberosity avulsion fracture in terms of the pathogenesis and treatment. We propo

BACKGROUND: Traumatic pneumocephalus is commonly encountered after basal skull fractures and rarely associated with blunt chest trauma. Here, we report a case of pneumocephalus caused by traumatic pneumothorax and brachial plexus avulsion.
CASE PRESENTATION: A 20-year-old male was admitted to our hospital following a motorcycle accident with complete paralysis of the right upper limb. 2 days later, follow-up computed tomography revealed a slight right pneumothorax, pneumomediastinum around the neck, and intracranial air without skull fracture. Air migrates into the subarachnoid space through

PURPOSE: To assess the pathogenesis of canalicular lacerations.
METHODS: This is a retrospective, clinical case series of 236 patients who sustained a canalicular laceration. All patients who presented to the oculoplastic service of 3 individuals (D.R.J., S.M.G., L.A.M.) from May 1, 1998 to September 30, 2007, with a canalicular laceration were included in the study. Case histories were carefully reviewed in an attempt to classify the mechanism of injury as: "direct (penetrating) injury," "indirect (avulsive)," or "diffuse (avulsive)." Associated injuries (floor fractures, soft tissue lacerat

Most of the previously reported lumbosacral nerve root avulsions presented with pseudomeningoceles at the time of delayed initial imaging. We report a case of traumatic lumbosacral nerve root injury associated with an isolated femur fracture and demonstrate the evolution of pseudomeningoceles following nerve root avulsions and edema in the perineural fat identified on the initial MR imaging.

All perilunate fracture-dislocations combine ligament ruptures, bone avulsions, and fractures in a variety of clinical forms. The most frequent is the dorsal trans-scaphoid perilunate dislocation. In rare cases, however, these dislocations also have been associated with capitate fractures, triquetral fractures, or lunate fracture. We report a combined scaphoid and lunate fracture of the wrist that was not associated with perilunate dislocation.

Avulsion fractures of the pelvic apophyses are a result of repetitive strain injuries or sudden, forceful eccentric or concentric contractions of corresponding muscle groups in the leg. Using a case study approach, we present the clinical and radiological features, and management of a 14-year-old boy who presented to our hospital with an avulsion fracture of the anterior inferior iliac spine. The literature on the subject, along with the management of the condition, is reviewed and presented. A condition often treated nonoperatively, the focus of treatment is based on effective nursing and reh

We describe a case report of dorsal radiocarpal dislocation with fracture of the radial styloid process. A 21-year-old right-handed male was injured in a fall while snowboarding. Plain radiographic findings revealed dorsal dislocation of radiocarpal joint with fracture of the radial styloid process; we performed CT studies to examine the radius fracture in greater detail. These findings revealed a radial styloid fracture with avulsion fracture of dorsal articular margin of the distal part of the radius. Surgical treatment was performed 4 days after the injury. However, it was impossible to red

OBJECTIVE: The purpose of this report is to describe the imaging findings of nontraumatic avulsions of the pelvis.
CONCLUSION: A diagnosis of avulsion fracture of the pelvis in an adult without appropriate history of substantial trauma must raise the suspicion of an underlying malignancy. If a patient has not been diagnosed as having a primary neoplasm, additional imaging evaluation is recommended. Also, biopsy may be considered in the proper clinical setting.

INTRODUCTION AND IMPORTANCE: Simple closed lateral elbow dislocation is rare, often associated with neurovascular injury and closed reduction may be difficult. Open complex postero-lateral dislocation has not been previously reported.
CASE PRESENTATION: We present a case of open left posterior lateral elbow dislocation secondary to assault with a cutlass causing an avulsion fracture of the lateral epicondyle and common extensor origin tendon which prevented congruent reduction until the avulsed fragment was fixed with a bone screw.
CLINICAL DISCUSSION: The patient had hemorrhagic shock from

Nail bed avulsion injuries associated with a distal phalanx fracture are very common injuries occurring in industrial workers dealing with heavy machines. Often patients come with an open wound that needs a thorough wash and reduction of fracture with manipulation and traction along with suturing of the nail bed. The study was conducted on 20 patients with nail bed avulsion injuries associated with distal phalanx fracture. Patients were treated by a vertical figure-of-eight suturing of the nail plate with the nail bed after a reduction of the distal phalanx fracture. Patients were followed up

Mallet finger is a common injury involving either an extensor tendon rupture at its insertion or an avulsion fracture involving the insertion of the terminal extensor tendon. It is usually caused by a forceful blow to the tip of the finger causing sudden flexion or a hyperextension injury. Fracture at the dorsal aspect of the base of the distal phalanx is commonly associated with palmar subluxation of the distal phalanx. Most mallet finger injuries are recommended to be treated with immobilisation of the distal interphalangeal joint in extension by splints. There is no consensus on the type of
