Unilateral transnasal endoscopic approach to frontal sinuses: Draf IIc.

For chronic sinusitis surgery, the Draf III approach provides a common median drainage pathway for bilateral frontal sinuses from orbit to orbit. The Draf IIb provides unilateral drainage from orbit to septum. In several cases, inclusion of the nasal and frontal sinus septum in a Draf IIb was advantageous without extension to the opposite frontal recess. The proposed nomenclature is Draf IIc. This study was designed to (1) develop a surgical option for chronic frontal sinusitis where access to one frontal recess is limited or unnecessary and (2) minimize unnecessary surgical manipulation of un

Craniofacial correction of occipital encephalocele.

Occipital encephaloceles are commonly seen birth defects. The incidence is 1 in every 10,000 live births. These defects represent about 15% of the total birth defects reported and treated in the United States. The birth defects encountered today comprise over 50% of all pediatric admissions to general hospitals. Six patients with occipital encephaloceles are treated by a combined craniofacial approach. The corrective measure allows reduction of the herniated encephalocele and correction of the craniofacial deformity in the same operation procedure.

Delayed development of frontal mucocele after fronto-orbital advancement in a child with craniosynostosis.

BACKGROUND: Sinus mucoceles rarely develop as a consequence of inadequate sinus ventilation that arises due to inflammation, allergy, polyps … d suggest that the mucocele development after fronto-orbital advancement supports the hypothesis of frontal bone-inducing role in frontal sinus development.

Treatment of frontal sinus osteoma using a craniofacial approach.

Osteoma is one of most common benign tumors of the nose and paranasal sinuses, with the frontal sinus being its most frequent location. It may be locally destructive and aggressive with possible intracranial complications. Osteoma of the frontal sinus comprises 57% of all osteomas in the paranasal sinuses. In 1939, Childrey reviewed 3,510 consecutive sinus radiographs taken for any reason and found an incidence of 0.43% of paranasal sinus osteomas. There are many operative approaches for frontal sinus osteomas, such as external frontoethmoidal approach and osteoplastic frontal sinusectomy, bot

Surgical technique for the prevention of cerebrospinal fluid leakage after bifrontal craniotomy.

BACKGROUND: Cerebrospinal fluid leakage and meningitis caused by frontal sinus (FS) exposure are characteristic complications of bifrontal craniotomy used for treating skull base tumors and anterior communicating artery aneurysms. Prevention of these complications is of utmost importance. We describe in detail our procedure for sealing exposed FSs during bifrontal craniotomy and present the results and outcomes of the procedure. METHODS: A total of 51 consecutive patients who had undergone bifrontal craniotomy for tuberculum sellae meningiomas, craniopharyngiomas, anterior cerebral artery ane

Extensive subgaleal abscess and epidural empyema in a patient with acute frontal sinusitis.

Acute frontal sinusitis can be a serious condition because of its potential life-threatening complications. These complications, including spread of infection to the frontal bone and intracranially, require prompt diagnosis and intervention to avoid morbidity and mortality. We report a case of acute frontal sinusitis in a 16-year-old girl who presented with fever, severe headache, and vomiting of 3 days' duration. Generalized fluctuant swelling of the nasal root, and bilateral supraorbital and frontoparietal regions was noted. Computed tomography (CT) demonstrated left pansinusitis, extensive …

Contouring of the forehead irregularities (washboard effect) with bone biomaterial.

Calvarial vault defects may be repaired with autologous bone or alloplastic materials, such as methyl methacrylate, hydroxyapatite, titanium, or porous polyethylene. The criterion standard for repairing small cranial defects is autogenous bone from iliac crest or split calvarial grafts. However, autogenous grafts may result in donor-site morbidity, increased operative time, reabsorption, blood loss, and additional time for recovery. An alloplastic material should have some ideal properties, including easy adaptation, biocompatibility, which permit ingrowth of new tissue, stability of shape, an

[Ipsilateral exophthalmos in chronic extradural fluid-hematoma, case report (author's transl)].

A case of chronic extradural hematoma with liquid contents that had showed ipsilateral exophthalmos was reported. A 10-year-old girl fell and struck on her head. Neurological examination, on admission to our hospital on tenth day after the injury, revealed left exophthalmos and slight right hemiparesis. Plain craniogram showed linear fractures of frontal bone and orbital roof on left side. Left carotid angiogram and computerized tomogram revealed a huge extradural hematoma with liquid contents in the left frontal region. Extirpation of the hematoma was performed on the twenty-third day after t…

Multiple primary cranial Ewing's sarcoma in adulthood: case report.

OBJECTIVE: Ewing's sarcoma is a malignant bone tumor occurring most frequently in the long bones and flat bones as a solitary lesion during the first 2 decades of life. Ewing's sarcoma and peripheral primitive neuroectodermal tumor have recently been considered to be the same entity because of histological and molecular similarities. CLINICAL PRESENTATION: A 25-year-old man presented with swelling in the right parietal region. Magnetic resonance imaging scans showed 2 mixed intense mass lesions in the right parietal and left frontal areas with heterogeneous gadolinium enhancement. A computed

[Preoperative clinical, radiological and nuclear medical diagnostic and intra-operative findings in frontobasal cerebrospinal fluid fistula (author's transl)].

In 71 patients with rhinosurgical closed fronto-basal cerebrospinal fluid fistula the pre- and intra-operative findings are compared. Cerebrospinal fluid rhinorrhea in 80% and purulent meningitis in 44% were seen. In X-ray 52% fractures and 32% cloudiness of the paranasal sinuses were found, there were 21% false negative results. By nuclear medical examination the cerebrospinal fluid leak was identified in 59%, the result was false negative in 15%. The site of the cerebrospinal fluid fistulas could be preoperative determinated by clinical signs in 96%, by X-ray in 22%, and by scintigraphy in 4
