Dangerous diplopia: A case of pansinusitis.

PURPOSE: To report a case of pansinusitis after swimming in a common pool. CASE REPORT: Acute sinusitis is ranked the fifth-most common indication for antibiotic prescriptions.2 Although sinusitis is often diagnosed clinically, cases that are resistant to conventional antibiotic therapy or recurrent cases may require diagnostic imaging in order to confirm the diagnosis. The complications of sinusitis, though rare, may lead to serious consequences if not diagnosed and treated early. We report a 33-year old man with pansinusitis presenting with a sudden onset of peripheral gaze diplopia associa

[Intracranial complications from ENT infections].

Sinonasal and temporal bone infections may extend to the skull, skull base, meninges, pericerebral spaces, brain parenchyma, dural sinuses, deep cerebral or cortical veins, intracranial arteries and cranial nerves either via contiguous or hematogeneous spread. The site of infection dictates the sites of potential complications: orbital with ethmoid sinusitis, cavernous sinus thrombophlebitis and oculomotor palsies with sphenoid sinusitis, transverse sinus thrombophlebitis with mastoiditis and superior sagittal sinus thrombophlebitis with frontal sinusitis. All may result in brain abscess. Cong…

A sheep model for the study of biofilms in rhinosinusitis.

BACKGROUND: Bacterial biofllms have been shown in chronic diseases such as cystic fibrosis, cholesteatoma, and otitis media with effusion. Recently, their detection on the mucosal tissue of sinusitis patients has implicated them in the pathogenesis of this condition. We present an animal model using sheep experimentally infected with Staphylococcus aureus to study the possible association between biofilm and sinusitis. METHODS: Twenty-four sheep underwent bilateral endoscopic sinus surgery to identify their frontal ostia. The frontal sinuses were treated in one of the following ways according

Giant destructive sinonasal polyposis.

INTRODUCTION: Authors report their clinical experience in managing a 46-year-old male patient with long lasting nose breathing difficulties caused by nasal obstruction due to a large bilateral tumor masses in both nasal cavities. CASE OUTLINE: Physical examination, laboratory and biochemistry analyses, as well as computed tomography showed an inhomogeneous soft-tissue tumor mass completely filling both nasal cavities, maxillary, ethmoidal, sphenoidal, and frontal sinuses on both sides, accompanied by destruction of bony walls of all sinuses. Preoperative histopathology analysis showed a polyp

Giant frontal sinus osteoma and its potential consequences: illustrative case.

BACKGROUND: Osteomas of the paranasal sinuses are benign, slow-growing bone tumors that can cause a variety of clinical features depending on their size and location. Most osteomas are asymptomatic and located in the frontal sinus. In rare cases, they may grow to extend into the cranial or orbital cavities, resulting in atypical presentations. The authors presented an aggressive case of a frontoethmoidal sinus osteoma with intracranial extension of an inflammatory sinonasal polyp. OBSERVATIONS: A 30-year-old man with a history of chronic sinusitis presented to the hospital after three episode

Synchronous multicentric bilateral sinonasal inverted papilloma and frontal sinus osteoma.

Inverted papilloma is generally considered a benign unilateral sinonasal tumor. Its synchronous bilateral multicentric occurrence is extremely rare. A 22-year-old male patient presented with stage III inverted papilloma involving both ethmoid sinuses, both frontal sinuses, and cribriform area. The patient also had a large osteoma emanating from the basal part of the frontal sinus septum, which completely obstructed both nasofrontal recesses, leaving no communication between the sinuses and the nasal cavity. The frontal sinus septum was intact, so there was no communication between the 2 sides

Comparison of airflow characteristics after Draf Ⅲ frontal sinus surgery and normal person by numerical simulation.

Draf III frontal sinus surgery is confirmed as an effective surgical treatment for refractory sinusitis and frontal sinus tumors, etc. Although it has been reported to improve symptoms and reduce the recurrence rate of polyps significantly, the study of airflow characteristics in frontal sinus after Draf III is still rare, especially compared with normal person. This study was designed to describe the airflow characteristics of frontal sinus after Draf III procedure and differences compared with normal subject. One patient with refractory sinusitis received Draf III procedure15 months ago, and

Chronic anterior sinus atelectasis due to middle meatus retraction.

INTRODUCTION: Chronic sinus atelectasis is a rare but classical etiology in maxillary sinusitis. Advanced forms induce orbital deformities. Here we report a case of chronic maxillary-ethmoidal-frontal sinus atelectasis, and detail treatment. CASE REPORT: A 59 year-old man was referred for recurrent right periorbital pain with 2 years' progression, resistant to local corticosteroid treatment. Clinically, there was right middle turbinate lateralization and ipsilateral enophthalmos. CT showed filling of the right maxillary, anterior ethmoidal and frontal sinuses, with characteristic anterior chr

Pott's Puffy Tumor in an Adult Female: A Case Report in a Rare Demographic.

Pott's puffy tumor (PPT) is a localized forehead swelling with underlying subperiosteal abscess formation and osteomyelitis of the frontal bone. It is a rare complication of frontal sinusitis, and it is especially rare in adult females. A careful review of existing literature identified only 17 cases in adult females. Treatment requires antibiotic therapy and often surgical drainage/debridement. Here, we present a case of a 76-year-old female diagnosed with PPT. She was placed on amoxicillin-clavulanate 875 mg twice daily for six weeks. Her symptoms resolved with the antibiotic course, and she…

A Rare Case of Orbital Cellulitis with Progressive Calvarial Osteomyelitis.

Orbital cellulitis is a potentially sight and life-threatening complication of acute sinusitis, and the association with osteomyelitis is rare in the era of antibiotic-use. A 13-year-old girl presented with coryzal symptoms and severe headache, with a CT head being consistent with a diagnosis of pansinusitis and orbital cellulitis with abscess formation. She proceeded to have surgical drainage through a combined endoscopic and external approach to intraorbital abscess drainage with frontal trephine. She was also diagnosed with progressive calvarial osteomyelitis involving the right frontal bon

[Optimization of reconstructive surgery on the frontal sinus].

Results of the treatment of three patients with frontal sinus injuries of different severity demonstrate that it is desirable to start surgical intervention for their management in the early period after injury, i.e. as soon as the acute process is completed. The extent of surgery should be estimated based on the results of computed tomography of the accessory sinuses of the nose in the preoperative period. The frontal sinus was approached through the modified Zutter skin incision which allowed to avoid a cosmetic facial defect, an injury to temporal muscles and frontal branch of the facial ne

Growth and microanatomy of the paranasal sinuses in two species of New World monkeys.

Paranasal sinuses of living apes and humans grow with positive allometry, suggesting a novel mechanism for bone enlargement. Here, we examine the paranasal sinuses of the owl monkey (Aotus spp.) and a tamarin (Saguinus midas) across postnatal development. The prediction that paranasal sinuses grow disproportionately faster than the main nasal chamber is tested. We used diffusible iodine-based contrast-enhanced computed tomography and histology to study sinuses in eight Aotus and three tamarins ranging from newborn to adult ages. Sinuses were segmented at the mucosa-air cavity interface and mea

Use of medical aural and encephalic glue-soaked gelfoam for frontal sinus repair: a single-centre experience.

BACKGROUND: Frontal sinus (FS) perforation is a common complication in frontal craniotomy. The primary goal of treatment is to seal the FS without destroying physiological function. OBJECTIVE: This article describes a new FS cavity reconstruction technique using medical aural and encephalic glue (EC glue)-soaked gelfoam. METHODS: Between 2007 and 2012, 118 patients underwent FS reconstruction using EC glue-soaked gelfoam. The FS cavity was reconstructed in all patients and no patient experienced intracranial infection, frontal sinusitis, or cerebrospinal fluid (CSF) leakage. RESULTS: Restor…

Frontal sinus "mega-trephination" in a tertiary rhinology practice.

BACKGROUND: Frontal sinus trephination is traditionally performed through a small cutaneous incision and osteotomy, allowing irrigation of the frontal sinus. Utilizing the trephination osteotomy for endoscopic visualization and surgical manipulation requires a larger opening. This "mega-trephination" is thought to carry an increased risk of cosmetic deformity given the increased bony removal at the anterior table. The purpose of our study was to clarify the risks of frontal sinus mega-trephination and examine how this technique is incorporated into a modern, tertiary care rhinology practice.
