Tibio-femoral loading during human gait and stair climbing.

Surgical intervention of the knee joint routinely endeavors to recreate a physiologically normal joint loading environment. The loading conditions resulting from osteotomies, fracture treatment, ligament replacements, and arthroplasties of the knee are considered to have an impact on the long term clinical outcome; however, knowledge regarding in vivo loading conditions is limited. Using a previously validated musculoskeletal lower limb model, we predicted the tibio-femoral joint contact forces that occur in the human knee during the common daily activities of walking and stair climbing. The a

Gait analysis after talonavicular joint fusion: 2 case reports.

The purpose of this study was to analyze the alterations in frontal-plane hindfoot motion and in plantar pressure in 2 patients (3 feet) after talonavicular joint fusion. Three-dimensional movement of the hindfoot was recorded by using an electromagnetic motion analysis system. The results indicated that frontal-plane hindfoot motion of the operative foot was substantially reduced during the stance phase of walking. In addition, each patient's frontal-plane hindfoot angle while in the resting standing position was decreased and plantar pressure under the medial longitudinal arch was reduced af

Gait mechanics in chronic ACL deficiency and subsequent repair.

OBJECTIVE: To determine how normal gait patterns may change as a result of chronic anterior cruciate ligament deficiency and subsequent reconstructive surgery. DESIGN: Gait testing of 10 chronic anterior cruciate ligament deficient subjects prior to and 3 months following reconstructive surgery, and 10 uninjured controls. BACKGROUND: There is controversy whether persons with chronic anterior cruciate ligament deficiency develop a "quadriceps avoidance" pattern and how anterior cruciate ligament reconstructive surgery influences gait mechanics in these same individuals. METHODS: Gait analysi…

Effects of prophylactic ankle supports on pronation during gait.

We performed a gait analysis of 12 healthy subjects in order to analyse the rear foot pronation-supination angle and the shift of centre of pressure (COP) during walking. We used a video system and a force plate and examined the effects of semi-rigid Air-Stirrup brace, lace-up cloth RocketSoc brace, ankle taping and no bracing. Both the lace-up cloth brace and taping increased the maximum pronation angle during the stance phase. The lace-up cloth brace alone was associated with a larger maximum pronation velocity. With the lace-up cloth brace and taping, the COP was more laterally placed, allo…

Muscle power compensatory mechanisms in below-knee amputee gait.

OBJECTIVE: This three-dimensional and bilateral gait study on five below-knee amputees was undertaken to demonstrate the following: (1) how hip muscle powers can compensate for the lack of ankle function on the amputated side; and (2) how these compensatory mechanisms can influence muscle power activities in the sound limb. DESIGN: Gait data were assessed by an eight-camera high-speed video system synchronized to two force plates. The three-dimensional mechanical muscle powers were calculated at the joints of the lower limbs. Significant differences between each limb were determined using the…

Biomechanical assessment of gait in below-knee walking casts.

The introduction of modern synthetic casting bandages for splinting of fractures and soft tissue injuries has allowed the development of new casting techniques. Casts can be constructed with a greater degree of function so that controlled motion and stabilisation can be provided within the same cast. This study has shown that a very efficient gait can be achieved with modern synthetic bandages, if they are correctly applied. The authors have compared the gait of volunteer subjects fitted respectively with below-knee walking casts constructed from a rigid glass fibre bandage and a flexible glas…

Temporal-spatial feature of gait after traumatic brain injury.

The temporal-spatial characteristics of the gait of patients with traumatic brain injury (TBI) were investigated and compared with those of normal gait and the gait of stroke survivors. A slower walking velocity is evident in the TBI population when compared with normal. The average walking speed of TBI survivors is faster than that of stroke patients and is mainly related to a longer step length. TBI survivors produce a gait pattern with a prolonged stance period for the unaffected limb, without prolonged stance period for the affected limb, and a shorter step length for the unaffected limb.

Rhythmic facilitation of gait training in hemiparetic stroke rehabilitation.

Experimental and control groups of 10 hemiparetic stroke patients each underwent a 6 week, twice daily gait training program. The control group participated in a conventional physical therapy gait program. The experimental group trained in the same basic program with the addition of rhythmic auditory stimulation (RAS). Patients entered the study as soon as they could complete 5 strides with hand-held assistance. The training program had to be completed within 3 months of the patients' stroke. In the experimental group RAS was used as a timekeeper to synchronize step patterns and gradually entr…

Gait pattern in the early recovery period after stroke.

The gait patterns of eighteen patients who had had a single infarct due to obstruction of the middle cerebral artery were evaluated within one week after the patients had resumed independent walking and before a gait rehabilitation program had been initiated. Gait was analyzed with use of motion analysis, force-plate recordings, and dynamic surface electromyographic studies of the muscles of the lower extremities. The patterns of motion of the lower extremity on the hemiplegic side had a stronger association with the clinical severity of muscle weakness than with the degree of spasticity, bala…

Gait patterns in children with hemiplegic spastic cerebral palsy.

Twenty-six nonoperated spastic hemiplegic children underwent three-dimensional kinetic gait analysis. We identified five patterns based on sagittal plane kinetics. Group I had a minimal gait disturbance, a drop foot pattern, and normal kinetics, Group II had a flexed knee but normal hip extension, Group III a flexed knee and hip, Group IV knee hyperextension and tibial arrest, and Group V knee hyperextension and persistent ankle dorsiflexion. We concluded that Group I had weak anterior tibial muscles, and in stance Group II a functionally tight gastrocnemius, Group III a functionally tight gas…

Modelling and gait evaluation of asymmetrical-keel foot prosthesis.

The paper documents a new concept in prosthetic foot design. It is based on the capacity of a flexible keel to allow a greater medio-lateral … t least twice as active in storing energy compared with a completely symmetrical one. A preliminary gait study is carried out for a 24-year-old below-knee amputee fitted with the new design, the SPACE foot and a dynamic elastic response foot with a symmetrical keel. With the SPACE foot, there is a 14% increase in walking speed combined with a reduction …

[A gait disorder caused by hereditary motor-sensory neuropathy].

Two sisters suffering from a hereditary motor-sensory neuropathy (HMSN) are described. A walking disturbance, deformations of foot, muscle atrophy, areflexia of the legs with disturbance of sensibility are detectable of them. Each of them, their motor nerve conduction velocity (NCV) of all extremities was slowed. The younger of the two sisters had graver symptoms and her sural nerve biopsy exhibited a reduced number of myelinated nerve fibres, some distinguished axon-calibre and onion bulb-formations. The relation of those findings to the classification of HMSN is demonstrated and the importan

Associations of Laboratory-Based Gait Speed and Daily-Life Gait Speed With Frailty, Sarcopenia, and Life-Space Mobility in Community-Dwelling Older Adults.

OBJECTIVES: Although daily-life gait speed (DGS) may better reflect real-world walking than laboratory- or clinic-based gait speed (LGS), it is unclear whether DGS provides superior discriminatory value for clinical outcomes. We compared LGS and DGS on their discriminative abilities concerning the risk of prefrailty/frailty, probable sarcopenia, and restricted life-space mobility (RLSM) in community-dwelling middle-aged and older adults. DESIGN: Cross-sectional study. SETTING AND PARTICIPANTS: A sample of 1025 participants (mean [SD], 63 [7.3]years) completed a survey and fitness assessment,…

Dual-Task vs. Single-Task Gait Training to Improve Spatiotemporal Gait Parameters in People with Parkinson's Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: People with Parkinson's disease (pwPD) present alterations of spatiotemporal gait parameters that impact walking ability. While preliminary studies suggested that dual-task gait training improves spatiotemporal gait parameters, it remains unclear whether dual-task gait training specifically improves dual-task gait performance compared to single-task gait training. The aim of this review is to assess the effect of dual-task training relative to single-task gait training on specific gait parameters during dual-task tests in pwPD. METHODS: We conducted a systematic review and meta-an…

For subject-wise trained models, calibration on a single gait cycle per surface yielded a 70% increase in F1-score, the harmonic mean of precision and recall, while 10 gait cycles per surface were sufficient to match the performance of a random-wise trained model. Code to generate calibration curves may be fou …

Bilateral High Hip Center Provides Gait Parameters Similar to Anatomical Reconstruction: A Gait Analysis Study in Hip Replacement Patients With Bilateral Developmental Dysplasia.

BACKGROUND: Total hip arthroplasty in severe dysplasia is challenging due to diminished periacetabular bone stock and the highly placed femoral head. Although anatomical reconstruction of the hip, with required interventions such as subtrochanteric osteotomy and graft usage, is the main aim of the procedure, good long-term clinical outcomes of the high hip center technique have also been reported. Information regarding the effect of hip center placement on gait characteristics is limited; therefore, the aim of this study is to analyze the differences in gait parameters between the high hip cen…
