Changing global demography is resulting in older people presenting to emergency departments (EDs) in greater numbers than ever before. They present with greater urgency and are more likely to be admitted to hospital or re-attend and utilize greater resources. They experience longer waits for care and are less likely to be satisfied with their experiences. Not only that, but older people suffer poorer health outcomes after ED attendance, with higher mortality rates and greater dependence in activities of daily living or rates of admission to nursing homes. Older people's assessment and manageme

OBJECTIVES: As key team members caring for people with advanced illness, nurses teach patients and families about managing their illnesses and help them to understand their options. Our objective was to determine if nurses' personal healthcare experience with serious illness and end-of-life (EOL) care differs from the general population as was shown for physicians.
DESIGN: Observational propensity-matched cohort study.
SETTING: Fee-for-service Medicare.
PARTICIPANTS: Nurses' Health Study (NHS) and a random 20% national sample of Medicare beneficiaries aged 66 years or older with Alzheimer's

PURPOSE: Nursing home residents increasingly enter care with advanced frailty, multimorbidity, and functional dependence, underscoring the need for engaging, adaptable physical activity (PA) programmes. Join4Joy is a co-created approach designed to promote movement and social connection among older adults through enjoyable activities. This study examined its feasibility, acceptability, and preliminary effects across five nursing homes in Spain, France, and Germany.
METHODS: This multicentre mixed-method study (ClinicalTrials.gov: NCT06100835) was delivered in two phases. Residents aged ≥ 65 y

BACKGROUND: in 2020, a new form of coronavirus spread around the world starting from China. The older people were the population most affected by the virus worldwide, in particular in Italy where more than 90% of deaths were people over 65 years. In these people, the definition of the cause of death is tricky due to the presence of numerous comorbidities.
OBJECTIVE: to determine whether COVID-19 was the cause of death in a series of older adults residents of nursing care homes.
METHODS: 41 autopsies were performed from May to June 2020. External examination, swabs, and macroscopic and micros

UNLABELLED: PROBLEM AND QUESTIONS: The consequences for elderly patients with hip fractures are well known. In Switzerland, the introduction of diagnosis related groups (DRG) will bring additional challenges. New models of care, such as Geriatric Fracture Centres (GFC), may be the key to minimising negative outcomes. This study documents outcomes of hip fracture patients in the Swiss healthcare system, for use as baseline data prior to DRG- and GFC-implementation, and compares them to results reported in the literature, for example by Cooper (1997).
METHODS: This was a prospective cohort qual

BACKGROUND CONTEXT: Type II odontoid fractures (OF) are among the most common cervical spine injuries in the geriatric population. However, there is a paucity of literature regarding their epidemiology. Additionally, the optimal management of these injuries remains controversial, and no study has evaluated the short-term outcomes of geriatric patients presenting to emergency departments (ED).
PURPOSE: This study aims to document the epidemiology of geriatric patients presenting to EDs with type II OFs and determine whether surgical management was associated with early adverse outcomes such as

AIMS AND OBJECTIVES: To explore the experience of HCAs encouraging residents living in residential care to complete the sit-to-stand activity and to identify the strategies HCAs used to integrate the activity into their daily work routines.
BACKGROUND: Decreased mobility in advanced ageing is further reduced when entering a residential care facility. Interventions such as the sit-to-stand activity have been shown to have a positive effect on the mobility of older people. There is evidence to suggest that healthcare aides are able to support residents to complete the sit-to-stand activity as p

OBJECTIVE: To examine the relationship between cognitively stimulating activities and cognitive function among older adults in residential care homes.
METHODS: A convenience sampling method was used to recruit older adults from three residential care homes in East China. Data were collected on cognitive function, sociodemographic characteristics and participation in cognitively stimulating activities (foreign language learning, musical instrument skills, hobbies, smart product use, physical exercise and household chores). Statistical analyses included univariate analysis (rank sum test and ch

PURPOSE: Well meaning nursing facility staff may compromise the independence of older adults with dementia by performing activities of daily living for residents. We assessed the impact of teaching certified nursing assistants (CNAs) to use a graduated prompting procedure to increase dressing independence of persons with dementia.
DESIGN AND METHODS: This study was conducted in a seven-bed dementia care unit. Three residents and two CNAs participated in the study. We used a repeated-measures experimental design to evaluate the effect of a training workshop for CNAs on a strategy for increasin

The clinical diagnosis of delirium has traditionally been based on an assessment by one or more physicians. Because of the transient, ubiquitous, and fluctuating nature of the symptoms of delirium, however, this approach may be flawed. Therefore, we decided to compare diagnosis based on one assessment by a psychiatrist, diagnosis by a nurse clinician (using the Confusion Assessment Method [CAM] and multiple observation points), and diagnosis by consensus. The study subjects were 87 patients aged 65 and over who were admitted consecutively from the emergency department to the medical wards, and

AIM: This study aimed to investigate the associations between bed-rest time and food intake and between bed-rest time and constipation in older nursing home residents.
METHODS: We conducted a cross-sectional study using data from the Long-term Care Information System for Evidence (LIFE) database. We used data collected from older nursing home residents registered in the LIFE database between April 2022 and March 2023. We compared outcome data between the short-bed-rest group (≥9 h out of bed per day) and the long-bed-rest group (<9 h out of bed per day). The outcomes were the percentage of fo

The Home Hospitalization Programme was initiated in Jerusalem in 1991 to provide intensive medical care at home in order to prevent or shorten hospitalizations. The programme was based upon regular home visits by physicians, and nursing assessment to determine the need for regular nursing care. Primary-care physicians and nurses were renumerated by a global monthly fee, and were on 24-h call in addition to their periodic visits. Patients were recruited by senior geriatric physicians from acute hospital wards, as well as from the community, at the family doctor's request. Ancillary services ava

Evaluation and management of older adults within emergency care settings is often complex and has the potential for avoidable complications. In an effort to prevent harm to the patient, treatment strategies need to be multifactorial. For geriatric patients presenting with rectal prolapse, unique management strategies, including the use of granulated sugar and gentle pressure, may assist in the reduction of the prolapsed tissue. Additional interventions that are important to incorporate into the older adult's plan of care include gentle approaches to positioning, involvement of family/caregiver

PURPOSE OF THE STUDY: This small scale study examined the influence of an interprofessional (IP) evidence based patient-centered communication intervention (PCCI), delivered by trained nurses in collaboration with Speech Language Pathologists, on patient outcomes in an active stroke rehabilitation setting.
DESIGN AND METHODS: The setting constraints imposed the use of a single-group pretest and post-test design to determine the influence of the intervention on patient outcomes. Validated tools measured patient quality of life (QOL), depressive symptoms and satisfaction with care.
RESULTS: Pa

We propose a two-step process for the assessment of dementia using standardized instruments. The family physician performs a screening consisting of taking a medical history, gathering information from relatives and friends of the patient, and administering the combined Mini-Mental State Examination (MMSE) and Clock Drawing Test (CDT). Specialists examine patients with suspected dementia to confirm the diagnosis of dementia and, after a thorough differential diagnostic process, provide the family physician with recommendations for treatment. Specialists should perform neurological and psychiat

Clinical symptoms of heart failure in older patients can be similar to those of other diseases in geriatrics. Therefore, the diagnosis should be confirmed by echocardiography. The treatment of older patients with heart failure with reduced left ventricular ejection fraction (HFrEF) should be carried out in accordance with the guidelines with regular control of volume status, renal function and serum electrolytes. Due to the lack of effective treatment of heart failure with preserved ejection faction (HFpEF) blood pressure control as a measure of prevention is important. Patients can benefit fr

Due to the aging population the number of vertebral fractures will increase in the next years. In the acute care setting the pain treatment of patients with acute vertebral fractures is highly important. Part of the acute pain treatment is the early rehabilitative intervention of the treating team consisting of physicians, nurses and physiotherapists. In the acute care setting mostly rheumatologists, orthopaedic surgeons and physical therapy physicians are the treating physicians. Individual physiotherapy and patient communication and instruction are most important in this acute care setting.

Two women aged 60 and 81 years were reported by their GPs to the department of Outpatient Geriatric Psychiatry because they seriously neglected themselves and their dwellings and their cognitive functioning was markedly reduced. A management involving regular contacts, home care, assistance in money matters, structuration, diagnostics and checking for risks was instituted, coordinated by a social-psychiatric nurse, to enable timely intervention when necessary. The first patient deteriorated greatly and was committed to a psychiatric hospital, the second is living at home with home care. The di
