Identification of the Potential Pharmacotherapeutic Risk in Hospitalized Geriatric Patients.

INTRODUCTION: Geriatric patients need carefully tailored pharmacotherapeutic approaches to minimize the risks of inappropriate treatment and hospitalizations. In Slovakia, there is a lack of studies documenting the medication use patterns in hospitalized geriatric patients. The primary outcome of our study was to identify potential pharmacotherapeutic risks in hospitalized geriatric patients. METHODS: We performed a retrospective analysis of the medical records from 122 patients (65 years and older) hospitalized in the geriatric department. Potential pharmacotherapeutic risk was assessed by i

Comprehensive Approach of a Traumatized and Discolored Anterior Tooth in Geriatric.

The prevalence of dental trauma, particularly in permanent maxillary anterior teeth, is high and can significantly impact a patient's quality of life. Furthermore, the growing desire of elderly patients to maintain their teeth has led to an increased need for the performance of complex dental treatments. Traumatic injuries often lead to root resorption, pulp necrosis, as well as wide root canals and open apices due to disruption of tooth development. Therefore, precise strategies are required for working length determination, cleaning, and shaping, along with obturation. A 64-year-old female p

Contribution of Tinnitus Burden and Hearing Loss to Geriatric Depression.

OBJECTIVE: Determine if tinnitus is a potential confounding variable in the study of hearing loss and depression in older populations. METHODS: This is a retrospective cross-sectional study using data from the 2005 to 2006, 2009 to 2010 and 2017 to 2020 National Health and Nutrition Examination Survey. The subpopulation (n = 2352) was aged > 69 years. We selected a cutoff of 69 years to focus specifically on elderly adults; as a result, these findings are not generalizable to younger adults with tinnitus and hearing loss. A logistic regression model used depression as the binary response vari

Hallways Feel Like Homelessness: The Geriatric Boarder Experience.

BACKGROUND: Boarding in the Emergency Department (ED) is associated with increased delirium, morbidity, and mortality. Geriatric patients are especially vulnerable to these harms and are more likely to board for a prolonged time. Although multiple studies have investigated clinical outcomes for older adults, we present the first study examining the subjective experience of geriatric boarders through qualitative methods. METHODS: We conducted semi-structured interviews between June 2024 and July 2024 at a large urban academic hospital. We recruited patients over the age of 65 boarding in the E

Immunosenescence and the Geriatric Giants: Molecular Insights into Aging and Healthspan.

Aging is associated with complex immune dysfunction that contributes to the onset and progression of the "geriatric giants", including frailty, sarcopenia, cognitive decline, falls, and incontinence. Central to these conditions is immunosenescence, marked by thymic involution, the loss of naïve T cells, T-cell exhaustion, impaired B-cell class switch recombination, and increased autoreactivity. Concurrently, innate immunity deteriorates due to macrophage, neutrophil, and NK cell dysfunction, while chronic low-grade inflammation-or "inflammaging"-amplifies systemic decline. Key molecular pathwa

Stabilizing the Unstable: Cone Hemiarthroplasty in Geriatric Intertrochanteric Fractures.

BACKGROUND: Unstable intertrochanteric fractures in the elderly pose a significant treatment challenge due to poor bone quality, comminution, and associated comorbidities. Bipolar hemiarthroplasty offers the advantage of early mobilization and reduced fixation-related complications. This study evaluates short-term outcomes of bipolar hemiarthroplasty using an uncemented titanium fluted, tapered cone femoral prosthesis in such fractures. METHODS: A retrospective analysis was conducted on 43 consecutive elderly patients treated with uncemented bipolar hemiarthroplasty using a tapered, fluted co

Geriatric Tuberculosis in India: Emerging challenges and practical perspectives.

Geriatric tuberculosis (TB) is an emerging but under-recognized challenge in India. Older adults often present with atypical or advanced disease, leading to delayed diagnosis, treatment complications, and increased mortality. While pulmonary TB is common, extrapulmonary and disseminated forms are also frequent. Diagnosis is hindered by non-specific symptoms and reduced microbiological yield, and treatment is complicated by co-morbidity, polypharmacy, and poor adherence. Despite elevated risk, older adults are not prioritized under current TB preventive strategies. This review highlights the un

Measures for Identifying Malnutrition in Geriatric Rehabilitation: A Scoping Review.

Malnutrition is a common condition in geriatric rehabilitation settings; however, the accuracy and predictive validity of the measures to identify malnutrition have not been established. The current scoping review followed the Joanna Briggs Institute's evidence synthesis manual and the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping Reviews checklist. Literature published through September 2023 was searched using MEDLINE and CINAHL. The inclusion criteria selected studies reporting malnutrition measures, which include static body weight and weight loss.

Complex Management of a Geriatric Patient With Multimorbidity: A Case Study.

Geriatric patients' distinct physiological responses to diseases and treatments, combined with multimorbidity and polypharmacy, make their care highly challenging. This case study examines the complex management of a 77-year-old female with multiple comorbidities, focusing on the primary issue of splenic hematoma leading to anemia. It highlights the importance of a multidisciplinary approach in geriatric care. The care she received underscores the necessity of ongoing supervision through the Bridge Model of Transitional Care, family engagement in the care process, and a customized, interdiscip

Prediction of Fragility Fractures and Mortality in a Cohort of Geriatric Patients.

BACKGROUND: Risk factors of refracture after fragility fractures include osteoporosis, female gender and advanced age among others. We hypothesized that the assessment of functionality, muscle health and nutrition status contribute to the risk prediction for further fractures and death. METHODS: We assessed 334 patients admitted to the department of acute geriatrics for sociodemographic data, bone fragility, selected laboratory tests, body composition and data on functionality using the comprehensive geriatric assessment. Patients had follow-ups until the occurrence of further fractures or de…

The Evolution of Geriatric Ankle Fracture Management to Promote Mobility.

Geriatric ankle fracture management requires a comprehensive approach that balances appropriate patient optimization, fracture reduction, stability, and early mobilization. Surgical and nonsurgical options should be cautiously considered based on patient and fracture characteristics. Early weight-bearing treatment options should be pursued when possible. There are a myriad of treatment options including fibular intramedullary devices or minimally invasive joint prep tibiotalocalcaneal nailing that minimize soft tissue disruption and can provide robust stability sufficient to allow early weight

[Geriatric syndromes: the importance of the problem, controversial issues of terminology.].

The foundation of healthy aging is the prevention of disability. In modern medical usage, a syndrome refers to a collection of symptoms and signs with a single underlying cause that may not yet be known. Geriatric syndromes, on the other hand, refer to multifactorial health conditions and occur when the accumulated effects of impairments in multiple systems make an older person vulnerable to situational changes. The use of the term "syndrome" in geriatrics emphasizes the multiple causes of a single manifestation involving an abundance of factors involving multiple organs and systems and is cha…

Geriatric medicine and Olympic elite sports-parallels and philosophies.

The 30th Olympiad took place in July 2024. At first glance, sports science and training of elite athletes may appear to be of little relevance to geriatric medicine. However, there are important parallels between the practice of geriatric medicine and elite sports and lessons that we can learn from our sports science colleagues. Elite athletes and older people are operating at the margins of physiological capacity. Both benefit from tailored, scientifically informed training programmes delivered and monitored by a multidisciplinary team. There are parallels between the comprehensive geriatric

Regenerative therapy in geriatric patients with low back pain.

Low back pain (LBP) is a prevalent and debilitating condition, particularly among older adults, with degenerative spinal disease being a major contributor. Regenerative therapy, which aims to repair and regenerate damaged spinal structures, has shown promise in providing long-term pain relief and functional improvement. This review focuses on the application and efficacy of regenerative therapies such as mesenchymal stem cells, platelet-rich plasma, and atelocollagen in older patients with LBP. Despite the potential benefits, there is a notable scarcity of studies specifically targeting the ol
