Chest computed tomography (CT) of a 22-year-old man with a history of long-term low fever and nonproductive cough demonstrated lymphadenopathy in the superior, middle, and posterior mediastinum. Slight bilateral gynecomastia was also observed on the CT scan. Subsequent physical examination and ultrasonography revealed a left testicular mass, and abdominal CT showed retroperitoneal lymphadenopathy. Left orchiectomy was performed, with the histological examination confirming the diagnosis of seminoma.

Two hundred forty cases of childhood gynecomastia were studied retrospectively. There were 13 cases aged 3 to 10 years and 227 cases aged 11 to 15 years. Of the 240 cases of gynecomastia, 160 presented with bilateral breast enlargement, 50, left breast enlargement, and 30, right breast enlargement. During the past three decades, there has been a gradual evolution toward less invasive surgery. In breast surgery, the introduction of vacuum-assisted biopsy devices with larger bore cannulae has extended their role to include a therapeutic as well as a diagnostic function. Gynecomastia is a common benign male breast disease, which may exhibit mild cellular atypia in cytology specimens. However, marked cytologic atypia can be seen in gynecomastia superimposed by chemotherapy. A 54-year-old patient with myotonic dystrophy presented unilateral painful gynecomastia, which occurred 3 months after aggravation of diabetes mellitus. Serum luteinizing hormone (LH) and follicle stimulating hormone (FSH) levels were slightly high. LH was elevated than 2 years before. Breast pain and gynecomastia disappeared by daily administration of 10 mg tamoxifen. He could not have intramuscular injection therapy because of marked muscle atrophy. We investigated the distribution of type VI collagen in 36 cases of routinely fixed and paraffin-embedded gynecomastia using an immunoperoxidase method for light microscopic visualization. A young man presented with a 4-month history of progressive, bilateral tender gynecomastia and underwent a bilateral subcutaneous mastectomy without regard for a possible occult testicular tumor. Following the mastectomy, metastatic testicular cancer was discovered; the patient had an overlooked scrotal mass and a very elevated serum human chorionic gonadotropin tumor marker. The possibility of testicular tumor must be considered in any male, especially those in the younger age group, who present with gynecomastia.

As part of a 3-year longitudinal study of lipid and hormonal changes during puberty, 536 boys aged 10 to 15 years were prospectively followed every 6 months to assess development of gynecomastia.
