Ligament of Marshall ablation for persistent atrial fibrillation.

Beyond pulmonary vein isolation, the two main additional strategies: Cox-Maze procedure or targeting of electrical signatures (focal bursts, rotational activities, meandering wavelets), remain controversial. High-density mapping of these arrhythmias has demonstrated firstly that a patchy lesion set is highly proarrhythmogenic, favoring macro-re-entry through conduction slowing and providing pivots for localized re-entry. Secondly, discrete anatomical structures such as the Vein or Ligament of Marshall (VOM/LOM) and the coronary sinus (CS) have epicardial muscular bundles that are more frequent

[Conduction defects and arrhythmias in peripheral myopathies].

The association between peripheral myopathies and cardiac complications is well established. However, until recently, the clinical and genetic variability of these pathologies limited our ability to recognise individual risk of complications, particularly in the more rare pathologies. Advances have been made in the understanding of the progression, in the physiopathology of molecular deficits and cardiac complications of the different types of muscular dystrophy. This has partially helped to identify the risk of cardiac complications. The commonest condition, Steinert's disease, is associated

Cardiac disease in myotonic dystrophy.

Cardiac disease is a well-known complication of myotonic dystrophy, understanding of which has been increased by recent advances in both molecular techniques and cardiological investigations. Conduction disturbances and tachyarrhythmias occur commonly in myotonic dystrophy. These have been shown to have a broad correlation in severity with both neuromuscular disease and the extent of the molecular defect in some, but not all, studies. Clinical evidence of generalised cardiomyopathy is unusual. The rate of progression differs widely between individuals; sudden death may be caused by ventricular

The role of pacemaker therapy in the prevention of atrial fibrillation.

Atrial fibrillation (AF) has been recognized, with increasing concern, as a potentially disabling illness, occurring either as a symptom of many cardiac diseases or as an isolated disorder. It can independently contribute to mortality and morbidity and may have serious prognostic importance in acute or chronic cardiac disease. In patients with symptomatic drug refractory atrial fibrillation, ventricular rate control by atrioventricular nodal ablation or modification commonly results in pacemaker implantation. The concept of AF prevention by pacemaker therapy has been introduced in patients wit

[Diagnosis and treatment of paroxysmal tachycardia].

The practising physician's knowledge of paroxysmal attacks of tachycardia is frequently disappointing. In this article a description is given of mechanisms and possible sites of origin of tachycardia. This is followed by a discussion on the typical history and the findings in these patients. Lastly, therapeutic guidelines are given. A discussion of the electrocardiographic features of the different types of paroxysmal tachycardias falls outside the scope of this article.

[Non-pharmacologic therapy of supraventricular tachyarrhythmias].

In the last ten years different forms of nonpharmacological therapy of supraventricular tachyarrhythmias were developed. One of the major advances is the catheter fulguration, which has reduced significantly need for surgical procedures in the treatment of atrial or junctional tachyarrhythmias. Operative treatment for dissection or ablation of accessory atrioventricular pathways is currently highly successful with minimal morbidity in any pathway location. The use of an antitachycardia pacemaker is limited to some reentrant tachycardias. The choice of nonpharmacological therapy is strongly ind

Fatal paroxysmal supraventricular tachycardia in an infant.

A 37-day-old infant, without associated congenital anomalies, died following a 48-hour episode of supraventricular tachycardia. The histopathologic findings of nodoventricular and fasciculoventricular Mahaim's fibers in a setting of persistent fetal dispersion were consistent with an anatomic substrate for a reentry circuit at the specialized A-V junction.

Mechanisms of supraventricular tachycardia.

One of several mechanisms may be responsible for the arrhythmia in patients with attacks of SVT. When conventional therapy is ineffective in preventing the attacks and when the attacks occur frequently, then electrophysiologic studies should be performed. These studies help determine the mechanism of the arrhythmia and provide the means for testing antiarrhythmic drugs for prophylactic use. When drugs fail, sophisticated, specially designed antiarrhythmic pacemakers can be used when the electrophysiologic mechanisms are known.

Symptomatic arrhythmia in the form of multiple premature ventricular extrasystoles as the first symptom of polymyositis.

Polymyositis (PM) is an autoimmune disease characterized by the involvement of multiple internal organs, including the cardiovascular system. The involvement of heart is observed in up to 75% of patients with PM. Conduction and rhythm disorders are one of the most common cardiological abnormalities in these patients. The presented clinical case is the patient where ventricular arrhythmia (in the form of multiple premature ventricular extrasystoles) was the first symptom of polymyositis.

Accidental Conduction System Pacing in Patient with Displaced Cardiac Resynchronization Therapy Leads.

A 69-year-old woman with a history of heart failure with reduced ejection fraction presented for device interrogation of her cardiac implantable electronic device (CIED), revealing lead and pulse generator displacement. Surprisingly, she exhibited a narrow QRS on the ECG despite an underlying right bundle branch block, suggesting unintentional conduction system pacing (CSP). Traditional cardiac resynchronization therapy has been widely used for patients with heart failure, but alternatives like CSP are emerging as viable options. Given the global rise in CIED utilization, regular follow-up, de

Discordant expression of maternal SLE in twin pregnancy with a single fetal AV block: A case report.

There are multiple manifestations in the neonatal period for infants born to mothers with systematic lupus erythematosus (SLE), ranging from cardiac, hematologic and dermatologic abnormalities. Cardiac complications may arise in utero in the form of heart block with a resulting increase in fetal mortality. The fetal conduction system is suspected to be affected by transplacental maternal antibodies, however additional environmental and fetal factors appear to play a role. We describe a rare case of a dichorionic-diamniotic twin pregnancy in which only one twin developed a complete heart block

Accessory pathway ablation during atrial fibrillation in Ebstein anomaly.

An 84-year-old woman with type B Wolff-Parkinson-White (WPW) with Ebstein anomaly was admitted with heart failure. She had rapid wide QRS tachycardia due to accessory pathway (AP) conduction associated with atrial fibrillation (AF). Since transesophageal echocardiography before catheter ablation showed a left atrial thrombus, ablation was performed using a 3D mapping system under AF. After marking the functional tricuspid anulus with intra-cardiac echocardiography, 3D intra-cardiac electrogram visualization (ripple map) during AF enabled clear identification of location of the AP. After ablati

Lyme Carditis Manifesting as Wenckebach Heart Block.

Lyme disease caused by Borrelia burgdorferi is a multisystem disease and can lead to Lyme carditis. The most common presentation of Lyme carditis is conduction system disturbances such as atrioventricular (AV) block. A 72-year-old male with a past medical history of gastroesophageal reflux disease (GERD) and prostate cancer presented with chest tightness, lightheadedness, and presyncope. During hospitalization, he developed bradycardia with heart rates ranging between 30 and 40 beats per minute and Wenckebach heart block. Lyme serology was consistent with a recent Lyme infection. He was treate

Normal and Abnormal Atrial Anatomy Relevant to Atrial Flutters: Areas of Physiological and Acquired Conduction Blocks and Delays Predisposing to Re-entry.

This article reviews the structure of the atrial chambers to consider the anatomic bases for obstacles and barriers in atrial flutter. In particular, the complex myocardial arrangement and composition of the cavotricuspid isthmus could account for a slow zone of conduction. Prominent muscle bundles within the atria and interatrial, and myoarchitecture of the walls, could contribute to preferential conduction pathways. Alterations from tissue damage as part of aging, or from surgical interventions could lead to re-entry.

Signature signal strategy: Electrogram-based ventricular tachycardia mapping.

Multiple decades of work have recognized complexities of substrates responsible for ventricular tachycardia (VT). There is sufficient evidence that 3 critical components of a re-entrant VT circuit, namely, region of slow conduction, zone of unidirectional block, and exit site, are located in spatial vicinity to each other in the ventricular scar. Each of these components expresses characteristic electrograms in sinus rhythm, at initiation of VT, and during VT, respectively. Despite this, abnormal electrograms are widely targeted without appreciation of these signature electrograms during conte
