Mechanical circulatory support for destination therapy.

Patients with chronic heart failure who are not eligible for heart transplant and whose life expectancy depends mainly on the heart disease may benefit from mechanical circulatory support. Mechanical circulatory support restores adequate cardiac output and organ perfusion and eventually improves patients' clinical condition, quality of life and life expectancy. This treatment is called destination therapy (DT) and we estimate that in Switzerland more than 120 patients per year could benefit from it. In the last 10 years, design of the devices, implantation techniques and prognoses have changed

Advanced Heart Failure Therapies for Hypertrophic Cardiomyopathy: State-of-the-Art Review and an Updated Analysis From UNOS.

Hypertrophic cardiomyopathy (HCM) is most commonly associated with obstructive symptoms and sudden cardiac death; however, predominantly nonobstructive advanced heart failure in HCM, marked by medically refractory disease with severe functional impairment, occurs in 5% to 7% of patients with HCM. The diagnosis relies on the integration of imaging (echocardiography/cardiac magnetic resonance), hemodynamic data, and cardiopulmonary exercise testing to identify the patients who will benefit from advanced heart failure therapies. Most advanced heart failure therapies focus on systolic dysfunction

Current indication and practical management of percutaneous left ventricular assist device support therapy in Japan.

Impella (Abiomed, Danvers, MA, USA) is a recently-innovated (commercially available from 2017 in Japan) percutaneous left ventricular assist device which is inserted percutaneously and transfers blood from the left ventricle to the ascending aorta, improving systemic circulation and end-organ dysfunction as well as unloading left ventricle in patients with cardiogenic shock. Impella has not yet shown a significant survival benefit in patients with cardiogenic shock compared to intra-aortic balloon pump in randomized control trials, but gives powerful circulatory support immediately with minima

Vortical flow characteristics of mechanical cavopulmonary assistance: Pre- and post-swirl dynamics.

Surgical optimization of the cavopulmonary connection and pharmacological therapy for dysfunctional Fontan physiology continue to advance, but these treatment approaches only slow the progression of decline to end-stage heart failure. The development of a mechanical cavopulmonary assist device will provide a viable therapeutic option in the bridging of patients to transplant or to stabilization. We hypothesize that rotational blood flow, delivered by an implantable axial flow blood pump, could effectively assist the venous circulation in Fontan patients by mimicking vortical blood flow pattern

Device-Related Thrombosis in Continuous-Flow Left Ventricular Assist Device Support.

Advanced heart failure therapy has been revolutionized with the advent of continuous-flow ventricular assist devices (CF-LVADs) which have improved both survival and quality of life. Despite this, support with CF-LVADs is frequently complicated, with 70% of recipients experiencing a major complication in the first year of durable support. The most concerning of these complications to emerge is device-related thrombosis, which is associated with increased morbidity and mortality. Pathophysiology and diagnosis are multifaceted and complex, with pump-specific and patient-specific factors to be co

Initial tests with a new cardiac assist device.

Before, during, and after cardiac intervention, there is occasionally a need for circulatory support because of hemodynamic deterioration. For this purpose, a new minimally invasive cardiac assist device has been developed, and an early prototype has been studied in a bench test and in three pigs. The pump is a catheter system with a distal motor driven propeller (0-15,000 rpm) surrounded by a cage. The catheter was first tested in a tube in a water bath, where efficiency with respect to pressure generation and flow properties was measured. In the pig experiments, the pump was placed in the de

[Present status of cardiomyoplasty].

Direct ventricular assist and myocardial replacement using skeletal muscle, so-called dynamic cardiomyoplasty, are promised as an effective treatment of choice in severe cardiac failure. Owing to the use of autologous muscle, the risk of rejection can be ignored, which is problematic in cardiac transplantation. More than 300 cases have been reported in the world. Since a cardiac transplant protocol has not been started yet in our country, we are hoping that clinical experience with these treatments solves our problem.

Implementing a Programmatic Approach to Identify and Promote Ventricular Recovery in Pediatric Patients Supported With Ventricular Assist Devices.

BACKGROUND: In October 2022, our center implemented a standardized program to promote ventricular recovery in pediatric patients supported with durable ventricular assist devices. We report our experience and outcomes. METHODS: The initiative consists of 4 core components for all patients with ventricular assist devices: (1) cultural shift: routine assessment for ventricular recovery for possible device explant or, in complex congenital heart disease (CHD), for further surgical palliation; (2) reverse remodeling-use of goal-directed medical therapy as tolerated; (3) assessment of recovery: st

Sixteen-year experience with 1,000 heart transplants at UCLA.

1. The consecutive pre- and post-1994 eras have demonstrated improved survival for all age groups. This is linked to improved preservation methods, surgical technique and immunosuppression agents. 2. The use of marginal donor hearts for Status I and alternate elderly patients has followed the model of matching donor and recipient risk without affecting patient outcome and minimized the use of implantable assist devices. 3. A donor history of systemic gram-negative infection, hypertension, or traumatic intracranial bleeds was an important marker for risk. Younger age and shorter ischemia time c

Enlarged and echogenic kidneys while on a pediatric ventricular assist device.

Pediatric ventricular assist devices (VAD) specially designed for small children have been increasingly implanted in North America. We present unexpected renal findings developing in three children receiving circulatory support using VAD (EXCOR Pediatric Ventricular Assist Device; Berlin Heart Inc., The Woodlands, TX). The mechanisms of heart failure in these children included: 1) hypoplastic ventricle with aortic stenosis; 2) anomalous coronary artery arising from the pulmonary artery; and 3) idiopathic progressive heart failure. During circulatory support with VAD, each child had coagulation

Ventricular Assist Devices and Other Mechanical Circulatory Support Devices: Types, Complications, and Updates.

Mechanical circulatory support devices (MCSDs) have revolutionized the management of advanced heart failure and critical cardiopulmonary conditions. These devices, categorized into temporary and long-term systems, provide crucial circulatory and respiratory support in patients with cardiogenic shock, refractory heart failure, and/or acute respiratory distress syndrome or act as a bridge to recovery or transplant. Temporary devices such as intra-aortic balloon pumps, percutaneous heart pumps, percutaneous ventricular assist devices (VADs), and dual-lumen cannulas are essential for acute hemodyn

Hemodynamic and Echocardiographic Predictors of Mortality in Pediatric Patients on Venoarterial Extracorporeal Membrane Oxygenation: A Multicenter Investigation.

BACKGROUND: Venoarterial extracorporeal membrane oxygenation (ECMO) supports patients with advanced cardiac dysfunction; however, mortality occurs in a significant subset of patients. The authors performed a multicenter, prospective study to determine hemodynamic and echocardiographic predictors of mortality in children placed on ECMO for cardiac support. METHODS: Over 8 years, six heart centers prospectively assessed echocardiographic and hemodynamic variables on full and minimum ECMO flow. Sixty-three patients were enrolled, ranging in age from 1 day to 16 years. Hemodynamic measurements in

Heartmate 3 left ventricular assist device system in patients with glucose-6-phosphate dehydrogenase deficiency.

BackgroundGlucose-6-phosphate dehydrogenase (G6PD) deficiency is a genetic enzymatic disorder that affects millions of people worldwide and characterized by hemolysis under oxidative stress. Left ventricular assist devices (LVADs) have substantially enhanced survival and quality of life for individuals with advanced heart failure. However, their use is associated with the risk of hemolysis, thrombosis, and embolic events. These risks may be heightened in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency. Given the limited published research on this subject, the primary objectiv

Bleeding with the artificial heart: Gastrointestinal hemorrhage in CF-LVAD patients.

Continuous-flow left ventricular assist devices (CF-LVADs) have significantly improved outcomes for patients with end-stage heart failure when used as a bridge to cardiac transplantation or, more recently, as destination therapy. However, its implantations carries a risk of complications including infection, device malfunction, arrhythmias, right ventricular failure, thromboembolic disease, postoperative and nonsurgical bleeding. A significant number of left ventricular assist devices (LVAD) recipients may experience recurrent gastrointestinal hemorrhage, mainly due to combination of antiplate

Considerations of Left Ventricular Assist Device Therapy in the United States' Prison System.

Left ventricular assist device therapy for advanced heart failure is contraindicated if a patient lives in an unsafe environment and recent guidelines declare that "legal history is pertinent for determining personal constraints or financial responsibilities due to parole requirements, pending charges, and possible imprisonment," implying that incarceration would be a contraindication. International guidelines and precedent in the United States establish that medical care for incarcerated persons should match access in the community. We present a case example and practical considerations for a
