[Diseases of the upper segment of the gastrointestinal tract and Helicobacter pylori infection].

Helicobacter pylori infection is the most frequent infection in humans. It is assumed that over 50% of total world population suffer from this infection. Chronic gastritis is the most usual consequence of this infection. Development of peptic ulcer appears in case of 10 to 20 percent of infected people with the increased risk of malignancy. In the group of 819 examined patients at the Clinic for gastroenterology and hepatology of the Institute for internal diseases, the presence of helicobacter pylori was confirmed in case of 69.18% of patients. The infection was equally observed in both sexes

Antral and fundic D-cell numbers in Helicobacter pylori infection.

OBJECTIVES: Helicobacter pylori infection is associated with an exaggeration of gastrin release following meals or bombesin stimulation attributed to a defect of somatostatin secretion of antral D-cells. Nevertheless, these modifications of gastric physiology do not explain the increase of gastric acid secretion which is only observed in duodenal ulcer patients. The inhibitory effect of somatostatin secretion of fundic D-cells on parietal cells is well known. The aim of our prospective study was to compare the number of fundic D-cells and likewise the number of antral G-cells and D-cells betwe

Eosinophils and Mast Cells in Helicobacter Pylori Infected Gastric Mucosa.

Although Helicobacter pylori (HP) is frequently associated with chronic active gastritis and peptic ulcers, its exact pathogenic role or the pathomechanism is still unclear. Here, we describe a striking, statistically significant increase of eosinophils in HP infected gastric mucosa compared to HP negative gastritis with similar activity. In both cases, the mean number of the mast cells in the mucosa was comparable, although the individual values showed wide distribution. The source and role of eosinophilia in HP infected mucosa, the potential link between the degree of eosinophilia and the cl

Therapeutic immunization against Helicobacter mustelae in naturally infected ferrets.

BACKGROUND & AIMS: Helicobacter infection of the gastric antrum is responsible for a number of gastric disorders. Antibiotic therapy is lengthy and is not always effective. It has been shown previously that oral immunization against Helicobacter felis in mice can prevent colonization after challenge. The aim of this study was to investigate the efficacy of therapeutic immunization in eradicating an established Helicobacter infection and in reducing gastritis. METHODS: Domestic ferrets, confirmed to be infected with Helicobacter mustelae by gastric endoscopy, were orally immunized with varying

Why guidelines are required for the treatment of Helicobacter pylori infection in children.

Helicobacter pylori is firmly established as a human pathogen; it fulfils all of Koch's postulates as the infectious agent causing chronic, active (type B) gastritis. Infection is strongly associated with duodenal and gastric ulcer. Recently, gastric mucosal-associated lymphoid tissue lymphoma has been successfully treated by curing H. pylori infection. Because of the evidence that the organism causes chronic gastritis and an increased risk of gastric cancer, it has been classified as a category 1 carcinogen by the World Health Organization. However, the overwhelming majority of people infecte

The chemotherapeutic effects of H+/K+ inhibitors on Helicobacter pylori infection.

Helicobacter pylori's powerful urease enzyme is essential for colonisation and adaptation to the acid milieu of the stomach. Eradication of infection with "standard triple therapy" abolishes the chronic immunological and inflammatory responses to H. pylori and, thus, cures gastritis and peptic ulcer. In vitro, proton pump inhibitors (PPI) are active against H. pylori with minimum inhibitory concentrations that compare favourably with bismuth salts. PPIs are also potent urease inhibitors, but because PPIs are also active against urease-negative mutant Helicobacter spp., it is unlikely that urea

Gastric emptying and Helicobacter pylori infection in duodenal ulcer disease.

The pathogenetic link between Helicobacter pylori gastritis and duodenal ulcer is still unknown. Fast gastric emptying of liquids might be important in the pathogenesis of gastric metaplasia of the duodenum and duodenal ulcer through an increased exposure of the duodenum to gastric acid. In H. pylori-infected subjects, an abnormal gastric emptying could affect urea breath test results and correlate with histological gastritis. This study was performed to evaluate the gastric emptying of liquids in duodenal ulcer patients with H. pylori infection and the possible relation between the bacterial

[Evaluation of polymerase chain reaction for diagnosis of Helicobacter pylori infection].

To evaluate diagnostic tests of Helicobacter pylori (H. pylori), we compared polymerase chain reaction (PCR) for the specific detection of H. pylori, culture and positivity of serum antibody against the bacteria in 59 patients with gastroduodenal disease. PCR assay was designed to amplify 203bp of DNA fragments. PCR detected 10pg of H. pylori DNA, or 40 CFU of the bacteria. PCR was positive in 82.1% of histological gastritis with neutrophil infiltration. Culture and serum antibody to H. pylori were positive in 65.0%, and 94.1% of those patients. Detection rate by PCR was significantly higher t

Selection of patients for treatment of duodenal ulcer infected with Helicobacter pylori.

Ninety patients with endoscopically proven duodenal ulcer were followed for 1 year. All patients were initially treated with an H2-receptor antagonist. Those with more than two relapses within 1 year received triple therapy consisting of colloidal bismuth subcitrate, amoxicillin, and metronidazole. Results showed that 27 patients were symptom-free after one full course of H2 antagonist, 47 had one or two clinical recurrences, and 16 patients had more than two recurrences. Twelve of the latter group were Helicobacter pylori positive and were treated with triple therapy. We conclude that only a

Helicobacter pylori: a predominantly childhood infection--predominantly adult disease.

It is now 12 years since Helicobacter pylori was first cultured in an Australian laboratory. During these years significant understanding of the organism, its epidemiology and its disease associations have been made. Diagnostic methods and therapeutic regimens have appeared and been refined. In this article microbiological, epidemiological and clinical aspects of adult and childhood H. pylori are linked to present and future public health problems.

Helicobacter pylori infection in children with antral gastrin cell hyperfunction.

Antral gastrin cell hyperfunction (AGCH) is a rare syndrome characterized by persistent hypergastrinemia and important peptic symptoms in the absence of a gastrin-producing tumor. The pathogenesis of AGCH is still unknown and debated. Helicobacter pylori (Hp) infection has been reported as a possible cause of sustained hypergastrinemia. To assess the relevance of Hp infection in pediatric AGCH patients, Hp status, G cell function, acid secretion, and antral G and D cell populations were investigated in six children presenting with gastrointestinal bleeding of unknown origin, sideropenic anemia

Gastric cancer--a long-term consequence of Helicobacter pylori infection?

Chronic gastritis and chronic atrophic gastritis in particular are risk factors for gastric cancer. In Western societies, for example Finland, up to 70% of gastric cancer cases are associated with Helicobacter pylori-positive chronic non-atrophic or atrophic gastritis, approximately 15% appear in conjunction with autoimmune chronic corpus-limited H. pylori-negative atrophic gastritis, and 15% develop in individuals with a histologically normal stomach. The role of H. pylori infection in chronic gastritis has led to the hypothesis that this infection could be involved in the pathogenesis of gas

Serological diagnosis of infection by Helicobacter pylori in chronic ulcer disease.

The authors estimated the frequency of occurrence of the infection by Helicobacter pylori in the long term gastroduodenal ulcer disease. The serological diagnosis (ELISA test) was carried out in 175 patients, suffering from this disease for at least two years, to reveal the existence of specific IgG against Helicobacter pylori. The study confirmed the frequent occurrence of infection by Helicobacter pylori in the gastro-duodenal ulcer disease of the stomach (59.2%) and the duodenum (83.7%). It was found that mainly in men the existence of specific IgG against Helicobacter pylori (in the ulcer
