The efficacy and safety of eculizumab in patients and the role of C5 polymorphisms.

Eculizumab is an orphan drug with indications for extremely rare autoimmune disorders. It is primarily prescribed for use in patients with paroxysmal nocturnal hemoglobinuria and atypical hemolytic uremic syndrome; but is also highly effective in the treatment of myasthenia gravis, among others. By binding to the C5 protein in the complement system, eculizumab effectively inhibits cellular hemolysis and autoimmune reactions. Despite this effective treatment, some patients reported no improvement in symptoms. Genetic sequencing revealed three distinct C5 mutations in the non-responders and thes…

[Atypical hemolytic uremic syndrome with C3 p.I1157T missense mutation successfully treated with eculizumab].

A 23-year-old man from Mie Prefecture, Japan, with past and family history of hematuria was diagnosed with influenza A and admitted to our hospital on the following day because of hemoglobinuria. He was diagnosed with thrombotic microangiopathy and was suspected of having atypical hemolytic uremic syndrome (aHUS). C3 p.I1157T missense mutation, which we had previously reported in eight aHUS patients from six families in Mie Prefecture, was identified. The laboratory findings and symptoms of our patient promptly improved after administering eculizumab. Little information is available on abnorma…

Puerarin-induced immune hemolytic anemia.

Drug-induced immune hemolytic anemia (DIIHA) is a relatively uncommon condition characterized by a sudden drop in hemoglobin, putatively following exposure to drugs. Severe forms of hemolysis characterized by rapidly falling hemoglobin levels and hemoglobinuria are extremely rare. Here we report the case of a patient who exhibited severe DIIHA due to puerarin. Direct antiglobulin testing and drug-dependent antibody testing indicated that the antibodies were drug-dependent and reacted only with RBCs in the presence of the drug. Puerarin is the major isoflavonoid derived from the Chinese medical…

Autoimmune hemolytic anemia. 3. Cold antibody type.

In cold autoimmune hemolytic anemia, the causative autoantibodies (cold agglutinins) are most reactive at low temperatures. The anti-I type of cold agglutinin is found in cold hemagglutination disease and in hemolytic anemia secondary to malignant lymphoma, leukemia, or atypical pneumonia. The anti-I type of cold agglutinin is found in many patients with infectious mononucleosis and in an occasional patient with other lymphoproliferative disorders. Paroxysmal cold hemoglobinuria, a disorder in which exposure to cold temperatures triggers massive hemolysis, is characterized by a unique biphasic…

Persistent Hematuria With Late Diagnosis of Cardiac Origin: A Case Report.

Dark-colored urine is often referred to as hematuria; however, this is an assumption that can often be misleading. In the evaluation of dark-colored urine, distinguishing between true hematuria, hemoglobinuria, and myoglobinuria is essential, as each indicates different underlying pathologies and requires targeted diagnostic and management strategies. Visualization of dark-colored urine often leads to an isolated urologic evaluation, and assessment for more infrequent causes of this phenomenon may be obscured. Clinicians tend to anchor on common urologic causes of dark-colored urine or rely on…

Pyoderma gangrenosum associated with paroxysmal nocturnal hemoglobulinuria and monoclonal gammopathy.

Pyoderma gangrenosum developed in a man with a five-year history of paroxysmal nocturnal hemoglobinuria and monoclonal gammopathy. He had multiple walnut sized ulcers on his back and extremities, plasma IgM-k type M-protein and low erythrocytic CD55 expression. This is an extremely rare association. However, clonal expansion of plasma cells and chimeric expression of hematopoietic cell glycosylphosphatidylinositol (GPI)-anchored proteins may represent somatic mutations of hematopoietic stem cells in PG as well as PNH. PNH is based on abnormalities in the GPI-anchor formation on various hematop…

Human babesiosis in Europe.

Human babesiosis in Europe came to medical attention in 1957 and until now 19 cases have been reported, most of them due to Babesia divergens. The onset of the disease is characterized by hemoglobinuria, high fever and renal failure ensue rapidly. The patients were generally asplenic and resident in a rural area. Intraerythrocytic pleomorphic parasites (1-3 microns) observed in stained thin blood smears are essential for Genus diagnosis. Parasitemia varied from 5 to 80% of red blood cells. Massive blood exchange transfusion (2-3 blood volumes) followed by intravenous clindamycin (3-4 times dai…

Cold urticaria associated with C4 deficiency and elevated IgM.

Various immunologic abnormalities have been implicated in cold urticaria. This is the first report of cold urticaria associated with C4 deficiency and elevated IgM. A 12-year-old male developed urticaria upon exposure to cold. He denied fever, purpura, hemoglobinuria, Raynaud's disease, or arthralgias. Family history was negative for cold urticaria. Immunologic studies revealed elevated IgM (186 mg/dL) as well as decreased CH100 and C4 (8.0 mg/dL). C1, C2, and C3 were normal. Ice cube skin test was positive, but passive transfer tests were negative. Biopsy was not diagnostic for vasculitis, al…

Loxoscelism of the eyelids.

Loxoscelism is a reaction to the bite of spiders of the genus Loxosceles. Several species have been found in the United States; the most commonly encountered is L reclusus, the small brown recluse spider. Two types of reactions occur from the bite. In the localized type, necrotic loxoscelism, a cutaneous lesion with extensive gangrene develops. About 25% of patients have systemic manifestations of viscerocutaneous loxoscelism. There is the same local reaction, but, in addition, fever, chills, vomiting, joint pain, and hematologic abnormalities occur. Hemoglobinemia and hemoglobinuria suggest s…

Decrease in PNH-type Blood Cells after the Cessation of Thrombopoietin Receptor Agonist in a Case of AA-PNH Syndrome.

A 64-year-old male with severe aplastic anemia was initially treated by immunosuppressive therapy, with a partial response. Subsequent thrombopoietin receptor agonist (TPO-RA) therapy resulted in a sustained recovery of cytopenia. After a few years, a gradual increase in the serum lactate dehydrogenase levels was observed, and a massive expansion of paroxysmal nocturnal hemoglobinuria (PNH) clones was established. TPO-RA was discontinued after a transient hemolytic episode of SARS-CoV-2 infection. A follow-up analysis revealed a decrease in the number of PNH clones. Although his blood counts d…

Copper toxicosis in 3 Japanese Black cattle with chronic cholangiohepatitis.

Three Japanese Black cattle raised in different farms died suddenly with severe jaundice and hemoglobinuria. Gross examination revealed diffuse hepatic discoloration and highly viscous bile in the gallbladder. Histologically, the livers showed chronic cholangiohepatitis with hepatocellular vacuolation, bile plugs, interlobular bile duct hyperplasia, and periportal fibrosis. Alcian blue-periodic acid-Schiff staining showed acidic mucin in the bile ducts. Rhodanine staining and metallothionein immunohistochemistry demonstrated copper accumulation mildly in hepatocytes and mainly in Kupffer cells…

Splanchnic vein thrombosis: new risk factors and management.

Splanchnic vein thrombosis (SVT) is a rather heterogeneous disease which involve one or more abdominal veins draining from different organs, including small and large bowel, liver, spleen and pancreas, and it may be associated with a wide spectrum of underlying disorders, either local or systemic. The role of new risk factors for SVT, including the JAK2 V617F mutation and the paroxysmal nocturnal hemoglobinuria clone, has been highlighted in recent years. The clinical presentations of SVT are variable and, not uncommonly, may include the concomitant presence of extensive thrombosis and gastroi…

Persistent complement-dependent anti-AnWj in a lymphoproliferative disorder: a case study and review.

AnWj is a high-incidence antigen present on the red blood cells (RBCs) of greater than 99 percent of the general population. A 58-year-old man underwent autologous hematopoietic stem cell transplantation (HSCT) for stage IVa mantle cell lymphoma. This procedure was complicated by failure to engraft, necessitating ongoing support with blood components. After a 2-month period of uneventful transfusion support, the patient experienced increasingly severe reactions with fever and evidence of intravascular hemolysis, including hemoglobinuria. Testing revealed a complement-dependent anti-AnWj. Pheno…
