Haemoperitoneum--a complication of acute cholecystitis in a patient on anticoagulant therapy. A case report.

A case of haemoperitoneum in a patient taking warfarin sodium, originating in the wall of a perforated, inflamed gallbladder, is presented. To the best of our knowledge such a case has not been reported previously. Diagnostic and therapeutic aspects are briefly discussed.

Intrauterine Pregnancy Combined With Ovarian Pregnancy Was Missed by Ultrasound: A Case Report.

A 32-year-old G2P0 female presented with 4+ weeks of amenorrhea and acute-onset lower abdominal pain with vaginal bleeding. Initial ultrasound confirmed a 6+ week intrauterine pregnancy. The patient underwent painless uterine curettage on the third day because the patient voluntarily chose to terminate the pregnancy. Five days postoperatively, she developed severe pain and hemorrhage. Repeat imaging revealed an 11.26 × 5.73 cm heterogeneous left ovarian mass with a gestational sac-like structure and vascular flow, along with 1500 mL of hemoperitoneum. Pathology confirmed an ovarian pregnancy. …

An Uncommon Presentation of a Ruptured Tubal Ectopic Pregnancy With Broad Ligament Hematoma.

Broad ligament hematomas are rare in obstetrics. These hematomas typically result from vaginal, cervical, or uterine tears that extend to the uterine or vaginal arteries. Although cases of intestinal ectopic pregnancies presenting as broad ligament hematomas with no intraperitoneal collection have been reported, no cases involving broad ligament hematomas along with hemoperitoneum following ampullary ruptured ectopic pregnancy have been documented. Internal iliac artery ligation (IIAL) is particularly useful in broad ligament hematomas. So, we present a case of ruptured ampullary ectopic pregn…

Ruptured Gastric Aneurysm in α-1 Antitrypsin Deficiency.

We present a unique vascular complication of α-1 antitrypsin deficiency (AATD) in a patient with an acute onset of epigastric pain and hemodynamic instability. Abdominal computed tomography angiography detected hemoperitoneum and hematoma within the gastrohepatic ligament with active extravasation. Abdominal angiography revealed left gastric aneurysms. An association between AATD and vascular aneurysms has been suggested to be secondary to unopposed proteolytic activity against arterial structural proteins. The aneurysm formation in aortic, superior mesenteric, inferior mesenteric, and splenic…

Primary ovarian choriocarcinoma mimicking ectopic pregnancy.

Nongestational ovarian choriocarcinoma is an exceedingly rare and highly aggressive tumor. Although early diagnosis and timely initiation of therapy is important, it is difficult in reproductive aged patients because of the frequent elevation of human chorionic gonadotropin. We report a primarily nongestational ovarian choriocarcinoma in a 12-year-old virgin female. Initial diagnosis based on abdominopelvic computed tomography and pelvis magnetic resonance imaging was ectopic pregnancy with hemoperitoneum. A diagnostic laparoscopy of the ovarian tumor revealed choriocarcinoma. Unilateral salpi…

Acute fetal asphyxia after gastric rupture in a pregnant patient with a gastric band.

We report a 37-year-old primigravida with a gastric band who developed the clinical picture of abdominal pain, vomiting and regurgitation coexistent with a cardiotocogram with severe variable decelerations with absent variability at 33 weeks' gestation. After partial improvement with gastric band enlargement, new aggravation of symptoms and recurrence of a pathological cardiotocogram led to an emergency cesarean section. Intraoperatively, hemoperitoneum from gastric rupture was verified and partial gastrectomy was performed. After bariatric surgery, pregnant women are at increased risk of gast…

[Acute abdomen in anticoagulated patients. Its assessment and the surgical indications].

Bleeding complications associated with anticoagulant drugs can development abdominal pain simulating acute surgical conditions of abdomen. We report six cases of intraabdominal spontaneous haemorrhage presenting as an acute abdomen in patients on anticoagulant therapy: two rectus sheath hematomas, a abdominal wall hematoma, a spontaneous hemoperitoneum, a retroperitoneal hematoma and an intramural hematoma of the small bowel. The article conclude that computed tomography examination provided useful information in this complications because can demonstrate the size and location of the mass and …

Hepatic arteriovenous malformation in a calf.

A case of hepatic arteriovenous malformation is described in a calf that presented with a marked increase in abdominal volume and died 12 h after birth. At necropsy, the calf had marked abdominal distension and haemoperitoneum. The left hepatic lobe was reduced in size and covered by slightly whitish and intensely vascularized membranous tissue. At the edge of the left lobe there were cystic structures filled with translucent fluid. On cut surface, there were multiple cavitations well delimited by whitish walls and multiple dilated blood vessels that communicated with the cavitations. Histopat

Splenic Laceration Post Colonoscopy: A Rare Complication.

BACKGROUND: The number of colonoscopies performed in the United States is increasing each year. Although the procedure is generally safe and effective, complications can arise. Splenic laceration is an extremely rare complication of colonoscopy. CASE REPORT: A 71-year-old woman presented to the Emergency Department with abdominal pain and left shoulder pain accompanied by nausea within 24 h of undergoing a colonoscopy. A computed tomography scan showed hemoperitoneum, with findings suggestive of a splenic laceration. Upon laparoscopy, the patient was found to have a splenic laceration with ac…

Gastric artery apoplexy presenting as an acute abdomen in an adolescent.

Nontraumatic abdominal apoplexy, which is the occurrence of hemorrhage into the peritoneal cavity, is uncommon in the pediatric literature. Adult case reports of nontraumatic abdominal apoplexy include visceral artery aneurysms from the aorta, splenic, celiac, hepatic, gastric, and gastroepiploic arteries as well as gynecologic pathology. We identified 2 case reports of spontaneous abdominal apoplexy due to injury of the short gastric arteries in adult patients with histories of vomiting after alcohol consumption. This is the first reported case of nontraumatic abdominal apoplexy due to injury

[Infrahepatic hematoma secondary to gangrenous cholelithiasic hemocholecyst treated with antivitamins K].

Acute haemocholecyst is a rare disorder but often reported in patients with gallstones. Rupture of the gallbladder causing hypovolaemic shock secondary to massive intraperitoneal bleeding has been reported in about fifty patients to date. We report a new case of haemocholecyst with rupture of the gallbladder in a patient with gallstones treated by anticoagulant therapy. This case is unusual in terms of the nature of the gallbladder disease: massive gangrene with complicating hemorrhage and destruction of deficient regions of the wall of the gallbladder induced an infrahepatic haematoma without
