Chest Tube Size Selection: Evaluating Provider Practices, Treatment Efficacy, and Complications in Management of Thoracic Trauma.

BACKGROUND: The standard for managing traumatic pneumothorax (PTX), hemothorax (HTX), and hemopneumothorax (HPTX) has historically been large-bore (LB) chest tubes (>20-Fr). Previous studies have shown equal efficacy of small-bore (SB) chest tubes (≤19-Fr) in draining PTX and HTX/HPTX. This study aimed to evaluate provider practice patterns, treatment efficacy, and complications related to the selection of chest tube sizes for patients with thoracic trauma. METHODS: A retrospective chart review was performed on adult patients who underwent tube thoracostomy for traumatic PTX, HTX, or HPTX at …

Time to diagnosis of acute complications after cardiovascular implantable electronic device insertion and optimal timing of discharge within the first 24 hours.

BACKGROUND: More than 3 million cardiovascular implantable electronic devices (CIEDs) are implanted annually. There are minimal data regardi … ewed for the timing of diagnosis of acute complications. Acute complications included pneumothorax, hemothorax, pericardial effusion, lead dislodgment, and implant site hematoma requiring surgical intervention. RESULTS: A total of 2421 patients underwent implantation. Pericardial effusion or cardiac tamponade was diagnosed in 13 patients (0.53%), pneumothor …

A meta-analysis of the diagnostic accuracy of chest ultrasound for the diagnosis of occult penetrating cardiac injuries in hemodynamically stable patients with penetrating thoracic trauma.

BACKGROUND: We performed a systematic review (SR) and meta-analysis (MA) to determine the diagnostic accuracy of chest ultrasound (US) compa … penetrating cardiac injuries in hemodynamically stable patients. The presence of a concomitant left hemothorax was frequent in patients with false-negative results. CONCLUSION: This SR and MA found that, compared with PW, US was 79% sensitive and 92% specific for detecting occult penetrating cardiac injuries in hemodynamically stable patients with penetrati …

Percutaneous placement of Hickman catheters: comparison of sonographically guided and blind techniques.

We studied the value of sonographically guided puncture of the subclavian vein for placement of indwelling right atrial and permanent subcutaneously tunneled catheters (Hickman) for long-term venous access. The commonly used blind puncture is associated with complications such as pneumothorax, arterial puncture, and hemothorax. The results and complications of 40 sonographically guided punctures and fluoroscopically controlled catheterizations of the subclavian vein (group 1, 31 patients) performed in the radiology department were compared with those of 40 blind percutaneous punctures and fluo…

A case report: A man creates holes in his own heart.

We report a rare case of a 59-year-old male with a history of depression stabbed himself from xiphoid to heart. Following initial cardiothoracic surgery for hemothorax and cardiac repair, the patient was discharged and transferred to a psychiatric facility for depression treatment. Four weeks later, he returned with complaints of chest pain, cough, and expectoration. Further examination found abnormal blood pathways in heart and second surgery confirmed these were attribute to the original penetrating injury. This case highlights the delayed complications of penetrating cardiac trauma and unde…

Kasabach-Merritt Syndrome in Neonates: A Case of Complexity and Care.

Kasabach-Merritt syndrome (KMS) is a rare and potentially life-threatening pediatric coagulopathy characterized by thrombocytopenia, microangiopathic hemolytic anemia, and coagulation abnormalities. Clinically, it presents as a reddish-purple mass resembling a hemangioma. Diagnosis relies on a combination of clinical and biological assessments, sometimes supplemented by biopsy to confirm the hemangioma. We present the case of a one-day-old male neonate with an angioma affecting the right hemothorax and upper limb, accompanied by thrombocytopenia. The patient responded favorably to treatment wi…

Spontaneous Intercostal Artery Bleeding in a Patient With Buerger's Disease: A Case Report.

Intercostal artery (ICA) injury and bleeding are well-known complications of thoracic procedures and trauma; however, spontaneous ICA bleeding is a rare condition usually associated with specific underlying disorders that typically lead to the weakening of vasculature. Herein, we present a 42-year-old male with a history of Buerger's disease who developed spontaneous bleeding of the second left ICA after undergoing lower limb angioplasty. The bleeding was complicated by a large hemothorax and retropleural hematoma, resulting in hemorrhagic shock that necessitated massive transfusion, embolizat…

Internal Drainage of an Esophageal Perforation in a Patient with a High Surgical Risk.

A 71-year-old man presented with a productive cough and fever, and he was diagnosed as having an esophageal perforation and a mediastinal abscess. He had a history of traumatic hemothorax and pleural drainage for empyema in the right chest and was considered unable to tolerate thoracic surgery because of sepsis and progressive aspiration pneumonia. In order to aggressively drain the mediastinal contamination, we performed internal drainage by placing a Levin tube into the mediastinum through the perforation site. This procedure, in conjunction with controlling sepsis and providing sufficient p…

Brachiocephalic Vein Perforation During Cannulation of Internal Jugular Vein: A Case Report.

We report a rare complication of right brachiocephalic vein perforation during ultrasound-guided cannulation of the right internal jugular vein (IJV) in a patient with a tortuous common carotid artery (CCA). We suspect that the tortuous CCA displaced the IJV, which caused misplacement of the J-tip guidewire into the subclavian vein. The stiff dilator sheath introduced over the guidewire then perforated the wall of the brachiocephalic vein, causing massive hemothorax. This was diagnosed by videothoracoscopy. Anesthesiologists should be aware of the possibility of guidewire malposition during IJ…

Application of positive end-expiratory pressure in a case with large laceration on the superior vena cava.

Iatrogenic injury is a difficult situation for a surgeon. Being successful in saving a patient at risk is the major concern in this situation. Once an iatrogenic injury to the superior vena cava (SVC) is found, increasing the intrathoracic pressure is theoretically able to overcome the venous pressure and to alleviate or even stop bleeding from injury. A 76-year-old female patient, who had suffered from end-stage diabetic nephropathy, developed tension hemothorax during insertion of the cuffed hemodialysis catheter. The successful course of resuscitation without emergent operation or endovascu…

[Intraspinal air, a rare complication of blunt chest trauma].

Pneumorachis, defined as the presence of air within the spinal canal, has rarely been described, and is exceptionally due to thoracic trauma. We report the case of a 37-year-old patient who sustained a motor vehicle accident. The chest CT-scan showed a bilateral-hemothorax, a small right pneumothorax, rib fractures, and a fracture of the fourth thoracic vertebra associated with air in the spinal canal extending from T6 to L2. The fracture of T4 associated with the pleural lesion is probably responsible for the pneumorachis. Possible pathogenic mechanisms of this rare blunt chest complication a…

History of surgery for penetrating chest trauma.

The military surgery experience of the past several centuries has been an important determinant of the evolution of the clinical management of penetrating thoracic trauma. The major management problems fall into two main categories: acute, life-threatening conditions such as open pneumothorax and exsanguinating hemorrhage, and chronic conditions such as clotted hemothorax, empyema, and fibrothorax. Better treatment and prevention of the latter has greatly reduced morbidity. Although hospital mortality has fallen by a factor of ten since the middle of the nineteenth century, the total mortality…

[Value of imaging procedures in the diagnosis and therapy of severe thoracic injuries].

Plain chest radiographs allow the diagnosis of most intrathoracic injuries. However, they are only momentary pictures and give no information on the respiratory function. A tension pneumothorax, rib fractures and subcutaneous emphysema should be diagnosed clinically before radiographs are taken. Computed tomography is helpful in evaluation of intrapulmonary lesions, hemothorax, rupture of the diaphragm and dislocation of the heart. Further diagnostic tools include aortography in suspected aortic rupture, sonography in cardiac injuries and hemopericardium, bronchoscopy in suspected bronchial or…

Endotracheal tube as chest tube: a back-up alternative in resource limited settings.

Thoracostomies, and subsequent placements of chest tubes (CTs), are a standard procedure in several domains of medicine. In emergency medicine, thoracostomies are indicated to release a relevant hemothorax or pneumothorax, particularly a life-threatening tension pneumothorax. In many cases, an initial finger-assisted thoracostomy is followed by placement of a CT to ensure continuous decompression of blood and air. CTs prevent the reoccurrence of a hemothorax or pneumothorax, which may otherwise develop by closure of the initial thoracostomy incision. CTs are commercial, purpose-made products; …

Role of low-pressure negative pleural suction in patients with thoracic trauma - a randomized controlled trial.

BACKGROUND: Thoracic trauma frequently includes a pneumothorax, hemothorax, or hemopneumothorax, which may necessitate an Intercostal drainage (ICD) for air and fluid evacuation to improve breathing and circulatory function. It is a simple and life-saving procedure; nevertheless, it carries morbidity, even after its removal. Efforts have been made continuously to shorten the duration of ICD, but mostly in non-trauma patients. In this study, we evaluated the impact of negative pleural suction over the duration of ICD. METHODS: This study was a prospective randomized controlled interventional t…

Post-traumatic liver injury caused by chest tube placement: myths and controversies in conservative management.

This case report describes the conservative management of an iatrogenic hepatic injury secondary to chest tube thoracostomy in a 54-year-old male with severe thoracic polytrauma. A right-sided chest tube inserted in the seventh intercostal space for hemothorax resulted in transhepatic malposition. Given hemodynamic stability and absence of active hemorrhage, non-operative management (device removal and primary suture) was undertaken with strict monitoring and favorable evolution. This case underscores the feasibility of conservative management in selected patients and cautions against automati…
