The availability of vaccine since 1982 for immunization against hepatitis B virus (HBV) has had minimal impact on the disease; mass immunization has been suggested. In the period 1970-1984 alcoholic hepatitis was diagnosed by liver biopsy in 52 females. Thirty-six patients with cirrhosis were generally in a worse clinical and biochemical state than those without cirrhosis. Biochemical tests for liver function showed significant improvement from admission to the time of liver biopsy. At follow-up liver function tests were generally better in patients who had stopped drinking alcohol compared to those who continued to do so. The prevalence of hepatitis D virus (HDV) infection in patients with hepatitis B virus (HBV) infection in the mid-United States is not well defined. A 65 year old woman taking quinine sulphate for nocturnal leg cramps presented for investigation of episodes of malaise, fever, nausea, vomiting, and polyarthralgia. Panniculitis occurred in a 34-year-old man. The clinical features of the panniculitis were not consistent with erythema nodosum, Weber-Christian disease, or the other previously described panniculitides. Subsequent investigation showed concurrent hepatitis possibly caused by type A hepatitis virus. The skin lesions resolved following treatment with oral indomethacin. We believe this is the first report of a case of nonerythema nodosum panniculitis associated with viral hepatitis.

Hepatitis A is a common communicable disease in this country. Each diagnosed case of hepatitis A involves the problem of selecting contacts for post-exposure immunoglobulin. These decisions should be systematic, based on current knowledge of epidemiology, kinetics of viral excretion by infected persons and the influence of time on the efficacy of immunoglobulin prophylaxis. An algorithm incorporates this information into a strategy consistent with the United States Public Health Service guidelines.
