Impact of an Enhanced Recovery after Surgery Protocol on Postoperative Outcomes in Cardiac Surgery.

Background: Enhanced recovery after surgery (ERAS) protocols are perioperative care pathways designed to achieve early recovery after procedures. ERAS protocols have shown shortened recovery time, and lower opioid utilization and postoperative complication rates. Evidence to support the use of ERAS protocols is robust, however, minimal data exists in cardiac surgery patients. Methods: This observational cohort compared adults receiving post-operative care after coronary artery bypass or valve procedures who received an ERAS protocol containing acetaminophen, gabapentin, and methocarbamol to hi

Granulocyte colony-stimulating factor after allogeneic bone marrow transplantation.

Hematopoietic growth factors have been shown to be effective in reducing the period of neutropenia after autologous bone marrow transplantation (BMT). Initial concerns over potential aggravation of graft-versus-host disease (GVHD) and increase in the incidence of relapse in patients with myeloid leukemias influenced the number of studies using hematopoietic growth factors after allogeneic BMT. We report the experience with 50 patients treated at a single institution using granulocyte colony-stimulating factor (G-CSF) after allogeneic sibling (n = 30) and matched unrelated (n = 20) BMT. The tim

[Long-term results of an eradication regime against Staphylococcus aureus in patients treated with peritoneal dialysis].

Catheter exit site infection (ESI) remains a common complication in peritoneal dialysis patients. All the efforts for controlling ESI have been focused on the preventive eradication of Staphylococcus aureus (SA) colonization, because this microorganisms has been shown to be implicated in most of ESI. The main aims of the present study was to analyse the long-term results from an eradicative regimen of SA colonization, and to compare them with those obtained from a historical control group. From january 1993 to december 1999, 60 unselected patients on PD underwent an exhaustive protocol of SA e

Early procedural training increases anesthesiology residents' clinical production: a comparative pre-post study of the payoff in clinical training.

BACKGROUND: Competency-based education has been shown to enhance clinical skills, improve patient care, and reduce number of complications resulting in a better return on investments. Residents constitute an important workforce at many hospitals. Yet, the effect of training on residents' contribution to production in patient care is scarcely studied. This study evaluated the effects of early competency-based procedural training on residents' contribution to patient care in central venous catheterization and spinal and epidural anesthesia. METHODS: The design was a non-randomized cohort study

Evolution of Epidemiological Studies in Movement Disorders.

In celebration of the 40-year legacy of Movement Disorders Journal (MDJ), this historical reflection traces the evolution of epidemiological research in movement disorders through the lens of papers published in MDJ from 1986 through 2025. Over four decades, the discipline has transformed from clinic-based descriptive observations into population-based methodologies, with both genetic and environmental factors taken into consideration. The initial decade (1986-1995) established foundational measures of movement disorders and identified key environmental risks. These clinic-based studies expand

Predicting the Antigenic Relationship of Foot-and-Mouth Disease Virus for Vaccine Selection Through a Computational Model.

Foot-and-mouth disease virus (FMDV) is an antigenic-variable RNA virus that is responsible for the recurrence of foot-and-mouth disease in livestock and can be prevented and controlled using a vaccine with broad-spectrum protection. Current anti-genicity evaluation methods, which involve animal immunity experiments and serum preparation, are unable to fulfill the needs of high-throughput antigenicity measurements. This study designed an antigenicity scoring model to rapidly predict the antigenicity of FMDV. Antigenic-dominant sites were initially determined on the VP1 protein, a position-speci

Clinical Experience with Omadacycline for the Treatment of Diabetic Foot Infections and Acute Osteomyelitis.

INTRODUCTION: Diabetic foot infections (DFIs), frequently complicated by acute osteomyelitis (AOM), are associated with substantial morbidity and are increasingly difficult to manage due to antimicrobial resistance and treatment-related toxicities. Omadacycline is a broad-spectrum aminomethylcycline with favorable safety characteristics and oral bioavailability, but real-world data in DFI and AOM are limited. METHODS: We conducted a prospective, open-label, two-center study evaluating omadacycline for hospitalized adults with moderate-to-severe DFI with or without AOM who were at high risk fo

Periodic leg movements during sleep associated with antidepressants: A meta-analysis.

Studies explicitly reporting data concerning the evaluation of the effect of antidepressants on the periodic leg movements during sleep (PLMS) index obtained by polysomnography were reviewed and selected. A random-effects model meta-analysis was carried out. The level of evidence was also assessed for each paper. Twelve studies were included in the final meta-analysis, seven interventional and five observational. Most studies were characterized by Level III evidence (non-randomized controlled trials), with the exception of four studies, which were classified as Level IV (case series, case-cont

Postoperative Physical Therapy to Prevent Hospital-acquired Pneumonia in Patients Over 80 Years Undergoing Hip Fracture Surgery-A Quasi-experimental Study.

BACKGROUND: Hip fracture requiring surgical fixation is a common condition with high mortality and morbidity in the geriatric population. The patients are usually frail, and vulnerable to postoperative complications and delayed recovery. Few studies have investigated physical therapy methods to prevent hospital-acquired pneumonia (HAP) after hip fracture surgery. OBJECTIVE: To explore whether an intensified physical therapy regimen can prevent HAP and reduce hospital length of stay in patients aged 80 and older, following hip fracture surgery. PATIENTS AND METHODS: The inclusion criterion wa

Continuous intercostal nerve blockade for rib fractures: ready for primetime?

BACKGROUND: Providing analgesia for patients with rib fractures continues to be a management challenge. The objective of this study was to examine our experience with the use of a continuous intercostal nerve block (CINB). Although this technique is being used, little data have been published documenting its use and efficacy. We hypothesized that a CINB would provide excellent analgesia, improve pulmonary function, and decrease length of stay (LOS). METHODS: Consecutive adult blunt trauma patients with three or more unilateral rib fractures were prospectively studied over 24 months. The cathe

Racial and ethnic inequities to noise pollution from transportation- and work-related sources in the United States.

BACKGROUND: Racial and ethnic inequities in environmental noise exist in the US, partially attributable to historical structural racism. However, previous studies have not considered the totality of people's exposures. Since people spend most of their waking time at work, there is a need to consider cumulative exposure to noise both in and out of the workplace to understand who is most at risk of noise pollution-related adverse health outcomes. OBJECTIVES: To (1) investigate whether racial and ethnic minority communities are disproportionately burdened by transportation- and workplace-related

FPIN's Clinical Inquiries. Aspirin use in children for fever or viral syndromes.

Aspirin should not be used to treat acute febrile viral illness in children. (Strength of Recommendation [SOR]: C, based on case-control studies). Although no causal link has been proven, data from case-control and historic cohort studies demonstrate an association between aspirin use and Reye syndrome. The risk of Reye syndrome decreases with age, becoming extremely rare by the late teenage years. Other nonsteroidal anti-inflammatory drugs are effective antipyretics and are not associated with the constellation of symptoms seen in Reye syndrome, which includes nausea, vomiting, headache, exci

Change in Swallowing Function and Substance P Levels Associated with Nicergoline in Neurological Disease: A Pilot Study.

Background/Objectives: Dysphagia is a prevalent consequence of neurological conditions, particularly stroke and Parkinson's disease, leading to aspiration pneumonia and reduced quality of life. Currently, there are no specific recommendations for pharmacological therapy, although studies indicate that elevated substance P levels may improve swallowing function. While nicergoline is known to increase substance P, the role of its major metabolite, 10-methoxy-dihydro-lysergol (MDL), in this therapeutic effect remains unclear. This study examined the therapeutic effects of nicergoline and its corr

Study protocol for LOMCAD Trial: Effect of lomerizine hydrochloride to prevent recurrence of cerebral ischemic events in CADASIL patients.

OBJECTIVES: Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is one of the most common monogenic cerebral small vessel diseases. Our previous observational study suggested that lomerizine hydrochloride, a calcium channel blocker approved in Japan in 1999 for the prevention of migraine headaches, is also effective for preventing recurrent ischemic stroke in CADASIL patients. The aim of this study (LOMCAD trial) is to verify the efficacy of lomerizine hydrochloride. MATERIALS AND METHODS: This is a multicenter, prospective, single-arm trial, u
