Epidermolysis bullosa House Austria and Epidermolysis bullosa clinical network : Example of a centre of expertise implemented in a European reference network to face the burden of a rare disease.

Accurately addressing the diverse and complex issues of rare diseases (RD) in terms of prevention, recognition, diagnosis, treatment, care and research along key RD specificities, such as great heterogeneity, a limited number of patients, scarcity of relevant knowledge and expertise as well as enormous costs for patient care is a challenging task for healthcare providers and authorities that makes a supranational approach particularly feasible. The European Union has acknowledged RD matters by several initiatives, including efforts to implement national centres of expertise and European refere

[Variations in Dutch National Medical Registration hardly affect the hospital standardised mortality rate (HSMR)].

OBJECTIVE: To analyse the variation in the registration of hospital admissions across Dutch hospitals and determine how this variation affects the Hospital Standardised Mortality Rate (HSMR). DESIGN: Retrospective, descriptive. METHOD: We used data from the National Medical Registration (LMR), covering the records of all hospital admissions in 2005 in Dutch hospitals, to analyse the variation between hospitals in 3 variables: the number of secondary diagnoses, the percentage of unplanned admissions, and the percentage of non-specified diagnoses ('other diagnoses'). The impact of this variati

From Person to Patient: Power, Control and the Construction of Vulnerability in the Operating Room.

The operating room is a highly organised clinical environment in which patients experience a shift from autonomous individuals to dependent recipients of care. This transformation is often regarded as a clinical necessity, but it is also actively shaped by everyday practices, organisational structures and professional interactions. This study explored how healthcare professionals interpret mechanisms of power, control and patient vulnerability in the operating room. A qualitative design was used, employing semi-structured interviews with 32 healthcare professionals (13 perioperative nurses, 6

Quantitative patient graph analysis for transient ischemic attack risk factor distribution based on electronic medical records.

A transient ischemic attack (TIA) affects millions of people worldwide. Although TIA risk factors have been identified individually, a systemic quantitative analysis of all health factors relevant to TIA using electronic medical records (EMR) remains lacking. This study employed a data-driven approach, leveraging hospital EMR data to create a TIA patient health factor graph. This graph consisted of 737 TIA and 737 control patient nodes, 740 health factor nodes, and over 33,000 relations between patients and factors. For all health factors in the graph, the connection delta ratios (CDRs) were d

Prevalence and determinants of workplace violence of health care workers in a psychiatric hospital in Taiwan.

Workplace violence, a possible cause of job stress, has recently become an important concern in occupational health. This study determined the prevalence of workplace violence and its risk factors for employees at a psychiatric hospital in Taiwan. A questionnaire developed by ILO/ICN/WHO/PSI was first translated and validated. It was then used to survey the prevalence of workplace violence in the last 12 months experienced by all nursing aides, nurses, and clerks at the hospital. Multiple logistic regression models were constructed to discover the determinants of violence. A total of 222 out o

Total teamwork--the Mayo Clinic.

Typical of the Mayo Clinic is its century-old team approach to treating patients. Physicians work in teams, with each team driven by the medical problems involved in a case and by the patient's preferences. Occasionally, a team will be expanded or even taken apart and reassembled. At Mayo, diagnosing a complex problem, proposing treatment and slotting the patient for surgery can happen within 24 hours of the diagnosis. The overall effect at Mayo is one of orderliness, function and, above all, vigor. Even as other medical institutions are cutting staff and reducing services, Mayo is a robust, t

Predicting the "revolving door" phenomenon among patients with schizophrenic, schizoaffective, and affective disorders.

OBJECTIVE: A subpopulation of chronically mentally ill patients, sometimes referred to as "revolving door" patients, are frequently readmitted to psychiatric units. This study examined the relationships among demographic features, diagnostic characteristics, and frequency of hospitalization of patients from four state hospitals. METHOD: Two semistructured, standardized instruments, the Schedule for Affective Disorders and Schizophrenia and a life events history, were administered to 135 inpatients who met the Research Diagnostic Criteria for schizophrenia (N = 56), schizoaffective disorder (N

The emergence of AIDS in Guatemala: inpatient experience at the Hospital General San Juan de Dios.

Little is known about the effect of human immunodeficiency virus (HIV) infection on the Central American healthcare system. We describe HIV-related admissions in a Guatemalan medical service. The study was conducted at Guatemala City's largest public hospital. Data were derived from standardized data collection sheets maintained by the HIV testing service and by HIV clinic physicians. Data were collected for 295 medicine admissions of 257 HIV-infected adults during an 18-month period in 1999 and 2000; 30% of the patients were women. Average age was 33 years. Only 12.5% of the patients had been

Optimizing radiologic costs in the DRG environment.

Previously developed consensus algorithms expressing a suggested radiologic workup for the diagnostic related groups (DRGs) specified by the prospective reimbursement policy have proven to be useful tools for investigating radiologic decision making and the resulting economic implications. The mathematical equations for determining diagnostic and therapeutic costs for two alternative algorithms for suspected acute cholecystitis are formulated. Illustrative examples and graphic displays are given regarding how such algorithms and equations are useful in finding answers to questions about the ap

Emergency Service/Department as an (Un)Favorable Nursing Practice Environment.

INTRODUCTION: Emergency services/departments are complex environments, characterized by high nursing workload and unpredictable patient flow. National and International organizations recommend that health care institutions should develop strategies to promote favorable nursing practice environments. The Practice Environment Scale of the Nursing Work Index is a widely used assessment tool. METHODS: Quantitative, cross-sectional study that aimed to evaluate nurses' perceptions of the nursing practice environment in a Portuguese emergency service/department using the Practice Environment Scale o

Methadone Dose and Patient-Directed Discharge in Hospitalized Patients With Opioid Use Disorder.

IMPORTANCE: Patient-directed discharge (PDD), when patients leave the hospital prior to completing recommended medical treatment, is associated with increased morbidity and mortality and occurs in 10% to 20% of hospitalizations for patients with opioid use disorder (OUD). Understanding risk factors associated with PDD is essential to improving outcomes for this population. OBJECTIVE: To investigate whether hospitalized patients with OUD who received higher doses of methadone during the first 24, 48, and 72 hours after first contact with the emergency department had decreased odds of PDD. Nurses' and parents' perceptions of participation and partnership in caring for a hospitalized child.

A descriptive study was undertaken to determine whether parents were able to participate in the care of their hospitalized child and work in partnership with the nurses. The study utilized method triangulation and was conducted in two phases. In phase one a postal survey of 65 nurses was undertaken in three children's wards within an NHS trust hospital, while in the second phase, 12 in-depth interviews were completed with parents in their own homes following their child's discharge from hospital. The quantitative data were analysed using descriptive statistics and the qualitative data were sub
