Intra-articular corrective osteotomy for the malunited intercondylar humeral fracture: a case report.

A 35-year-old patient sustained comminuted intercondylar fracture of the distal humerus. At six months post-open reduction and internal fixation, the malunion was corrected with an intra-articular osteotomy. The patient obtained a painless, functional elbow joint with increased grip strength after corrective osteotomy, although she had complained of severe elbow pain, limited range of motion, and loss of grip strength before osteotomy. A 15 degrees cubitus varus deformity was also corrected . Radiographs of the elbow joint did not show accelerated degenerative changes at over three years follo

The open reduction and internal fixation of humeral diaphysis fracture treatment with a medial approach.

From 1995 to 2000, twenty-three patients with fracture of the humeral shaft were treated by medial approach and internal fixation using AO/ASIF plate and screws. Follow-up was possible for 21 fractures. There were 16 male and 5 females. The ages ranged from 16 to 55 years (mean, 29 years). The average period of follow-up was 4.5 months (range, 3-12 months). There was no evidence of implant failure and nonunion during follow-up. The average lack of complete extension of the elbow was five degrees. There was one case of transient radial nerve palsy. As such, the medial approach to humeral shaft

Vascular injuries and their sequelae in pediatric supracondylar humeral fractures: toward a goal of prevention.

Between 1988 and 1994, 128 consecutive children with grade III supracondylar humeral fractures presented for treatment at our hospital. Seventeen had absent or diminished (detected with Doppler but not palpable) radial pulses on initial examination. Fourteen of these 17 children recovered pulse (palpable) after reduction and stabilization of their fractures. The remaining three had persistent absence of radial pulse. Each of these three children was explored immediately and found to have a significant vascular injury requiring repair. Two of the 14 children who had initially regained their pul…

Posterolateral dislocation of the elbow with concomitant fracture of the lateral humeral condyle: case report.

We describe a case of a 6-year-old girl with a posterolateral elbow dislocation and a concomitant fracture of the lateral humeral condyle. After reduction of the dislocation, the fracture was diagnosed and treated by open reduction and fixation, with a good functional result. In doubtful cases, oblique, heterolateral, and varus stress films, or even arthrography may be necessary.

[Sequestered bone infection after Kirschner wire osteosynthesis of humeral head fracture].

Percutaneous stabilization of fractures of the humeral head by K-wires is a proved therapy. This proceeding provides enough stability for early moving of the joint and has a low incidence for complications. In the case described here the special is the cause for local infection: local osteonecrosis induced by drilling-holes too close together.

Plate fixation of the humeral shaft for acute fractures, with and without radial nerve injuries.

We report herein the results of operatively treating 44 consecutive acute fractures of the humeral shaft using plates for internal fixation. Ninety-seven percent (43 of 44) of the fractures healed after plate fixation at an average of 12 weeks. One short plate pulled out and required revision with a longer plate and bone grafting; this fracture also healed. Cancellous bone grafting successfully filled in cortical bone gaps. Range of motion of the shoulder and elbow were essentially normal. Plate size varied depending on the location of the fracture and bone size. Eight of nine fractures treate

[Indications and technique of osteosynthesis in dislocated fractures of the humeral head in adults (author's transl)].

Fractures of the proximal end of the humerus are treated by early functional movement. Dislocated fractures of the humeral head should be treated by means of osteosynthesis. In younger patients osteosynthesis with a T- or L-shaped plate is the method of choice; in older patients, open reduction and K-wire fixations can be used. Indications for screw osteosynthesis are rare, and alloarthroplasty is a procedure reserved for exceptions. The functional results are demonstrated by 24 cases with dislocated fractures who were operated on, thus showing the principles of operative treatment.

[Results of an operative therapy of dislocated fractures of the humeral head in adults (author's transl)].

Dislocated fractures of the humeral head are problematic injuries. The result of an operative therapy of 30 adult patients are given. Intra- and postoperative complications, advantages and disadvantages are pointed out. Traumatic and posttraumatic damages of the cartilage, partial necrosis of humeral head and the combination with lesions of the soft tissues and nerves are the limits to a good functional result.

Does excessive lateralization influence tuberosity healing in reverse total shoulder arthroplasty for proximal humeral fractures?

BACKGROUND: This study evaluated the influence of stem global lateralization on tuberosity consolidation and functional outcomes in patients undergoing reverse total shoulder arthroplasty (rTSA) for proximal humeral fractures (PHFs). METHODS: A retrospective, two-institution study included patients who underwent rTSA for acute PHFs implanted with one of 3 stems: the Arrow (global lateralization 21.4 mm), the Humelock II (11.2 mm), or the Unique (14.7 mm). Stems were classified as very highly lateralized (VHL; >20 mm, Arrow) or lateralized (L; 10-15 mm, Unique and Humelock II). Tuberosity cons…

Incidence and Risk Factors of Ulnar Neuropathy After the Surgical Treatment of Distal Humeral Fractures.

BACKGROUND: One of the major complications associated with open reduction and internal fixation (ORIF) for distal humeral fractures is ulnar neuropathy. However, varying reports on its incidence have led to inconsistent opinions. This study aimed to investigate the incidence of ulnar neuropathy in patients who underwent open reduction and internal fixation using two precontoured anatomical locking plates for distal humeral fractures at our hospital. Additionally, we aimed to examine the symptoms and progression of ulnar neuropathy in these cases. METHODS: Among patients who underwent surgery …

Elastic intramedullary nailing: a viable solution to stabilise adult humeral diaphyseal fractures? A systematic review.

PURPOSE: Conservatively managed diaphyseal humeral fractures have demonstrated higher non-union rates than previously reported and there are drawbacks to functional bracing. There is a growing trend towards operative management of these injuries, yet there are several complications associated with current operative methods. This systematic review aimed to evaluate the role of elastic stable intramedullary nailing (ESIN) as a solution to mitigate these risks. METHODS: A systematic review and meta-analysis was performed using PubMed, EMBASE, MEDLINE and Cochrane databases. Methodology followed …

Outcomes for Completely Displaced and Rotated Lateral Humeral Condyle Fractures in Children: A Multicenter Study.

INTRODUCTION: Song V lateral humeral condyle fractures represent a displaced and rotated fragment, historically treated with open reduction and percutaneous pinning (ORPP) or screw fixation (ORSF). We aim to describe outcomes of Song V fractures and analyze the effect of fixation and reduction type on outcomes. METHODS: This retrospective review assessed data from 6 level 1 trauma centers from 2005 to 2019. Inclusion criteria were children aged 1 to 12 with a Song V fracture. Patient clinic and surgical notes until 1 year after the primary intervention were reviewed. Radiographic healing data

Percutaneous Screw Fixation for the Management of Delayed Unions in Pediatric Lateral Humeral Condylar Fractures.

BACKGROUND: The use of in situ screw fixation for the treatment of pediatric lateral humeral condyle (LCH) delayed unions has recently been reported. While attractive, the currently available literature consists of small retrospective studies with mixed results. As such, the present study aims to describe the outcome of a group of children with delayed unions of LCH fractures treated with in situ fixation using a single, percutaneously placed cannulated screw in compression. METHODS: A retrospective review of all pediatric patients (below 18 y) with lateral condyle fractures of the humerus tr
