Pseudohypoparathyroidism is a rare endocrine disorder with an estimated prevalence of 1/100,000. It is characterized by hypocalcemia and hyperphosphatemia in the absence of vitamin D deficiency or impaired renal function. Research studies during the last 20 years have led to the identification of the molecular underlying cause of the disease, the characterization of the clinical and biochemical characteristics and the observation of an overlap between genetic and clinical manifestations. The creation of networks both for specialists (including endocrinologists, pediatricians, dermatologists, g…

Tumor lysis syndrome (TLS) is a potentially deadly complication of tumors or their treatment. This syndrome consists of a constellation of laboratory parameters such as hyperuricemia, hyperkalemia, hyperphosphatemia, and hypocalcemia and clinical complications such as seizures, acute renal insult, cardiac dysrhythmias and death. TLS is especially common in patients with hematological malignancies with rapid cellular turnover rates such as acute lymphocytic leukemia and Burkitt lymphoma, but is very rare in patients with solid tumors. However, it is essential to keep in mind that solid tumors c…

In recent years the interest in understanding the clinical significance and the pathogenic mechanisms involved in vascular calcification has rapidly increased. There are three reasons that explain this increasing interest in vascular calcification by nephrologists: a) cardiovascular disease is the leading cause of death in uremic patients; b) hyperphosphatemia is a predictor of death and bone mineral disorders are not only responsible for renal osteodystrophy but are also involved in the pathophysiology of vascular calcification, and c) vascular calcification in large arteries reduces vascular…

Control of hyperphosphatemia is a major clinical challenge in children with end-stage renal disease. The best treatment option is a calcium-based phosphate binder, but there is evidence that this treatment protocol causes long-term complications, including vascular calcification. Sevelamer hydrochloride has been shown to be effective in reducing serum phosphorus levels and the calcium-phosphorus ion product in children on maintenance dialysis and may be key in preventing soft-tissue and vascular calcifications in such patients. Until the U.S. Food and Drug Administration approves the use of se…

Abnormal mineral metabolism has the potential to significantly increase mortality in patients with chronic kidney disease, especially by the time renal replacement therapy is required. While excess phosphorus and hyperphosphatemia have long been identified as risks for increased morbidity and mortality, the roles of calcium load and serum calcium levels have been less clear. Calcium-based recommendations in the K/DOQI Clinical Practice Guidelines for Bone Metabolism and Disease in Chronic Kidney Disease were labeled as expert opinion. The rationale behind the opinion-based recommendations is r…

During the past decade considerable progress has been made in the field of calcium metabolism associated with chronic renal failure. The main factor influencing the pathophysiology of calcium homeostasis is its dependence on parathyroid hormone and active vitamin D metabolites. Among several new diagnostic tools, and to establish the diagnosis of renal osteodystrophy, only the measurement of i-PTH activity has any clinical relevance. The main goal in treating renal osteodystrophy is the lowering of PTH activity by correcting hyperphosphatemia and administration of calcitriol given either orall…

The concurrent use of calcitriol (CAL) pulse therapy to reduce parathyroid hormone (PTH) secretion and of calcium (Ca) salts as the most appropriate phosphate binders was evaluated for over 1 year in a group of 14 patients with good divalent ion control on CaCO3 therapy but with increasing levels of serum intact PTH. CAL pulse therapy was effective and safe in only 2 patients; in the remaining subjects it resulted in hypercalcemia and/or hyperphosphatemia, not reversed by adjusting the dialysate Ca concentration and or CaCO3 dose, and had to be stopped. Therefore, CAL pulse therapy does not se…

Dysregulated mineral metabolism is a nearly universal sequalae of acute kidney injury (AKI). Abnormalities in circulating mineral metabolites observed in patients with AKI include hypocalcemia, hyperparathyroidism, hyperphosphatemia, decreased vitamin D metabolite levels, and increased fibroblast growth factor 23 levels. We review the pathophysiology of dysregulated mineral metabolism in AKI with a focus on calcium, phosphate, parathyroid hormone, and vitamin D metabolites. We discuss how mineral metabolite levels can serve as novel prognostic markers for incident AKI and other related outcome…

We describe a case of a 40-year-old male patient who was found to have multiple myeloma with spontaneous tumour lysis syndrome (TLS), following a compression fracture of the L-2 vertebrae. Multiple myeloma was confirmed by bone marrow analysis and the M-band on serum protein electrophoresis. Hyperuricaemia (26.2 mg/dL), hyperkalaemia (> 7.0 mEq/L), hyperphosphatemia (16.2 mg of phosphorus/dL), normocalcemia and acute kidney injury, prior to anticancer treatment suggested spontaneous TLS. Inciting events for tumour lysis, such as chemotherapy, dehydration and exposure to steroids were absent. P…

Pseudohypoparathyroidism (PHP) is a heterogeneous group of disorders characterized by hypocalcemia, hyperphosphatemia, increased serum concentration of parathyroid hormone (PTH), and insensitivity to the biological activity of PTH. Pseudohypoparathyroidism is rare sporadic or autosomal dominant disorder having several subtypes (type Ia, Ib, Ic, type II). We report a case who was 27 years old married lady having two children presented in the summer season 2009. Her trousseau's sign was positive, serum calcium 5.5 mg/dl, PTH 137 pgm/ml and CT scan of brain shows bilateral and symmetrical calcifi…

Low-molecular-weight heparins are routinely used to prevent deep venous thrombosis following renal transplantation in our department. We rep … recovery several weeks after discontinuation of nadroparin. Patients with chronic renal failure and hyperphosphatemia may be predisposed to develop calcinosis cutis. In addition, the role of the calcium content of nadroparin is discussed.

Hyperphosphatemic familial tumoral calcinosis (HFTC) is a rare, autosomal recessive condition characterized by fibroblast growth factor 23 signaling pathway dysregulation, hyperphosphatemia and ectopic calcifications (which manifest as joint motion limitations), inflammatory bony pain, and disability. Given the rarity and multiorgan involvement of HFTC, a multidisciplinary approach including Dermatology, Ophthalmology, Dentistry, Nephrology, Endocrinology, Rheumatology, and Genetics is necessary for diagnosis and treatment. We present a multidisciplinary diagnostic and treatment approach for a…

OBJECTIVE: Dietary phosphorus restriction and intake adequate amount of protein play a prominent role in control of hyperphosphatemia and prevention of protein-energy malnutrition for chronic kidney disease (CKD) and dialysis patients. To reduce dietary phosphorus intake, we focused on boiling, which can affect the bioavailability of nutrients by changing mineral solubility and breaking down food structure. The aim of this study was to assess the effect of boiling on the phosphorus and protein content of beef. We boiled beef in differing fluid mediums, varied the size of the meat, method of bo…

Excess and deficient actions of FGF23 cause hypophosphatemic rickets/osteomalacia and hyperphosphatemic tumoral calcinosis, respectively. There are two kinds of assays for FGF23. Full-length assay detects only full-length FGF23 with biological activity. In contrast, C-terminal assay measures both full-length FGF23 and processed C-terminal fragment of FGF23. FGF23 measurement is useful for the diagnosis and follow-up of patients with FGF23-related hypophosphatemic diseases. In addition, full-length assay can be used for the differential diagnosis of hypophosphatemic diseases. Furthermore, many

Hyperphosphatemia is present in nearly all patients with kidney failure who receive maintenance dialysis. The direct toxicity of elevated phosphate concentrations had been inferred from basic experimentation and large epidemiologic studies. This has influenced clinical practice in which great efforts are expended in normalizing serum phosphate concentration. Targeted dietary strategies, optimization of dialysis and phosphate-lowering medications are all effective at lowering serum phosphate levels yet each pose challenges for patients and clinicians. The effect of these interventions, individu…

Tumor lysis syndrome (TLS) is a life-threatening condition due to the rapid release of tumor cell contents into the bloodstream usually induced by cytotoxic therapy. It is characterized by the rapid onset of severe metabolic derangements such as hyperuricemia, hyperkalemia, hyperphosphatemia, and hypocalcemia. If not promptly recognized and treated, TLS can lead to serious clinical complications including acute kidney injury (AKI), cardiac arrhythmias, seizures, hypotension, and death. In particular, the occurrence of AKI could cause a delay in the administration of chemotherapy, thus compromi…

Patients with hypoparathyroidism can present with concurrent basal ganglia calcifications (BGCs). The exact pathogenesis is unknown, although it is thought to relate to calcium-phosphate deposition from chronic hypocalcemia and hyperphosphatemia. We present the case of a 65-year-old man with known idiopathic primary hypoparathyroidism and concurrent extensive BGC. Thirty years after diagnosis, he presented with focal seizures despite a decade of stable intracranial calcifications on imaging. Serum calcium, phosphate, 25-hydroxyvitamin D, and parathyroid hormone levels were well controlled duri…
