An Immune Recovery-Based Revaccination Protocol for Pediatric Hematopoietic Stem Cell Transplant Recipients: Revaccination Outcomes Following Pediatric HSCT.

Following hematopoietic stem cell transplant (HSCT), patients are at increased risk of vaccine-preventable diseases (VPDs) and experience worse outcomes of VPDs compared to immunocompetent patients. Therefore, patients are routinely vaccinated post-HSCT to restore VPD immunity. Published guidelines recommend revaccination based on time post-HSCT, although optimal revaccination timing and the value of using other clinical and laboratory variables to guide revaccination remain unclear. An institutional immune recovery-based protocol to guide timing of revaccination is used at Children's Hospital

Impact of BCG revaccination on the response to unrelated vaccines in a Ugandan adolescent birth cohort: randomised controlled trial protocol C for the 'POPulation differences in VACcine responses' (POPVAC) programme.

INTRODUCTION: There is evidence that BCG immunisation may protect against unrelated infectious illnesses. This has led to the postulation that administering BCG before unrelated vaccines may enhance responses to these vaccines. This might also model effects of BCG on unrelated infections. METHODS AND ANALYSIS: To test this hypothesis, we have designed a randomised controlled trial of BCG versus no BCG immunisation to determine the effect of BCG on subsequent unrelated vaccines, among 300 adolescents (aged 13-17 years) from a Ugandan birth cohort. Our schedule will comprise three main immunisa

New approaches to TB vaccination.

Pulmonary TB remains a leading global health issue, but the current Bacille Calmette-Guérin (BCG) vaccine fails to control it effectively. Much effort has gone into developing safe and effective boost vaccine candidates for use after the BCG prime vaccination. To date, almost all the lead candidates are being evaluated clinically via a parenteral route. Abundant experimental evidence suggests that parenteral boosting with a virus-based vaccine is much less effective than respiratory mucosal boosting, because the former fails to activate a type of T cell capable of rapidly transmigrating into t

The dynamical consequences of seasonal forcing, immune boosting and demographic change in a model of disease transmission.

The impact of seasonal effects on the time course of an infectious disease can be dramatic. Seasonal fluctuations in the transmission rate for an infectious disease are known mathematically to induce cyclical behaviour and drive the onset of multistable and chaotic dynamics. These properties of forced dynamical systems have previously been used to explain observed changes in the period of outbreaks of infections such as measles, varicella (chickenpox), rubella and pertussis (whooping cough). Here, we examine in detail the dynamical properties of a seasonally forced extension of a model of infe

Limb swelling in a pediatric sickle cell patient after revaccination with pneumococcal vaccine.

CASE (DESCRIPTION): This report describes an incident of limb swelling after revaccination with the 23-valent pneumococcal polysaccharide vaccine (PPSV23). A 5-year old female with sickle cell disease experienced severe arm swelling with edema to the extent that it was difficult to put a shirt over the arm. The day prior, she received PPSV23 concomitantly with the meningococcal vaccine. She had received the PPSV23 vaccine 2.5 years prior to the event. The patient was managed with analgesics, antiemetics, and antibiotic prophylaxis and the arm swelling resolved 24 h later. A decreased length of

Avidity maturation of anti-citrullinated protein antibodies in rheumatoid arthritis.

OBJECTIVE: Anti-citrullinated protein antibodies (ACPAs) are highly specific for rheumatoid arthritis (RA) and are present years before the onset of symptoms. The avidity of autoantibodies can have a strong impact on their effector potency. This study was undertaken to analyze the avidity of ACPAs in serum samples obtained from ACPA-positive healthy individuals (predisease), patients with early disease, and patients with established RA as well as the avidity maturation over time in samples from healthy subjects who later developed RA. METHODS: We measured ACPA avidity in serum samples from AC

Booster vaccination at 6-8 years of age with a reduced antigen content dTpa-IPV vaccine is immunogenic and safe after priming with whole-cell pertussis vaccine.

AIM: Microplastics, defined as plastic particles smaller than 5 mm and larger than 1 micrometer, along with nanoplastics (in the 1-1000 nm range), are ubiquitous environmental contaminants originating from both primary production and secondary fragmentation. Little is known about exposure to microplastics in ambient air and their health effects. This review summarises current knowledge regarding microplastics in ambient air and their potential health effects. METHODS: To capture the limited available evidence on airborne microplastics exposure and human health, a search strategy using five co

Hepatitis B vaccine boosters: is there a clinical need in high endemicity populations?

The Steering Committee for the Prevention and Control of Infectious Diseases in Asia recently conducted a survey of primary-care physicians in Asia, which revealed that many physicians administer boosters in their clinical practice and that there is considerable variation and uncertainty among physicians regarding this practice. This paper serves as a response to physicians' uncertainties by reviewing the literature regarding the administration of hepatitis B vaccine boosters in high endemicity areas and presenting the Steering Committee's guidelines for booster administration. While there are

[Evaluation on the effect of supplementary immunization for poliomyelitis elimination in Hainan province].

We appraised the effect on poliomyelitis elimination programme in Hainan Province, based on data as incidences of poliomyelitis in last years, the routine immunization and the activities on National Immunization Day (NID) and Provincial Immunization Day (PID). The result indicated that the effect of NID in 1993/1994 was remarkable. In Hainan Province, there has been no poliomyelitis case caused by wild strain of poliovirus since 1994. Laboratory data also demonstrated that NID could break the circulation of wild strain of type I poliovirus. We found that routine immunization alone could not ef

Booster effect in delayed hypersensitivity skin testing.

After a pilot study had suggested that a drug augmented delayed hypersensitivity, we conducted a control study in which 19 normal volunteers had the same skin test battery applied as in the pilot study, on two occasions one month apart with no intervening treatment. No change was observed in delayed hypersensitivity response to PPD, Candida, or tetanus antigens. However, a significant decrease in response to mumps antigen was observed, and there was a marked "booster" or augmented response to SKSD antigen. These observations have implications for studies of immunomodulation and the mechanism o

Vaccination during pregnancy and modulation of IgG response to pertussis vaccines in infants: The impact of different vaccine formulations.

The IgG response following infant diphtheria-tetanus-acellular pertussis (DTaP) immunization is influenced by the formulation of the infant and/or the adult vaccine (Tdap) given during pregnancy. DTaP vaccines containing either 3 (DTaP3) or 5 (DTaP5) pertussis antigens are commonly used. By conducting a secondary analysis of a large randomized controlled trial, we compared IgG levels against pertussis vaccine antigens in children of Td- and Tdap5-vaccinated mothers, after stratifying by infant vaccine formulation. After immunization with a primary series of DTaP5, but not DTaP3, IgG GMCs again

Generation and evaluation of a recombinant Newcastle disease virus strain R2B with an altered fusion protein cleavage site as a vaccine candidate.

Newcastle disease (ND) is a highly contagious and fatal disease of chickens. Newcastle disease virus (NDV) strain R2B is an Indian mesogenic strain used for secondary vaccination in chickens. Mesogenic strains have increased virulence and immunogenicity but may cause disease in vaccinated birds, thus rendering them ineffective for use. In this study, we generated a recombinant NDV by changing the fusion protein cleavage site of mesogenic rNDV-R2B from a polybasic amino acid motif RRQKRF to a dibasic amino acid motif GRQGRL leading to generation of an attenuated virus, rNDV-R2B-FPCS. The modifi

Induction of protective immunity to Listeria monocytogenes in neonates.

Neonates suffer unduly from infections and also respond suboptimally to most commonly used vaccines. However, a CD8 T cell response can be elicited in neonates if the Ag is introduced into the cytoplasm of APCs. Listeria monocytogenes (Lm) targets the cytoplasm of APC and is a strong CD8 and CD4 Th1-promoting vaccine vehicle in adult mice. We hypothesized that an attenuated strain of Lm would be safe and induce long-lasting protective immunity, even in neonates. We found that neonatal mice immunized only once with the attenuated strain DeltaactA-Lm developed robust primary and secondary CD8 an

Assessing the impact of serostatus-dependent immunization on mitigating the spread of dengue virus.

Dengue is the most rapidly spreading mosquito-borne disease that poses great threats to public health. We propose a compartmental model with primary and secondary infection and targeted vaccination to assess the impact of serostatus-dependent immunization on mitigating the spread of dengue virus. We derive the basic reproduction number and investigate the stability and bifurcations of the disease-free equilibrium and endemic equilibria. The existence of a backward bifurcation is proved and is used to explain the threshold dynamics of the transmission. We also carry out numerical simulations an

Dynamics and function of Langerhans cells in vivo: dermal dendritic cells colonize lymph node areas distinct from slower migrating Langerhans cells.

Langerhans cells (LCs) are prominent dendritic cells (DCs) in epithelia, but their role in immunity is poorly defined. To track and discriminate LCs from dermal DCs in vivo, we developed knockin mice expressing enhanced green fluorescent protein (EGFP) under the control of the langerin (CD207) gene. By using vital imaging, we showed that most EGFP(+) LCs were sessile under steady-state conditions, whereas skin inflammation induced LC motility and emigration to lymph nodes (LNs). After skin immunization, dermal DCs arrived in LNs first and colonized areas distinct from slower migrating LCs. LCs

In vivo immunomodulatory effects of dietary purple sweet potato after immunization in chicken.

This study was intended to determine the modulatory effects of dietary supplementation of purple sweet potato (Ipomoea batats Poir., PSP) on the immune response of chickens. PSP was included in a basal starter diet by 1% (PSP(L)) or 3% (PSP(H)) and continually fed. Newcastle disease (NDV) vaccine, Brucella abortus (BA) and sheep red blood cells (SRBC) were used for chicken immunization. Antibody titers against these antigens were used to estimate humoral immunity. Concanavalin A (Con A)-induced proliferations of splenocytes, thymocytes and peripheral blood lymphocytes (PBL), ratios of CD4- and
