Post-discharge nutrition: what does the evidence support?

Although there are no official recommendations for specific nutrient intakes in premature infants after hospital discharge, it is agreed that the goal should be to achieve the body composition and rate of growth of that of a normal fetus of the same postmenstrual age during the entire first year of life. A general recommendation to use the special formulas designed for preterm infants after hospital discharge in place of the formulas for term infants cannot be made from the available evidence at this time. Infants fed human milk after discharge are of the greatest concern as human milk does no

Dietary treatment of lactose intolerance in infants and children.

During the past several years there have been many reports of alternative dietary therapies for primary and secondary lactose intolerance. We have learned that it is useful to feed through most episodes of mild diarrhea that previously would have been treated with clear liquid diets. Infant formulas, including both soy-protein and hydrolysate formulas with specially designed carbohydrate, protein, and fat components, are available to treat the infant with diarrheal syndromes and secondary lactase deficiency. Also, the diet can be supplemented with lactase. Specialized lactose-reduced products …

Early infant feeding decisions in low-income Latinas.

Breastfeeding rates remain low, especially among low-income minority women. The objective of this qualitative study was to assess barriers to breastfeeding and reasons for combination feeding among low-income Latina women and their families. Meetings were held with key informants to inform the sampling plan and develop questions for focus groups. Data were collected from eight qualitative focus groups with primiparous mothers postpartum, mothers breastfeeding at 4 to 6 months, mothers formula feeding at 4 to 6 months, grandmothers and fathers, and 29 individual interviews with formula- and com

The reasons for early weaning among mothers in Teheran.

In order to identify the reasons for early weaning in Teheran, we interviewed 900 mothers using a systematic randomized sampling method. A total of 15% of the mothers were illiterate, 93% were housewives, and 97% had given birth in hospitals. Only 3% of the newborns benefited from rooming-in facilities in hospital, and 68% were bottle-fed while still in hospital. In 3.1% of cases the mother had not breast-fed her newborn at all. Of those who had breast-fed their infant, 38% used only their own milk, whereas 62% used a combination of breast milk and infant formula. The median duration of breast…

Early-Life Dietary Exposure to Perfluorooctanoic Acid (PFOA) Through Milk Consumption: A Systematic Review.

Perfluorooctanoic acid (PFOA) has raised public concern due to its widespread presence/use and toxic health effects including hepatotoxicity, neurotoxicity, and developmental toxicity. Because dietary intake is a major route of PFOA exposure, and milk is a primary source of nutrition in early life, the present systematic review discusses PFOA occurrence in milk samples and the employed determination methods. In the present article, 69 studies (published 2007-2024) reporting PFOA levels in infant formula, commercial milk, and human breast milk were included. The highest concentration of PFOA in…

Standardized ketogenic dietary preparation for metabolic PET imaging in suspected and known cardiac sarcoidosis.

AIMS: A major limitation of cardiac positron emission tomography (PET) with F18-fluorodeoxyglucose (F18-FDG) for the evaluation of cardiac sarcoidosis (CS) is associated with physiologic myocardial glucose uptake. The optimal dietary protocol to suppress physiologic myocardial F18-FDG uptake is not well-established. We aimed to evaluate the diagnostic performance of a novel dietary preparation using a ketone-based infant formula. METHODS AND RESULTS: Between 2018 and 2021, consecutive studies using a ketogenic dietary preparation were identified (n = 198). The rate of non-diagnostic studies d…

The new "breast from birth" growth charts. An updated version of the paper given at the Primary Care Conference and Exhibition, May 2003.

The Child Growth Foundation has published the first weight reference charts specifically designed for the breast-fed infant. Based on a sample of 120 long-term British breast-fed babies, the charts demonstrate the particular growth pattern of breast-fed babies. This differs from formula-fed infants in that breast-fed babies initially gain weight more rapidly, but from two to three months of age their weight gain decreases and they begin to move downwards across centiles. At present breast-fed babies are measured against the standard British 1990 weight reference chart rather than one that refl

Peptide molecular lock-engineered nanobodies enable an oriented dual-modal immunoassay for reliable detection of Cronobacter sakazakii.

Conventional nanobody ELISAs for trace Cronobacter sakazakii in powdered infant formula suffer from random orientation and low signal output. We developed an oriented dual-modal immunoassay that combines site-specific biotinylation via a C-terminal AviTag and a peptide molecular lock, enabling controlled surface orientation while preserving nanobody structural integrity. This strategy was further integrated with phage-displayed nanobodies for multivalent amplification and both fluorescent and colorimetric readouts. The assay exhibited a broad linear range of 103-106 CFU/mL, with limits of dete…

Bacteriological screening of expressed breast milk revealed a high rate of bacterial contamination in Chinese women.

A screening programme for expressed breast milk (EBM) revealed the alarming fact that our study group had the highest rate of contamination … for one month after childbirth. Previous studies have shown that feeding EBM rather than premature infant formula milk has advantages in terms of decreased incidence of necrotizing enterocolitis and neonatal sepsis. However, in this population, with such a high incidence of contaminated EBM, this may not be the case. Further studies to compare EBM with prematur …

Perinatal and infant nutrition. Nucleotides.

Nucleotides (NT) are ubiquitous intracellular compounds of crucial importance to cellular function and metabolism. Much recent interest has focused on NT as components of the non-protein nitrogen fraction of human milk. NT supplementation of infant formula has now been introduced in several countries. Biological effects of NT have been reported in several fields. Dietary NT have been shown to have important effects on several components of the immune system: they may enhance intestinal absorption of iron; they affect lipoprotein and long-chain polyunsaturated fatty acid metabolism; they may al…

[Serum gastrin levels and clinical conditions in response to early minimal feeding in premature infants].

OBJECTIVE: To study serum gastrin levels in response to early minimal feeding in premature infants and evaluate the clinical effect of early minimal feeding. METHODS: Premature infants with critical score < or = 90 were randomly assigned into two groups: early minimal feeding group (n = 48), non-early minimal feeding group (n = 47). Other premature infants (n = 30) without any complications (critical score > 90) were assigned as normal control group. The premature infants in normal control group were fed with water at 6 h after birth, 1 - 2 ml/kg every time, after once or twice, they were fed

Cracking the Codes for Congenital Diarrhea and Enteropathies (CoDEs): A Case Report and Review.

Congenital diarrhea and enteropathies (CoDEs) condition is a rare cause of chronic diarrhea in infants that can be challenging to diagnose. This article discusses key signs to recognize in considering a CoDEs diagnosis and provides an overview of the diagnostic process. We report a late preterm twin infant with intractable watery diarrhea starting shortly after birth. The infant was born from a twin pregnancy to non-consanguineous parents with an unremarkable family history. His twin brother had no complications after birth and continued to thrive. The patient initially presented with bloody s

Calcium and phosphorus metabolism in full-term infants.

Appropriate intake of dietary calcium and phosphorus is essential for the maintenance of mineral homeostasis and support of adequate bone mineralization in growing infants. Disturbances of calcium homeostasis such as newborn hypocalcemia, have been shown to be secondary to high oral phosphate intake. The incidence of hypocalcemic tetany has changed from epidemic proportions in newborns, who were formerly fed evaporated cow's milk formula, to the present sporadic occurrence in newborns who are now fed humanized cow's milk formula, with a calcium concentration of about 50 mg/dl and a phosphorus

Sleep SAAF responsive parenting intervention improves mothers' feeding practices: a randomized controlled trial among African American mother-infant dyads.

BACKGROUND/OBJECTIVE: Parents shape children's early experiences with food, influencing what is served, children's food choices, and how much children eat. Responsive parenting (RP) interventions such as INSIGHT have improved maternal infant feeding practices, but have only been tested among predominantly White families. This secondary analysis of data from the Sleep SAAF (Strong African American Families) RCT tests the effects of an RP intervention designed to prevent rapid infant weight gain on African American mothers' infant feeding practices. METHODS: Primiparous African American mother-
