Psychological implications of high-risk pregnancy.

The psychological adjustments of "normal" pregnancy are complex, and those of high-risk pregnancy are even more pronounced and severe. A pregnancy may be determined to be at high risk because of obstetric factors in previous pregnancies or the present one; more general medical factors, such as preexisting or emergent disease (often, diabetes); and conditions that are, themselves, psychosocial: anxiety disorders (GAD, OCD, panic disorder, PTSD), mood disorders, and schizophrenia, all of which are a background for a disturbed pregnancy and might complicate a pregnancy denominated high risk for s

[Early post-partum discharge : systematic review of the literature].

BACKGROUND: Hospital stays after vaginal delivery and cesarean birth have shortened over the past 30 years, that practice have contributed to the controversy about the safety of this policy for mother and infants. OBJECTIVE: To carry out a systematic review of the literature to determine the actual evidence of the effect of early post-partum and post-cesarean discharge on maternal and neonatal readmissions, breast feeding, maternal satisfaction with medical care and post-partum depression. METHODOLOGY: We included all randomized controlled trials that evaluate different institutional policie

[Caries prevalence and state of treatment in the case of Koblenz children in the first school class with reference to district-related social indicators].

The findings of the dental medical examinations of the school years 1996/1997 and 1997/1998 in the first classes of primary schools in Koblenz have been associated with district-related social indicators given in the social report for Koblenz, and more especially the "proportion of recipients of social welfare assistance among the 0-13 year-olds" indicator. The caries prevalence indices as well as the treatment deficits, the proportion of those needing treatment and the proportion of children with increased risk of caries clearly increased parallel with the proportion of social welfare assista

Fatalities assessed by the Orange County child death review team, 1989 to 1991.

Interagency child death review teams have emerged in response to the increasing awareness of severe violence perpetrated against children in the United States. Child death review involves a systematic, multidisciplinary, and multiagency process to coordinate data and resources from the coroner, law enforcement, the courts, child protective services, and health care providers. The Orange County, CA team reviews all coroner's cases (unattended death or questionable cause of death) for children 12 years old and younger. This paper describes the interagency review in Orange County and provides dat

Child mental health in the Americas: a public health approach.

The systematic, population-wide application of preventive measures based on what is known about the causes and outcomes of psychiatric disorders can markedly reduce morbidity from mental ill health among children in the Americas. The actions proposed here rely partly upon increasing access for all women and their children to thoroughly tested obstetric and pediatric care; in part they depend on improving nutrition and opportunities for cognitive stimulation; and in part they call for enhancing the mental health skills of primary care practitioners by appropriate in-service training. There are

Does paternal criminal legal system involvement increase children's risk of child protective services contact? Evidence from Wisconsin linked administrative data.

BACKGROUND: Research shows a strong overlap between paternal criminal legal system (CLS) involvement-especially incarceration-and children's involvement with Child Protective Services (CPS) and foster care placement. OBJECTIVE: To examine three explanations for the CLS-CPS overlap: (1) selection processes where a common set of factors drive CLS and CPS involvement, (2) mechanical processes, such as mandatory reporting by law enforcement to CPS, or (3) consequences that stem from paternal CLS involvement, such as reduced economic resources or increased harsh parenting. Reproductive justice for incarcerated mothers and advocacy for their infants and young children.

The United States has seen unprecedented growth in the number of incarcerated women, most of whom are mothers with minor children. Major public health concerns relate to the reproductive health of women in prisons and jails and the well-being of their infants and young children. In the current article, we use a reproductive justice framework to examine the intersection of incarceration and maternal and child health. We review (a) current research on the reproductive health of incarcerated women, (b) characteristics and experiences of pregnant incarcerated women, (c) outcomes of infants and you

Out of Africa: traveller malaria in paediatric patients presenting to Our Lady of Lourdes Hospital Drogheda.

Malaria is one of the world's most important parasitic infections and is endemic in over 100 countries. The number of cases of malaria in Ireland has increased significantly over the last 10 years. The aim of this study was to retrospectively examine cases of paediatric malaria presenting to Our Lady of Lourdes Hospital Drogheda over a one year period to identify the reason for travel to endemic areas, to establish if prophylaxis was received on travelling and to determine the clinical course and post-treatment follow up. Five children presented during the study period, all children were infec

Epidemiology and burden of malaria in pregnancy.

We reviewed evidence of the clinical implications and burden of malaria in pregnancy. Most studies come from sub-Saharan Africa, where approximately 25 million pregnant women are at risk of Plasmodium falciparum infection every year, and one in four women have evidence of placental infection at the time of delivery. P falciparum infections during pregnancy in Africa rarely result in fever and therefore remain undetected and untreated. Meta-analyses of intervention trials suggest that successful prevention of these infections reduces the risk of severe maternal anaemia by 38%, low birthweight b

Women's groups' perceptions of maternal health issues in rural Malawi.

BACKGROUND: Improvements in preventive and care-seeking behaviours to reduce maternal mortality in rural Africa depend on the knowledge and attitudes of women and communities. Surveys have indicated a poor awareness of maternal health problems by individual women. We report the perceptions of women's groups to such issues in the rural Mchinji district of Malawi. METHODS: Participatory women's groups in the Mchinji district identified maternal health problems (172 groups, 3171 women) and prioritised problems they considered most important (171 groups, 2833 women). In-depth qualitative data was

Post-natal corticosteroid use.

UNLABELLED: Corticosteroids given systemically in the first 2 weeks after birth may reduce chronic lung disease (CLD) and mortality, but there are concerns regarding the long-term adverse effects which may have influenced prescribing habits. The aim of this study was to determine whether corticosteroids were still prescribed and, if so, was there a consensus regarding the most appropriate regimen to use. Consultant paediatricians at all 223 neonatal units in the United Kingdom were sent a questionnaire asking which route, timing, dosage and indications they used when administering corticostero

Perinatal factors and risk of neuroblastoma.

Neuroblastoma is a malignancy commonly diagnosed during infancy or early childhood, raising speculation about the role of perinatal factors and risk of disease. Using a case-control design, cases included 155 infants and children aged 0-5 years with histologically confirmed neuroblastoma diagnosed and reported to the New York State Cancer Registry between 1976 and 1987. Controls were randomly selected from the State's Livebirth Registry and were frequency matched to cases on year of birth (n = 310). Medical records of cases were used to verify histology and stage of disease. Data on perinatal

South African children: a history of adversity, violence and trauma.

This paper examines the adverse situation that continues to face South African children. Children have until very recently, been regarded as one of the most neglected and overtly oppressed sectors of South African society. The installation of a democratic government ushered in a range of policies and measures designed to promote the wellbeing of children. Despite this new commitment and support, the position of children has remained one of considerable concern. In this paper we report on a range of social, educational, health and psychological indicators of the wellbeing of South African child

Prevalence of xerophthalmia and efficacy of vitamin A prophylaxis in preventing xerophthalmia co-existing with malnutrition in rural Indian children.

Data of 4302 children, 0-6 years old were analysed to study the prevalence of vitamin A deficiency and the efficacy of vitamin A prophylaxis in preventing xerophthalmia co-existing with malnutrition. Manifestation of ocular signs of vitamin A deficiency were seen in 10 per cent children. The prevalence of xerophthalmia was higher in the normal and mild to moderately malnourished children, and lowest in the severely malnourished ones. The ability of vitamin A prophylaxis to curb xerophthalmia was greatest at the extreme ends of the nutritional grade spectrum. These findings have important impli

Hospital boarder babies and their families: an exploratory study.

Data were abstracted from the medical and social work charts of 20 newborns who were classified as boarder babies and their mothers (N = 18) to identify bio-psycho-social factors associated with boarding. The findings show that the mothers whose newborns remained in the hospital as boarders were usually drug users, had other children in out-of-home placement, and over half are periodically homeless. Most of these mothers also lacked informal social support. The major health problems of infants were prematurity and associated infections. The total number of infant boarding days was 195 for a to
