Patient feedback on an educational tool to empower patients after hypertensive pregnancy.

INTRODUCTION: Hypertensive disorders of pregnancy (HDPs) are a leading contributor to preventable maternal morbidity and mortality and are associated with increased risk of cardiovascular disease. The postpartum period represents a critical opportunity to improve engagement in follow-up care and support cardiovascular disease prevention after HDP; however, few patient-facing, postpartum educational tools about HDP have been studied. We sought to develop and evaluate a patient educational discussion guide to support postpartum recovery and promote heart-healthy behavior after a hypertensive pre

Assessment of Patients Receiving Short-Interval Botulinum Toxin Chemodenervation Treatment for Laryngeal Dystonia and Essential Tremor of the Vocal Tract.

IMPORTANCE: The gold-standard treatment for laryngeal dystonia (LD) and essential tremor of the vocal tract (ETVT) is botulinum toxin (BoNT) chemodenervation. Although safe and effective, it is not curative, and periodic injections are required. Some medical insurance companies only cover injections at a 3-month interval, though some patients benefit from injections more frequently. OBJECTIVE: To determine the proportion and characteristics of patients who receive BoNT chemodenervation treatment in intervals shorter than 90 days. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort s

Central Precocious Puberty and Psychiatric Disorders.

IMPORTANCE: Within the physiological range, early pubertal timing is associated with an increased risk of mental health issues. Previous studies examining the associations of central precocious puberty (CPP) with mental health have yielded inconclusive results. OBJECTIVE: To describe the risk for development of psychiatric disorders in patients with CPP and to identify periods during which patients with CPP are at heightened risk of developing psychopathological conditions. DESIGN, SETTING, AND PARTICIPANTS: In this population-based, retrospective cohort study, patients with CPP and matched

Cost-effectiveness of heptavalent conjugate pneumococcal vaccine (Prevenar) in Germany: considering a high-risk population and herd immunity effects.

In Germany, the seven-valent conjugate vaccine Prevenar is recommended for use in children at high risk of pneumococcal disease. Recent data suggest that giving conjugate vaccine to all children may lead to a decline in pneumococcal disease in unvaccinated adults, a phenomenon known as herd immunity. This analysis evaluated the cost and economic consequences in Germany of vaccinating (1) children at high risk, (2) all children when considering only benefits for vaccinated individuals and (3) all children when also considering herd immunity benefits. Costs in the model included vaccination, man

[PASS units: mechanisms for delivering healthcare to underserved populations. The example of the Hôtel-Dieu PASS, Paris].

The 1998 French law on combating exclusion established PASS units (Permanences d'Accès aux Soins de Santé, or Healthcare Access Points) to guarantee access to healthcare for all, including people in precarious situations. PASS centers aim to address the social, economic, administrative, and linguistic barriers that prevent vulnerable populations from accessing the healthcare system. This article provides an overview of how PASS operate in France and describes the PASS at Hôtel-Dieu in Paris (Public Hospitals of Paris). This PASS offers patients a holistic approach that includes medical care, ps

The Tsuru Longitudinal Study: Design and Profile of Participants.

BACKGROUND: Physical activity, sedentary behaviour, and sleep (collectively referred to as physical behaviours) are promising preventive factors for adverse health outcomes. However, most previous studies have relied on subjective measures assessed at a single time point, which fail to capture changes in physical behaviours over time and can introduce measurement bias. METHODS: The Tsuru Longitudinal Study is a cohort study of older adults aged ≥65 years living in Tsuru, Yamanashi, Japan. The study includes mail surveys conducted in 2016, 2018, 2019, 2022, 2024, and 2026, accelerometer-based

Optimizing the Transition of Care for Postpartum Preeclampsia: A Scoping Review of Management Strategies and Missed Opportunities.

Hypertensive disorders of pregnancy remain a primary driver of preventable maternal morbidity, specifically during the high-risk transition from hospital discharge to long-term primary care. This scoping review maps the "transition gap" within the fourth trimester to determine why traditional management strategies often fail to protect patients at risk for late-onset postpartum preeclampsia. Following Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines, a systematic search of PubMed, the Cochrane Library, and Google Scholar w

Life expectancy after an ischemic stroke or transient ischemic attack in older adults - the role of frailty.

INTRODUCTION: Ischemic stroke and transient ischemic attack (TIA) reduce life expectancy in older adults. The impact of frailty on life expectancy following these events is unclear. METHODS: This nationwide retrospective cohort study used data from the Dutch health insurance claims database Vektis. We included patients aged ≥70 with ischemic stroke or TIA in 2018, selecting frail individuals using the U-PRIM frailty index. A non-frail control group was selected using frequency matching on age, sex, and socioeconomic status. Mortality data up to May 2024 provided 5.4-6.4 years of follow-up. Ka

Dexmedetomidine and postoperative dementia risk in older patients.

BACKGROUND: Postoperative cognitive decline (POCD) and dementia are significant risks in older patients after surgery. Dexmedetomidine, an α2-adrenergic agonist, has shown neuroprotective properties in preclinical models, but its role in preventing dementia in postoperative patients remains unexplored. METHODS: We conducted a retrospective cohort study using data from Taiwan's National Health Insurance Research Database from 2012 to 2020. The cohort included 59 194 older patients (≥70 years) who underwent hip fracture surgery. Patients were categorised based on postoperative dexmedetomidine u

Poor self-rated health predicts the incidence of functional disability in elderly community dwellers in Japan: a prospective cohort study.

BACKGROUND: Although previous large population studies showed elderly with poor self-rated health (SRH) to be at a high risk of functional disability in Western countries, there have been few studies in which the association between SRH and functional disability was investigated in Japanese community dwellers. The association between SRH and functional disability, defined as certification of the long-term care insurance (LTCI) system, in Japanese elderly community dwellers was examined in this study. METHODS: A total of 10,690 individuals (39.5% men, mean age of 71.4 years) who were 65 years

Green tea consumption and the risk of incident functional disability in elderly Japanese: the Ohsaki Cohort 2006 Study.

BACKGROUND: Previous studies have reported that green tea consumption is associated with a lower risk of diseases that cause functional disability, such as stroke, cognitive impairment, and osteoporosis. Although it is expected that green tea consumption would lower the risk of incident functional disability, this has never been investigated directly. OBJECTIVE: The objective was to determine the association between green tea consumption and incident functional disability in elderly individuals. DESIGN: We conducted a prospective cohort study in 13,988 Japanese individuals aged ≥65 y. Inform
