[Incomplete intestinal fistulas].

Data on the treatment of incomplete intestinal fistulas in 86 patients are presented. In 34 patients the incomplete intestinal fistulas were formed on the duodenum after a resection of the stomach. Tampons were responsible for the formation of small intestine incomplete fistulas in 47 patients. The operative treatment was used in 10 of 86 patients with incomplete intestinal fistulas, better results being obtained in the patients operated upon within the first three days after the appearance of the fistula. In cases with incomplete small intestine fistulas 54 patients died of 76; with colonic f

Volvulus of the small intestine.

A series of seventy-five cases of volvulus of the small intestine are reviewed. There is a 14.2% incidence of this condition among cases of acute intestinal obstruction. It is more common than cecal volvulus but less common than sigmoid volvulus. The patients are usually healthy, poor, adult villagers whose diet is heavy and vegetarian with a high roughage content; they mainly do hard manual work. It is difficult to make a correct preoperative diagnosis. In this series, the percentage of primary volvulus is 73.33% and of secondary is 26.66%. The etiology, incidence, symptomatology and mortalit

Small-intestinal lipomatosis.

Two cases of segmental, small-intestinal lipomatosis are described. In both, the patients had crampy abdominal pain secondary to partial intestinal obstruction. A review of the literature uncovered reports of 16 similar cases, in some of which virtually the entire small intestine was involved. Diverticulosis and intussusception are associated lesions. Diagnosis may be made by an abnormal roentgenographic pattern. Provided the bowel is not so extensively involved that complete resection is impractical, surgical removal of the affected segment results in cure.

Intestinal and urogenital flagellates.

Although the human alimentary and urogenital tracts are parasitized by seven species of flagellate protozoa [115], only two, Giardia lamblia and Trichomonas vaginalis, are generally considered to be pathogens. This review, therefore, concentrates on these organisms and in particular on the experimental and clinical chemotherapy of trichomoniasis and giardiasis. Due to the limited space and the availability of excellent recently published reviews, topics such as life cycles, epidemiology, prevalence and pathology are only briefly discussed in the sections entitled 'Biological Aspects'. [Diagnosis of intestinal amebiasis].

Three cases of intestinal amebiasis with varying clinical pictures are presented. One patient developed an amebic liver abscess and also amebic colitis, which was mistaken for Crohn's disease. A second patient with amebic dysentery had no Entamoeba histolytica trophozoites in fecal material because of previous therapy with an antiprotozoal drug. The third patient, who had colitis and severe constitutional symptoms, had trophozoites in the stool but negative serological tests. The diagnostic value of radiology and endoscopy, of examinations of stool and exsudate, and of histopathology and serol

[Traumatic intestinal infarction].

A study of 31 cases of traumatic intestinal infarction, often old cases, did not show any characteristic clinical picture or course but showed the following points: abdominal contusion, even mild, may induce intestinal infarction; the ileum and ascending colon are most commonly involved, and infarction usually follows a thrombosis of the mesenteric veins; the clinical course usually includes an interval between the trauma and the infarction.

Complicated intestinal atresias.

In this group of 45 intestinal atresia patients (duodenum, 16; jejunum, 24; ileum five) at the University of Mississippi Medical Center, individual hospitalizations ranged up to 245 days. Twelve patients required multiple operations, and the overall mortality rate was 22% (ten patients). While the patients with duodenal atresia had the greatest incidence of other congenital anomalies, including Down's syndrome, the patients with jejunal atresia presented with the most challenging surgical problems. Of the 24 jejunal atresia patients, only three had a single, simple area of obstruction. The rem

Intestinal obstruction in pregnancy.

Intestinal obstruction is an unusual complication of pregnancy, but may be encountered more frequently in the future. Two patients with such a complication are described. Relief of the obstruction was achieved by surgical intervention in one patient and by delivery of a term baby in the other. The management of intestinal obstruction in late pregnancy is discussed.

[Tumors of the small intestine].

On the basis of 20 observations of tumours of the small bowel a series of diagnostic, therapeutic and prognosis of these lesions are discussed. Malignant tumours of the small bowel (10 adenocarcinomas and 2 sarcomas in the present statistics) represent 1,9 per cent of the total number of malignant tumours of the small bowel. The patients were hospitalized following an acute complication occlusion, haemorrhage, perforation). Four of the patients benefited from a curative treatment, in 5 only a derivation was possible, and in 9 only exploratory laparotomy and bioptic sampling. Eight of the tumou
