A case of right diaphragmatic hernia is presented associated with a complex congenital heart disease (double-outlet right ventricle, transposition of the great arteries and left isomerism) diagnosed prenatally. Despite high-frequency oscillatory ventilation plus nitric oxide and uneventful repair of the hernia, the infant died after 6 days of uncontrolled pulmonary hypertension and severe aortic coarctation that developed postnatally.

This experimental and theoretical study illustrates how phosphaalkenes, which are isolobal to alkenes, can utilize a variety of external triggers for molecular switching. The E/Z isomerization of a truxene-based phosphaalkene, i. e. TruxC=P-Mes* (Mes*=2,4,6-tris-t-butyl-benzene), is accomplished by irradiation, metal coordination, or deprotonation of the truxene core. The reversible and quantitative Z/E double bond isomerization by gold(I) coordination/decoordination of the phosphorous lone pair represents a novel fuelling strategy for molecular switches.

OBJECTIVES: Heterotaxy syndrome is a recognized risk factor for surgical cardiac interventions. We evaluated the early- and middle-term results of a surgical intervention for patients with heterotaxy syndrome.
METHODS: A total of 42 patients with heterotaxy syndrome were enrolled (September 2008 to March 2015). Total anomalous systemic venous return is a very rare anomaly, where vena cava inferior, vena cava superior, and coronary sinus drain into left atrium. Two-day-old male baby was admitted with cyanosis and tachypnea after the birth. Left atrial isomerism with anomalous systemic venous drainage was found on echocardiographic examination. A 10-week-old infant with left atrial isomerism and a primum atrioventricular septal defect presented with life-threatening episodes of collapse. They proved due to paroxysmal pulmonary hypertension, although her pulmonary artery pressure was virtually normal between attacks. The pulmonary hypertensive crises persisted early but resolved late after surgical correction. Labile pulmonary vascular resistance may prove a problem preoperatively if the ventricular septum is intact.

A photoinduced solid-state SO₂ isomerism drives a larger mechanical change (benzene-ring rotation) in a neighbouring ion (i.e., the system acts as a solar-powered molecular transducer). The ring rotation and SO₂ photoisomerisation are observed using in situ X-ray crystallography and are controllable, reproducible, and metastable at low temperatures.
