Patellofemoral Arthritis After Lateral Patellar Dislocation: A Matched Population-Based Analysis.

BACKGROUND: The rate of patellofemoral arthritis after lateral patellar dislocation is unknown. Purpose/Hypothesis: The purpose of this study was to compare the risk of patellofemoral arthritis and knee arthroplasty between patients who experienced a lateral patellar dislocation and matched individuals without a patellar dislocation. Additionally, factors predictive of arthritis after patellar dislocation were examined. The hypothesis was that the rate of arthritis is likely higher among patients who experience a patellar dislocation compared with those who do not. STUDY DESIGN: Cohort study;

MPFL reconstruction corrects patella alta: a cohort study.

PURPOSE: Patella alta is an established risk factor for recurrent lateral patella dislocations. Medial patellofemoral ligament (MPFL) reconstruction has been shown to reduce patella height. Our hypothesis is that isolated MPFL reconstruction corrects patella alta, thereby reducing the risk of recurrent patella dislocation. METHODS: A prospective cohort study of 33 knees which underwent MPFL reconstruction for recurrent patella dislocation. The pre-operative and post-operative patella height and Kujala knee outcome scores, as well as tibial tuberosity-trochlear groove (TT-TG) distance, and the

Rearfoot and Ankle Dislocations.

Subtalar dislocations, ankle dislocations, and total talar dislocations are high-energy injuries. As such, there may be associated osseous or soft tissue injuries that can be diagnosed with advanced imaging such as computed tomography (CT) or MRI. With closed injuries, closed reduction may require sedation or general anesthesia, flexion of the knee to release the tension of the gastrocnemius-soleus complex, distraction is applied, the deformity is accentuated, then the deformity is corrected. Open injuries are usually associated with a higher level of energy and a higher rate of infection. It

Proximal Tibiofibular Joint Instability: An Underrecognized Cause of Lateral Knee Pain and Mechanical Symptoms.

Instability of the proximal tibiofibular joint is an uncommon and often unrecognized cause of lateral knee pain, mechanical symptoms, and instability. The condition results from one of three etiologies: acute traumatic dislocation, chronic or recurrent dislocations, and atraumatic subluxations. Generalized ligamentous laxity is a key predisposing factor for atraumatic subluxation. Instability of this joint may be in the anterolateral, posteromedial, or superior directions. Anterolateral instability is the most common type (80% to 85%) frequently occurring with hyperflexion of the knee with the

Arterial injury complicating knee disruption. Third place winner: Conrad Jobst award.

Because dislocation of the knee (DK) is accompanied by a substantial risk of popliteal artery injury, the importance of arteriography in ruling out occult arterial damage in such patients is well accepted. However, because antecedent DK cannot be ruled out in a trauma victim presenting only with severe knee ligamentous disruption (LD), we have routinely performed arteriography in all patients presenting with grossly unstable knees, whether or not DK is present. To evaluate this policy we reviewed the records of 30 patients with either DK (n = 19) or severe LD (n = 11). There was no significant

Irreducible fracture dislocation of the ankle due to posterior dislocation of the fibula.

In order to prevent permanent disability, a proper diagnosis of the fixed posterior fracture dislocation of the fibula with fracture dislocation of the ankle joint must be made. Full-length (knee to ankle) radiographs are necessary to make this diagnosis. Although a closed reduction has been described as successful, we feel that an open reduction is usually necessary. Freeing of the trapped fibula from behind the tibia results in a satisfactory reduction of this fracture dislocation. The reduction and fixation of the malleolar fragment is then simple, and an excellent result can be expected.

What is the appropriate revascularisation approach in traumatic popliteal artery injury?

BACKGROUND: Popliteal artery injury (PAI) is sometimes accompanied by proximal tibial fracture or knee dislocation. We hypothesized that revascularisation approach should be selected depending on the associated injury. The purpose of this study is to propose revascularisation approach that does not interfere with definitive surgery. METHODS: Patients with PAI who were treated between 2013 and 2023 were included. Associated injuries, revascularisation approach, and skin incision for the definitive surgery were investigated. We investigated whether varus-valgus instability remained as an outcom…

Intra-articular peroneal nerve incarceration following multi-ligament knee injury.

Knee dislocation with a common peroneal nerve injury is a serious problem. A case of multi-ligamentous knee injury with the unusual and interesting finding of a common peroneal nerve rupture incarcerated within the knee joint is presented. MRI and arthroscopic images are used to document this occurrence. To date, there are no published reports of a similar finding in the English orthopaedic literature. Level of evidence IV.

The Insall-Burstein posterior stabilized total knee replacement in the valgus knee.

Sixty-seven Insall-Burstein (Zimmer Inc, Warsaw, Indiana) posteriorly stabilized total knee replacements were implanted in knees with a preoperative alignment > 10 degrees of valgus (average: 19.5 degrees). An anteromedial approach with release of the lateral ligaments of the femur was used. A lateral patellar release was necessary in 49% of cases. Fifty-one knees (76%) were reviewed with an average follow-up of 6 years. Pain at follow-up was absent or mild in all of the knees except one with aseptic loosening of the tibial and patellar components. The average flexion was 105 degrees. The resu

Valgus instability as a cause for recurrent lateral patellar dislocation: a new mechanism for patellofemoral instability?

An association between lateral patellar dislocation (LPD) and medial collateral ligament injury (MCL) has recently been demonstrated on MRI. The same valgus injury that disrupts the femoral MCL insertion can also cause a simultaneous MPFL tear at its femoral insertion, due to the close anatomical relationship of both ligamentous structures. Valgus laxity due to MCL-deficiency increases the Q-angle and further adversely affects patellar stability. A knee diagnosed with a severe MCL tear, should be evaluated for patellofemoral instability, as a temporary patella dislocation could have been cause

Predictors of Recurrent Patellar Instability in Children and Adolescents After First-time Dislocation.

BACKGROUND: Patellar dislocations are one of the most common knee injuries in children and adolescents and are challenging to treat. Recurrence rates are relatively high and many patients have functional limitations, even in the absence of a recurrent instability episode. The purpose of this study was to examine the risk factors in patients with first-time patellofemoral dislocations to develop a prediction model of recurrence. METHODS: A single institution retrospective review of all patients with a first-time patellofemoral dislocation from 2002 to 2013 was performed. Demographic risk facto

Mobile bearing medial unicompartmental knee arthroplasty in patients whose lifestyles involve high degrees of knee flexion: A 10-14year follow-up study.

BACKGROUND: Because Asian populations have different lifestyles, such as squatting and sitting on the floor, from those of Western populations, it is possible that the clinical results and survival rate of unicompartmental knee arthroplasty (UKA) for Asian patients may be different. This study described outcomes of mobile bearing medial UKA for Korean patients. METHODS: A total of the 164 knees treated with mobile bearing UKAs in 147 patients (14 males and 133 females) were reviewed. The mean follow-up period was 12.1years (range 10.1-14). RESULTS: The clinical outcomes, such as the Hospital

Magnetic resonance imaging of acute patellar dislocation in children: patterns of injury and risk factors for recurrence.

BACKGROUND: Acute patellar dislocation is a common traumatic condition of the knee seen in the active adolescent. The patterns of injury to the ligamentous and chondral surfaces following dislocation have not been well defined in the pediatric population. The purpose of this study was to characterize the patterns of medial patellofemoral ligament (MPFL), vastus medialis obliqus (VMO), and osteochondral injury on magnetic resonance imaging (MRI) following first-time acute lateral patellar dislocation in pediatric patients. METHODS: Following approval by the Institutional Review Board, a radiol

Patellar Instability after Total Knee Arthroplasty: A Surgical Case Report.

INTRODUCTION: Patella instability and maltracking is a serious complication following total knee replacement that can lead to poor outcomes for the patient and contribute to early failure. The incidence of patella maltracking ranges from 1% to 20% after total knee arthroplasty (TKA), with post-operative anterior knee pain being an important indicator. There are many different surgical approaches for the management of post-operative patella maltracking with very good outcomes. However, the most crucial step is to identify the underlying etiology. CASE REPORT: A 71-year-old Caucasian woman pres

[Congenital dislocation of the hip: an osteo-neural growth disturbance (author's transl)].

Congenital dislocation of the hip may be readily explained as the result of the osteo-neural growth-in-length disproportion. The theoretical concept is supported by experimental findings in frog tadpoles with "congenital" dislocation of the knee joint produced by osteolathyrogenic diet. In cleared specimens with selective staining of the nervous trunks according to Williams (1943) a growth inhibition of the sciatic nerve responsible for the proximal shift (dislocation) of the tibia may be noted. The tibia is adapted to the growth peculiarities of the sciatic nerve in an essentially similar way
