The primary aim of treatment of cholesteatoma is to attain a dry, safe, stable ear, free of disease. Maintaining or improving hearing is important but the pursuit of a hearing result should not compromise this primary aim. This study reviews the long-term outcome of 133 patients, suffering from advanced disease, who underwent modified radical mastoidectomy between 1995 and 2000. Of these, 49 per cent had had previous mastoid surgery elsewhere. A dry, waterproof ear was attained in 95 per cent of patients. In 77 per cent of patients, hearing was unchanged or improved. In this period, there were

Sixty-six patients with this disease have been treated to date with cyclophosphamide and prednisolone resulting in significant improvement in hearing. In some patients the vestibular compartment may be involved as well, but the clinical pattern is significantly different enough from Meniere's disease to distinguish it by the careful historian. The cornerstone of treatment is cyclophosphamide, abetted by prednisolone. A test of treatment is to date the best method of determining who to treat for cure.

A multitude of general disorders of the vascular system may also affect the blood circulation of the cochlea and cause symptoms such as fluctuating or permanent hearing loss. Such is the case for arteriosclerosis combined with hypertension or hypotension, collagenosis, and diabetes. Blood disorders, like leukemia, sickle cell anemia, and polycythemia, and infectious diseases involving the blood vessels, such as lues, may also present their primary symptoms in the ear. The otorhinolaryngologist must be able to establish the correct diagnosis and refer patients requiring more general treatment t

The localization, frequency, and severity of endolymphatic hydrops in 22 temporal bones of 16 individuals with Meniere's disease were studied histopathologically. Endolymphatic hydrops was more often observed in the pars inferior (22/22) than in the pars superior (13/22) of the temporal bone, and severe hydrops was observed most frequently in the saccule, followed by the cochlea, the utricle, and the three semicircular canals. In the cochlea, the most severe hydrops was observed in the apical turn, followed by the hook portion, the middle turn, and the basal turn. Clinically interesting observ

Any evaluation of vestibular diseases following head and neck injuries must take into consideration the existence of other diseases which may act with the trauma causing lesions which might not have taken place otherwise or which would have had less import. In the present work the authors examine what medico-legal doctrine considers in regard to pre-existing, simultaneous and cropped up aggravations of lesions or of invalidity. Finally, an explanation is given of personal experience with 1500 patients with head and neck injury. It is pointed out how in 130 cases the presence of pathological co

The method for isolating and treating the vestibular and cochlear compartments of the inner ear was developed. Also, the procedure of histological treatment of temporal bones was modified. The pathogenesis of vestibulo-cochlear disorders due to acceleration and noise was investigated using histology, electron microscopy and histochemistry methods. It was found that exposure to acceleration of 10 Gx for 5 min and single and repeated (for 7 days) exposures to high-frequency noise of 100 dB produced histological changes and functional-morphological variations in all receptors located in the vesti

Ossification of the membranous labyrinth (labyrinthitis ossificans) develops as the final result of many inflammatory processes, for example, meningitis, blood-borne septic emboli, middle ear infection, and cholesteatoma. Labyrinthine ossification may also occur as a result of previous labyrinthectomy or secondary to trauma. Seven cases of labyrinthine ossification accompanied by severe vertigo and total hearing loss in the affected ear are discussed. The computed tomographic appearance of varying degrees of ossification, the clinical and surgical circumstances from which this disorder may dev

Looking for a suitable animal model of experimental labyrinthitis we performed in vivo infections of the domestic cat with feline rhinotracheitis virus. After instillation of the virus suspension into the nose of the anaesthetized cat we could find the complete virus particles in so-called virus factories in the nasal mucosa four days later by means of electron-microscopy. The adjacent ganglia of the olfactory nerve were stained by immunohistochemical methods and showed positive viral antigen. After embedding of the cochlea for electron microscopy virus particles could be demonstrated in the i

BACKGROUND: Intracranial schwannomas are most commonly associated with the vestibulocochlear nerve, often leading to hearing loss, tinnitus, and vestibular dysfunction. Much less often, a schwannoma can arise from the trigeminal nerve which can lead to facial pain, numbness, and weakness.
PURPOSE: We explored a case of a patient with an magnetic resonance imaging (MRI)-confirmed trigeminal schwannoma that was mistaken for a vestibulocochlear schwannoma because of a myriad of ipsilateral vestibulocochlear symptoms.
RESEARCH DESIGN: This is a retrospective chart review and case study, with no

Recurrent vestibulopathy is defined as an illness of unknown cause characterized by more than a single episode of vertigo of duration characteristic of that occurring with hydrops but without auditory or clinical neurological symptoms or signs. Eighty-six patients with this condition were diagnosed in the Dizziness Unit, and data on age and sex distribution, natural history and caloric pattern are presented. On follow-up of mean duration 3.5 year, 6 cases evolved to classic Ménière's disease, and 4 to benign positional vertigo, but none developed brain disease. The prognosis regarding vertigo

PURPOSE OF REVIEW: Ménière's disease is characterized by spontaneous attacks of vertigo, fluctuating sensorineural hearing loss, aural fullness, and tinnitus. The pathologic process involves distortion of the membranous labyrinth with the formation of endolymphatic hydrops. This review describes the pathogenesis and etiology as well as the diagnosis and treatment of Ménière's disease.
RECENT FINDINGS: Initial management of Ménière's disease can involve a low-salt diet and a diuretic. Treatment with intratympanic injection of gentamicin can be beneficial when vertigo persists despite optimal m

In 100 patients showing different lesions of the peripheral vestibular system and of the CNS, the influence of spontaneous vestibular nystagmus on optokinetic nystagmus was investigated. Considering the large interindividual differences in the results of vestibulo-visual interaction, the actual values of OKN slow-phase velocity obtained by visual stimulation are only of limited diagnostic value in a single case. However, by averaging the results obtained from various nosologic groups of patients very different but distinct patterns of vestibulo-visual interaction can be demonstrated: The diagn

PURPOSE OF REVIEW: To highlight the recent advances in the understanding of the diagnosis and management of viral inner ear disorders. Congenital sensorineural hearing loss (cSNHL), sudden sensorineural hearing loss (SSNHL), Ménière's disease, and vestibular neuritis/viral labyrinthitis are discussed.
RECENT FINDINGS: Cytomegalovirus infection during pregnancy is an under-recognized cause of hearing loss and central nervous system disease amongst the general population. Prevention of maternal infection and treatment of affected newborns with ganciclovir are promising interventions. Recent evi

HYPOTHESIS: Ossification of the cochlea was once considered to be a contraindication for cochlear implantation. Advances in cochlear implant technology and coding strategies have led to developments in different surgical procedures to manage cochlear ossification. The endoscopic technique allows a direct approach to the round window and the cochlea, especially in remodeled labyrinth, allowing a better vision of scala tympani.
