[Randomized clinical study of polydioxanone and nylon sutures for laparotomy clousure in high-risk patients].

INTRODUCTION: The complications of surgical wound closure in patients with risk factors significantly increases morbidity and mortality. The aim of the present study was to evaluate differences in abdominal wall closure in patients with risk factors with the same closure technique and slow-absorbable or non-absorbable sutures. MATERIAL AND METHODS: We performed a prospective, multicenter, comparative study of polydioxanone versus nylon sutures. Laparotomies performed for intestinal diseases and hepatobiliopancreatic procedures in patients with at least one risk factor were included. Exclusion

Heterotopic pregnancy with spontaneous vaginal delivery at 36 weeks and laparotomy at term--a case report.

Heterotopic pregnancy, although rare, is occurring more frequently because of an increase in genital infection and the escalating use of new reproductive technologies in infertility patients. The case of a 30-year-old para 2 + 1 prophetess is presented. She had a spontaneous vaginal delivery at term. Persistent abdominal pain and distension led to suspicion of heterotopic pregnancy. This was confirmed by ultrasonography. Laparotomy revealed a macerated fetus in the peritoneal cavity The purpose of this report is to sensitise practitioners about the reality and existence of the condition.

A comparison of serum interleukin-6 concentrations in patients treated by cholecystectomy via laparotomy or laparoscopy.

BACKGROUND/AIMS: Interleukin-6 (IL-6), a multifunctional cytokine, is expressed by various cells after many stimuli. This cytokine release i … tive protein. Laparoscopic surgery, associated with a small skin incision and the avoidance of open laparotomy, can thus minimize surgical stress, and provide more favorable postoperative conditions for patients. Indeed excessive and prolonged post-injury elevations are associated with increased morbidity.

Incidence of acquired immune deficiency syndrome (AIDS)-related disorders at laparotomy in patients with AIDS.

BACKGROUND: Patients with acquired immune deficiency syndrome (AIDS) present for surgical management of abdominal conditions that are complications of advanced human immunodeficiency virus (HIV) infection or that are caused by other disease not related to AIDS. This study compared the clinical details and postoperative outcomes of patients with AIDS-related diseases found at laparotomy with those of patients with non-AIDS-related disorders. METHODS: The medical records of 30 consecutive patients with AIDS who underwent laparotomy were examined retrospectively. RESULTS: Fourteen patients had …

[A case of local reactivation of tuberculous peritonitis with suspected gastric submucosal tumors after laparotomy].

After complaining of right hypochondrial pain, a 36-year old man was diagnosed as having a gastric submucosal tumor at a nearby hospital and admitted to Chiba Cancer Center Hospital. On admission, a hard and immobile mass was palpable on the right upper quadrant. Laboratory findings were within normal limits. A chest X-ray revealed bilateral pleural adhesions and streaking shadows in the bilateral lung area. These findings were assumed to be due to a prior tuberculous infection. Upper gastrointestinal series and endoscopic examination showed a protruding lesion on the stomach body. An abdomina

Fascial Dehiscence Despite Declared Adoption of European Hernia society guidelines: A retrospective study following laparotomy.

PURPOSE: Abdominal fascial dehiscence is a serious postoperative complication following laparotomy, associated with high morbidity and mortality. Although the small-bite technique is recommended by the European Hernia Society, its implementation in clinical practice, particularly in emergency surgery, remains uncertain. This study described documented fascial closure practices among patients with fascial dehiscence following laparotomy and characterised patient-related and technical features within this cohort. METHODS: This retrospective study included all patients who developed fascial dehi…

Prevalence, Predictors and Decompressive Laparotomy in Abdominal Compartment Syndrome in Patients Requiring Extracorporeal Membrane Oxygenation.

Background: Critically ill patients requiring extracorporeal membrane oxygenation (ECMO) have several risk factors to suffer from abdominal compartment syndrome (ACS). Little is known about this subgroup. The aim of this study was to investigate the prevalence and associated factors for ACS in patients requiring ECMO to assess the effect of decompressive laparotomy (DL) and the impact on mortality. Methods: This retrospective observational study analyzed adult patients requiring ECMO in four intensive care units at the University Medical Center Regensburg between 01/2010 and 06/2020. Patients …

Incidental appendectomy during laparotomy for idiopathic intussusception in children: a dual-center retrospective cohort study.

BACKGROUND: The role of incidental appendectomy (IA) during surgery for idiopathic intussusception in children remains controversial. This study evaluated the potential benefits and risks of performing IA during laparotomy. METHODS: We performed a dual-center retrospective cohort study of children who underwent open reduction for idiopathic intussusception (2014-2021). Patients were grouped by whether IA was performed. Primary outcomes were perioperative complications (PCs) and recurrent intussusception (RI); secondary outcomes were operative time, time to postoperative oral intake (PO time),…

The clinical consequences of burst abdomen after emergency midline laparotomy: a prospective, observational cohort study.

PURPOSE: The emergency midline laparotomy is a commonly performed procedure with a burst abdomen being a critical surgical complication requiring further emergency surgery. This study aimed to investigate the clinical outcomes of patients with burst abdomen after emergency midline laparotomy. METHODS: A single-center, prospective, observational cohort study of patients undergoing emergency midline laparotomy during a two-year period was done. Abdominal wall closure followed a standardized technique using monofilament, slowly absorbable suture in a continuous suturing technique with a suture-t…

Outcomes following combined median sternotomy and ventral midline laparotomy for bicavitary penetrating wooden foreign bodies.

Combined abdominal and thoracic pathology caused by extra-gastrointestinal migration of an ingested wooden foreign body (WFB) is an uncommon but serious injury. Presenting clinical signs are typically nonspecific and, in the absence of observed WFB ingestion, diagnosis is challenging. Treatment requires concurrent abdominal and thoracic surgical exploration to remove the WFB and address injuries caused by its migration. This case series describes perioperative characteristics and outcomes in 4 dogs following combined median sternotomy and ventral midline laparotomy (CMSVML) for bicavitary pene…

Mini-laparotomy for cholecystectomy in resourced limited settings: a 10-year retrospective hospital-based study.

Cholecystectomy is a commonly performed abdominal procedure, the gold standard currently is the laparoscopic approach and, however, the facilities and expertise for laparoscopy are not available widely, especially in developing countries. A Mini-laparotomy cholecystectomy is an additional approach that is performed through an incision that is less than 5 cm thus minimizing the complications of the traditional open cholecystectomy and the postoperative hospital stay. The study aims to evaluate the outcome of mini-laparotomy cholecystectomy in terms of operative duration, complications, and hosp…

Implementation Process and Evolution of a Laparotomy-Assisted 2-Port Fetoscopic Spina Bifida Closure Program.

INTRODUCTION: Prenatal closure of open spina bifida via open fetal surgery improves neurologic outcomes for infants in selected pregnancies. Fetoscopic techniques that are minimally invasive to the uterus aim to provide equivalent fetal benefits while minimizing maternal morbidities, but the optimal technique is undetermined. We describe the development, evolution, and feasibility of the laparotomy-assisted 2-port fetoscopic technique for prenatal closure of fetal spina bifida in a newly established program. METHODS: We conducted a retrospective cohort study of women consented for laparotomy-…
