Laser use is expanding rapidly in laboratories, where multiple use strategies of research are not often amenable to laser engineering safeguards employed in industry. We report on five cases of laser-associated incidents requiring medical follow-up from a single research institution, and present exposure circumstances from four other incidents. The use of lasers in otology is rather recent and their use in otoneurosurgery is even more so, as shown by the limited number of publications dealing with this subject in the literature. The effect of low-power laser therapy on acute ankle sprains was evaluated in a double-blind randomised clinical study consisting of 40 patients from the casualty ward. All patients received treatment until their ankle joint was painless. No statistically significant differences regarding discolouring, pain, oedema, and use of analgetics were observed between patients treated with laser and placebo. The patients treated with active laser had a significantly longer sick leave.

Transurethral laser resection of the prostate was performed on 25 patients for the management of bladder outlet obstruction secondary to prostatic hyperplasia. All of the patients in our series were treated on an outpatient basis and the majority of these under IV sedation. Patient selection was limited to older, high-risk patients who were poor surgical candidates for transurethral resection of the prostate or suprapubic prostatectomy because of underlying medical problems. Dry eye is a common problem and in its more severe form may be treated with punctal occlusion. Photothermal coagulation of the puncta with the argon laser has recently been advocated to occlude the lacrimal puncta. Its long-term efficacy has not been investigated. A case of acute cholecystitis in a high risk patient was treated in the acute phase with cholecystostomy guided by ultrasound and with introduction of a pig-tail catheter for drainage. Later, percutaneous laser lithotripsy was undertaken via a flexible choledochoscope. The recent percutaneous therapeutic principles and the possibilities in biliary calculous disease are reviewed.

In 10 rabbits, ablation of the uterine horn endometrium and the fallopian tube was performed with the Nd:YAG laser at a power of 60 W using the touch technique. Eight weeks postsurgery, biopsies of the tissue site were obtained showing that the endometrium and tubal mucosa were completely destroyed. Thirty women were sterilized using a similar technique hysteroscopically. Tubal occlusion was demonstrated by hysterosalpingography in all cases at 3 months postoperatively.

The outcomes of using low-energy helium-neon laser radiation in a complex management of 21 patients with complicated forms of nephrotuberculosis are illustrated. LG-75 laser (with 0.63 nm wavelength and 25 mW power) was used in this case. The effects appeared as a larger bladder capacity, cystoscopic picture normalization and an overall cessation of dysuria were observed as a result of 5 to 12 sessions of bladder mucosa radiation.
