Family Caregiver Support of Patient Self-Management During Chronic, Life-Limiting Illness: A Qualitative Metasynthesis.

Family caregivers play an integral role in supporting patient self-management, yet how they perform this role is unclear. We conducted a qualitative metasynthesis of family caregivers' processes to support patient self-management of chronic, life-limiting illness and factors affecting their support. Methods included a systematic literature search, quality appraisal of articles, data abstraction, and data synthesis to produce novel themes. Thirty articles met inclusion criteria, representing 935 international family caregivers aged 18 to 89 years caring for patients with various health conditio

Microbial synthesis and degradation of bio-based biodegradable plastics.

Microorganisms facilitate both the synthesis and breakdown of bio-based biodegradable plastics, which positions them as central players in the development of a future circular plastics economy. However, advances in microbial production and biodegradation of plastics are often considered in isolation, limiting our ability to integrate these processes into viable circular systems. In this Review, we discuss the contributions of microorganisms across the life cycle of key bio-based polymers, focusing on polyhydroxyalkanoates, bacterial nanocellulose, and the monomers of polylactic acid and polyet

[Palliative care: social and territorial inequalities].

SOCIAL AND TERRITORIAL. INEQUALITIES.Palliative care can be provided wherever the patient lives or receives care, considering its environment, in coordination with healthcare professionals, social and medico-social professionals. The way in which palliative care is provided varies from one country to another, depending on resources, cultural values, healthcare systems and policies of each country. There are disparities in access, particularly in rural areas and for vulnerable populations. One of the main limitations to the development of palliative care, after budgetary issues, is the insuffic

Review on Alopecia Areata in the Middle East and Africa: Landscape and Unmet Needs.

BACKGROUND: Alopecia areata (AA) is an autoimmune disease characterized by non-scarring hair loss in adults and children. Clinical manifestations range from hair loss in small, well-circumscribed patches to total hair loss on the scalp or any other hair-bearing areas. Although the exact pathogenesis of AA is not fully understood, it is thought that loss of immune privilege caused by immunological dysregulation of the hair follicle is key. Genetic susceptibility also plays a role. Response to currently available treatments is widely variable, causing patient dissatisfaction and creating an unme

Maternal weight and gestational diabetes impacts on child health.

PURPOSE OF REVIEW: To review recent evidence linking maternal body mass index and gestational diabetes mellitus (GDM) with offspring health outcomes. RECENT FINDINGS: It is now established that the rising prevalences of maternal obesity and GDM are both making substantial contributions to the growing burden of childhood obesity and associated disorders. Strengthening evidence also links maternal obesity with increased offspring risks of cardiovascular disease, nonalcoholic fatty liver disease, lower respiratory tract infections during infancy, wheezing illnesses, asthma and attention deficit

Asking women about mental health and social adversity in pregnancy: results of an Australian population-based survey.

BACKGROUND: Social adversity undermines health in pregnancy. The objective of this study was to examine the extent to which pregnant women were asked about their mental health and life circumstances in pregnancy checkups. METHOD: Population-based postal survey of recent mothers in two Australian states. FINDINGS: Around half of the 4,366 participants reported being asked about depression (45.9%) and whether they were anxious or worried about things happening in their life (49.6%); fewer reported being asked about relationship issues (29.6%), financial problems (16.6%), or family violence (14

Quantifying burden of disease to support public health policy in Belgium: opportunities and constraints.

BackgroundTo support public health policy, information on the burden of disease is essential. In recent years, the Disability-Adjusted Life Year (DALY) has emerged as the most important summary measure of public health. DALYs quantify the number of healthy life years lost due to morbidity and mortality, and thereby facilitate the comparison of the relative impact of diseases and risk factors and the monitoring of public health over time.DiscussionEvidence on the disease burden in Belgium, expressed as DALYs, is available from international and national efforts. Non-communicable diseases and in

Caregiving burden in palliative care: exploratory mixed-method study.

OBJECTIVES: Informal family caregivers are often the primary providers of care for patients with life-limiting illnesses. Although supporting these caregivers is a core component of palliative care, there is a notable lack of context-specific research on caregiver burden from Southeast Asia. This study aimed to examine the experience of caregiving in a tertiary hospital in Malaysia with consultative palliative care services, exploring both the burden experienced and the factors that mitigate or exacerbate it. METHODS: A qualitative-dominant mixed-method study using an explanatory sequential d

Socioeconomic Factors Associated With Diet Quality in Pregnancy: A Cross-Sectional Australian Study.

Prenatal diet affects maternal and child health; however, adherence to dietary guidelines in pregnancy is low. This cross-sectional study aimed to describe overall diet quality and to examine relationships between socioeconomic factors and diet quality in a sample of Australian pregnant women. Participants (n = 1580) completed an online survey and self-reported usual dietary intake (via a food frequency questionnaire [FFQ]) and socioeconomic factors, including highest educational attainment, income, perception of overall financial situation, residential postcode for area-level socioeconomic st

A hybrid decision support system using rule-based and AI methods: the OnCATs knowledge-based framework.

BACKGROUND AND SIGNIFICANCE: Clinical decision support systems (CDSS) can improve evidence-based oncology care, but many rely on opaque AI models that limit transparency and reproducibility. Rule-based approaches provide interpretability but often lack adaptability, a critical issue in prostate cancer where decisions depend on tumor stage, PSA, Gleason score, comorbidities, and life expectancy. Bridging explainability and adaptability is essential for trustworthy decision support. OBJECTIVE: To develop and evaluate OnCATs, a modular, explainable, hybrid-ready CDSS that encodes prostate cancer

Designing the Healthcare System for the Fulfilment of Patient Goals: A Patient-Centric Exploration in the Home Dialysis Context.

This study investigates patients' goal-oriented experiences in the context of home dialysis for chronic kidney disease (CKD). The aim is to understand how the home care context can be designed to support chronic patients in achieving their goals. Drawing on goal-oriented experience theory, we explore how patients' motivations, desired outcomes, and lived experiences shape their perceptions of home health care. To do so, we employed a co-design-based qualitative methodology, including postal kits, photo diaries, ethnographic observations, in-depth interviews, and participatory workshops with 22

People, places, systems and society:A qualitative exploration of socio-cultural factors in head and neck cancer survivorship.

PURPOSE: People living with head and neck cancer (HNC) often endure permanent and life changing adverse effects of treatment and reduced health-related quality of life. Study of post-traumatic growth (PTG), is gaining traction to understand why some people living with cancer have better psychosocial outcomes than others. Current theoretical models on PTG point to the importance of 'socio-cultural influences' but do not provide insight into how such socio-cultural factors influence survivorship outcomes. This research aimed to illuminate pathways to PTG following HNC by exploring socio-cultural

Critical care for patients with massive ischemic stroke.

Malignant cerebral edema following ischemic stroke is life threatening, as it can cause inadequate blood flow and perfusion leading to irreversible tissue hypoxia and metabolic crisis. Increased intracranial pressure and brain shift can cause herniation syndrome and finally brain death. Multiple randomized clinical trials have shown that preemptive decompressive hemicraniectomy effectively reduces mortality and morbidity in patients with malignant middle cerebral artery infarction. Another life-saving decompressive surgery is suboccipital craniectomy for patients with brainstem compression by

Using Collaborative Model Building to Better Understand the Mechanisms of Alcohol-Involved Sexual Violence on College Campuses: Post Hoc Protocol.

BACKGROUND: The Collaborative Model Building Project to Understand Sexual Violence (CAMPUS) study seeks to address alcohol-involved sexual violence (AISV) by collaboratively developing an agent-based model (ABM) that can support the decisions of college campuses seeking to address this issue among students. As a first step toward ABM development, we used collaborative model building (CMB), an adaptation of group model building, to co-develop a causal loop diagram (CLD) depicting key causes and effects of AISV and opportunities for intervention. Our goal of cocreating a CLD that can be translat

From research to practice: a qualitative study examining the integration of telehealth-based suicide prevention in HIV care in Tanzania.

Despite the life-saving potential of mental health interventions, their scalability and integration within healthcare systems present significant challenges, particularly in low-resource global settings. This study investigated scaling IDEAS for Hope, a telehealth counseling intervention designed to reduce suicide risk and improve HIV care engagement among people living with HIV in Kilimanjaro, Tanzania, and identified actionable implementation strategies for successful scale-up. We conducted 20 semi-structured interviews, between May and July of 2024, with purposively sampled stakeholders fro
