Plasma exchange has been widely used in autoimmune neurological diseases and is the standard treatment for myasthenia gravis crisis and Guillain-Barre syndrome. A growing body of research suggests that, in the clinical application of steroid-responsive encephalopathy, such as for Hashimoto's encephalopathy, limbic encephalitis, systemic lupus erythematosus encephalopathy, ANCA-associated vasculitis encephalopathy, and acute disseminated encephalomyelitis, plasma exchange is a safe, and effective option when steroids or other immunosuppressive therapies are ineffective in the short term or when…

Anti-Ma2/Ta antibodies are rare paraneoplastic antibodies, which are mostly associated with limbic encephalitis in male patients with testicular cancer. We report on a 50-year-old woman with a pure progressive spastic paraparesis. Next, she was diagnosed as having a Sjögren syndrome, with serological positivity for anti-SS-Ro antibodies. The patient's serum and cerebrospinal fluid samples were positive for anti-Ma2/Ta antibodies, which were also proved to be intrathecally produced. These findings, and the coexistence of systemic autoimmunity, led us to treat the patient with corticosteroids fi…

BACKGROUND: Neurological autoimmune disorders (NAD) are caused by autoimmune inflammation triggered by specific antibody subtypes. NAD may disturb the gut-brain axis at several levels including brain, spinal cord, peripheral, or enteric nervous system.
CASE REPORT: We present a case with antinuclear neuronal Hu (ANNA-1)- and antiglial nuclear (SOX-1) autoimmune antibody-positive limbic encephalitis and significant gastrointestinal dysmotility consisting of achalasia type II, gastroparesis, altered small intestinal interdigestive motility, and severe slow transit constipation. The autoantibodi…

We reported a female patient with severe memory loss and faciobrachial dystonic seizures. Anti-LGI1 antibodies were detected in both serum and cerebrospinal fluid. Brain MRI only showed subtle abnormality. She received immunotherapy but unfortunately underwent two relapses after the first attack, resulting in gradual memory decline. Intriguingly, we found higher distribution of 18F-DPA714 in her left medial temporal lobe by PET/CT scan. A novel non-invasive molecular imaging technology indicated potential microglial activation, which might be a promising biomarker for diagnosing and monitoring

Anti-leucine-rich glioma inactivated-1 (anti-LGI1) encephalitis is a subgroup of autoimmune encephalitis. We herein report the case of a 60-year-old man who presented with typical symptoms, including short-term memory loss, mental abnormalities, hyponatremia and seizures characterized by faciobrachial dystonic seizures and who was diagnosed with anti-LGI1 encephalitis. At the same time, he was diagnosed with essential thrombocythemia. A significant improvement was obtained by treatment with corticosteroid, immunoglobulin, mycophenolate mofetil, and hydroxyurea. Autoimmune diseases are associat

BACKGROUND AND OBJECTIVES: Antileucine-rich glioma-inactivated 1 (anti-LGI1) autoimmune encephalitis was first described in 2010 and is today the most common type of limbic encephalitis. During the course of the disease, 60%-88% of the patients develop hyponatremia. The etiology of the sodium disorder is unclear, often presumed to be the result of the syndrome of inappropriate antidiuretic hormone secretion (SIADH). Other electrolyte abnormalities have not been reported in association with anti-LGI1 antibody encephalitis. Due to the presence of hypomagnesemia and hypophosphatemia in our patien…

BACKGROUND: Antibody‑mediated encephalitis due to leucine‑rich glioma‑inactivated 1 (LGI1) is a frequent cause of immune‑responsive paroxysmal motor phenomena in older adults. The disorder is commonly associated with brief, stereotyped involuntary movements traditionally labeled faciobrachial dystonic seizures (FBDS). However, the "faciobrachial" descriptor may underrepresent the true phenotypic spectrum. We report a case of LGI1‑associated autoimmune encephalitis presenting with leg‑predominant tonic-dystonic attacks, expanding the recognized clinical presentation and highlighting variability

IMPORTANCE: Powassan virus is a rare but increasingly recognized cause of severe neurological disease.
OBJECTIVE: To highlight the diagnost … to prodromal orchiepididymitis. Low-titer anti-GAD65 antibodies identified in serum, suggestive of limbic encephalitis, were not detected in cerebrospinal fluid.
CONCLUSIONS AND RELEVANCE: Owing to the rarity of Powassan encephalitis, a high degree of suspicion is required to make the diagnosis, particularly in an immunocompromised patient, in whom antibody-based a …

BACKGROUND: Herpes simplex virus (HSV) encephalitis is the most common sporadic viral encephalitis, frequently complicated by delirium. Intracranial pressure (ICP) elevation is a common feature, but its role in delirium via brain functional network disruption remains unclear. This study investigated whether surrogate marker of ICP burden (LP opening pressure) mediates delirium in HSV encephalitis by reducing degree centrality (DC), a measure of functional network hub integrity, in key brain regions.
METHODS: In a prospective two-center cohort study (December 2023-November 2025), 55 HSV enceph

Glutamic acid decarboxylase (GAD) is an intracellular enzyme whose physiologic function is the decarboxylation of glutamate to gamma-aminobutyric acid (GABA), the main inhibitory neurotransmitter within the central nervous system. GAD antibodies (Ab) have been associated with multiple neurological syndromes, including stiff-person syndrome, cerebellar ataxia, and limbic encephalitis, which are all considered to result from reduced GABAergic transmission. The pathogenic role of GAD Ab is still debated, and some evidence suggests that GAD autoimmunity might primarily be cell-mediated. Diagnosis …

Autoantibodies against contactin-associated protein 2 (Caspr2) not only induce limbic autoimmune encephalitis but are also associated with pain conditions. Here, we analyzed clinical data on pain in a large cohort of patients included into the German Network for Research in Autoimmune Encephalitis. Out of 102 patients in our cohort, pain was a frequent symptom (36% of all patients), often severe (63.6% of the patients with pain) and/or even the major symptom (55.6% of the patients with pain). Pain phenotypes differed between patients. Cluster analysis revealed two major phenotypes including mo

PURPOSE OF REVIEW: Autoimmune-mediated inflammatory brain diseases represent a rapidly evolving area of medicine. The expanding spectrum of disease challenges providers as they strive to accurately diagnosis and treat children presenting with new onset neuropsychiatric deficits. This review examines recently published studies on primary central nervous system vasculitis and autoimmune encephalitis and utilizes key distinguishing features to guide a diagnostic approach.
RECENT FINDINGS: The spectrum of inflammatory brain disease continues to expand with the recognition of variable disease phen

BACKGROUND AND PURPOSE: Faciobrachial dystonic seizures (FBDS) and hyponatremia are the distinct clinical features of autoimmune encephalitis (AE) caused by antibodies against leucine-rich glioma-inactivated 1 (LGI1). The present study aims to explore the pathophysiological patterns and neural mechanisms underlying these symptoms.
METHODS: We included 30 patients with anti-LGI1 AE and 30 controls from a retrospective observational cohort. Whole-brain metabolic pattern analysis was performed to assess the pathological network of anti-LGI1 AE, as well as the symptom networks associated with FBD

CASE SERIES SUMMARY: Seven cases of feline hippocampal and piriform lobe necrosis (FHN) are described, with particular emphasis on clinical, radiographic and histopathological correlations. FHN is an uncommon acute epileptic condition resembling human autoimmune limbic encephalitis and temporal lobe epilepsy. Seizures are typically focal and feature uni- or bilateral orofacial or head twitching, hypersalivation, lip smacking, mydriasis, vocalisation and motionless staring, with inter-ictal behavioural changes such as unprovoked aggression and rapid running. Emerging evidence supports an autoim…

INTRODUCTION: The association between memory loss and Hodgkin's lymphoma has been given the eponym of Ophelia syndrome, in memory of Shakespeare's character in The Tragedy of Hamlet, Prince of Denmark. Nevertheless, there are differences between the disease and the character.
OBJECTIVE: To review the origins and uses of the eponym through an original article by pathologist Ian Carr, its relation to the character Ophelia, and the related autoantibodies.
METHODS: Historical narrative review.
RESULTS: Besides an eloquent description in the original article, Carr presaged the presence of autoan
