BACKGROUND: ART in the developing world has moved to a new era with the WHO recommendation to test and immediately treat HIV-positive individuals. A high frequency of pretreatment HIV drug resistance (PDR) can compromise ART efficacy. Our study presents updated estimates of PDR in seven countries from West Africa (Burkina Faso, Cameroon, Côte d'Ivoire, Mali and Togo) and Southeast Asia (Thailand and Vietnam).

Both vitamin A deficiency and trachoma are important causes of preventable ocular diseases. We examined 10 559 children in Mali for clinical signs of trachoma and of xerophthalmia. In five regions, the mean prevalence of trachoma was 39.3% (95% CI: 37.4-41.2). Clinical xerophthalmia was present among 1.89% of children (1.59-2.25). The diagnosis of xerophthalmia appeared to be associated with active trachoma (OR=2.04 [1.52-2.74]). This association remained after controlling for socio-economic status.

The results of food consumption surveys, epidemiological surveys (clinical and biochemical) undertaken in three sub-sahelian countries have established: - the existence of xerophthalmia in Upper Volta - a transitory vitamin A deficiency during the dry season in South Mali, without serious clinical signs - a good vitamin A status in Casamance, in South Senegal. A combined biochemical indicator which takes account of plasma carotenoids and vitamin A is recommended to assess the importance of vitamin A deficiency in a population.

Early failures to stavudine/lamivudine/nevirapine used as a generic fixed-dose combination in Mali showed resistance mutations in 50% of cases (mostly M184V and Y181C). No thymidine analogue mutations were seen, suggesting that most nucleoside reverse transcriptase inhibitors could be used in a second-line regimen. This highlights the importance of the accessibility of HIV-RNA assays for monitoring treated patients in resource-poor countries to detect early virological failure in order to preserve future therapeutic options.

A highly enantioselective 1,3-dipolar cycloaddition of ethoxyformylmethylene oxindole with iminoesters has been achieved using the Cu(I)-(S,Sp)-Ph Phosferrox catalytic system, generating a series of chiral spiro[pyrrolidin-3,3'-oxindole] compounds with four consecutive stereocenters, including a spirocycle quaternary center (71%-99% yield, up to >20:1 dr and 95:5 er). The compounds exhibited good inhibitory activity against Valsa mali (V.m.), Fusarium oxysporium (F.o.), and Alternaria brassicae (A.b.).

We report about three unliked cases of meningococcal meningitis caused by the ST-11/ET-37 strain of Neisseria meningitidis serogroup W. Two of the three cases, detected in Sicily on June and July 2014, were migrants from Mali and Eritrea. The third case was a fatal meningitis occurred on November 2014 in a 37 years old man, working in an immigrant center in Calabria. This report suggests that tetravalent conjugate vaccines (ACYW) should be actively offered to the staff of migrants' reception centers.
