Systemic and organizational barriers to primary chronic disease prevention: a qualitative study of public health organizations in Canada.

INTRODUCTION: Public health organizations in Canada play a central role in chronic disease prevention (CDP) but face persistent challenges, including system restructuring, persistent underfunding and shifting policy priorities. The growing complexity of these issues warrants qualitative insight to complement quantitative reports capturing CDP organizations' perspectives. METHODS: The Public Health Organizational Capacity Study (PHORCAST) is a repeat Canada-wide census of public health organizations engaged in primary CDP at national, provincial, territorial and regional population levels. The spectrum of injuries resulting from posterior abdominal stab wounds: a South African experience.

INTRODUCTION: The spectrum of injury associated with anterior abdominal stab wounds (SWs) is well established. The literature on the spectrum of organ injury associated with SWs to the posterior abdomen, however, is limited. METHODS: We reviewed our experience of 105 consecutive patients who had established indications for laparotomy managed over a 4-year period in a high volume trauma service in South Africa. RESULTS: Of the 105 patients, 97 (92%) were male and the overall mean age was 24 years. Fifty-seven patients (54%) had immediate indications for laparotomy. The remaining 48 patients (

Disparities in pain management among patients with life-limiting illnesses in Latin America: a scoping review.

INTRODUCTION: Patients with life limiting illnesses frequently experience moderate to severe pain, and many do not receive adequate analgesic control. In Latin America, palliative care coverage remains limited and access to essential opioids is often restricted, contributing to important inequities in pain management. OBJECTIVE: To map and characterize the available evidence on disparities in pain management among patients receiving palliative care in Latin America, with particular attention to structural, cultural, and educational determinants associated with unequal access and treatment. Barriers and Delays to Healthcare at Time of Death: Qualitative Analysis of Los Angeles County Death Records of People Experiencing Homelessness.

BACKGROUND: Human health and homelessness are incompatible with one another. People experiencing homelessness (PEH) experience extreme health and social inequities, including a significantly higher mortality rate and lower life expectancy compared to the general adult population. While many studies have attempted to identify the most common causes of death, no study to our knowledge has sought to contextualize these deaths using death records. The objective of this study was to conduct a qualitative analysis of the Los Angeles County medical examiner records of people experiencing homelessness

Silver linings: Observed reductions in aggression and use of restraints and seclusion in psychiatric inpatient care during COVID-19.

WHAT IS KNOWN ABOUT THE SUBJECT?: In a survey conducted by the World Health Organization (WHO) in the summer of 2020, 93% of countries worldwide acknowledged negative impacts on their mental health services. Previous research during the H1N1 pandemic in 2009 established an increase of patient aggression in psychiatric facilities. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: Despite expected worsening of mental health, our hospital observed reductions in aggressive behaviour among inpatients and subsequent use of coercive interventions by staff in the months following Covid-19 pandemic restricti

Innovations in plans of safe care implementation for infants with prenatal substance exposure.

INTRODUCTION: Federal legislation requires states to notify child welfare agencies of all newborns affected by prenatal substance exposure and to develop a plan of safe care (POSC). However, there is substantial variation in how states implement POSC and no best-practice standards. This study explored innovative state practices to understand how POSC policies can best support affected children and families across policy, health system, and child welfare contexts. METHODS: We conducted qualitative interviews with national experts on perinatal substance use policy, as well as state officials an

HIV epidemiology and responses among men who have sex with men and transgender individuals in China: a scoping review.

BACKGROUND: Despite global efforts to control HIV among key populations, new infections among men who have sex with men (MSM) and transgender (TG) individuals are still increasing. The increasing HIV epidemic among MSM/TG in China indicates that more effective services are urgently needed. However, policymakers and program managers must have a clear understanding of MSM/TG sexual health in China to improve service delivery. To meet this need, we undertook a scoping review to summarize HIV epidemiology and responses among MSM and TG individuals in China. METHODS: We searched MEDLINE, EMBASE an

Primary health care services for single homeless people: defects and opportunities.

BACKGROUND: An innovative residential centre in west London during 1997-1998 helped older rough sleepers leave the streets and resettle in conventional homes. Many clients presented with multiple physical illnesses complicated by chronicity and poor management. The centre initially experienced difficulties in obtaining health care for the residents, briefly relied on an A&E department for treatment of serious and minor ailments, and latterly was served by a GP practice supported by special funding. OBJECTIVE: The aims of this study were to describe the problems of providing at short notice pr

Developing strategies for integrating Hepatitis B Virus infection testing and treatment into routine PMTCT services in Ghana's Upper West Region: a rapid qualitative study applying the CFIR-ERIC matching tool.

INTRODUCTION: Despite Ghana's launch of the National Viral Hepatitis Control Programme in 2011, a comprehensive strategy for controlling hepatitis B Virus (HBV) infection among pregnant women, as envisioned by the World Health Organization's triple elimination goals for the vertical transmission of HIV infection, syphilis and HBV infection, remains undefined. This study aimed to explore pathways for integrating HBV infection testing and treatment into routine prevention of mother-to-child transmission (PMTCT) services in Ghana's Upper West Region. In doing so, it was guided by the Consolidated

Decoding the haemodynamics in sepsis to avoid drowning in salt water.

When a patient is described as 'septic', a clinical cascade occurs to simultaneously attempt to diagnose, risk-stratify, and treat a patient with a presumed infection. Elements of this clinical cascade include laboratory investigations, medication administration, and often specialty service consultations. The clinical consequences of each of these interventions have largely been independently investigated, and their impacts have varied. The primary focus of the management of septic shock has traditionally revolved around resuscitative efforts intended to rapidly restore perfusion to the organs

Rheumatic fever and rheumatic heart disease: Facts and research progress in Africa.

In recent years, the devastating effect of rheumatic fever (RF) and rheumatic heart disease (RHD) in Africa has been acknowledged by Institutions such as the Pan-African Society of Cardiology, the African Union Commission, and the World Health Organization. Key priorities set to eradicate RF and RHD include diagnosing and managing RF and RHD, building registries, improving adequate supplies of benzathine penicillin, reproductive health services, and cardiac surgery, developing multi-sectoral RHD awareness programmes, understanding RHD pathogenesis and fostering international partnership for re

A Clinician's Guide to Co-occurring ADHD among Adolescent Substance Users: Comorbidity, Neurodevelopmental Risk, and Evidence-based Treatment Options.

This article introduces neurodevelopmental and clinical considerations for treating adolescents with co-occurring Attention-Deficit/Hyperactivity Disorder (ADHD) and substance use (ASU) in outpatient settings. We first describe neurobiological impairments common to ADHD and ASU, including comorbidity with conduct disorder, that evoke a profile of multiplicative developmental risk. We then present two evidence-based options for targeting ADHD-related problems during ASU treatment. Medication integration interventions utilize family ADHD psychoeducation to prompt decision-making about ADHD medic

Extradural haematoma: trends in outcome over 35 years.

We have reviewed 35 years experience of extradural haemorrhage (EDH) in a large neurosurgical unit, based in two university hospitals, one dealing exclusively with children and the other a general hospital. A steady reduction in the mortality rate from 29 to 8.5% occurred during that period. A trend towards earlier diagnosis is noted and an increasing proportion of rural patients has been evident throughout the study period. During the time-period studied there were many significant developments: the establishment of a modern neurosurgical unit, the evolution of an intensive care unit, the ava
